According to our records, you are currently in violation of The Ohio State University's Housing Regulation. As a 21115 or
high school graduate who is enrolled full time at the Columbus Campus, you are required to liye on campus for two
academic years. The only exemption to the live on requirement is commuting from a parent or close relatives address
that is within a 25 mile radius of campus- If you are currently commuting from a parent or close relative?s address,
please complete the Parent?Close Relative Exemption request form and submit it to the Housing Services D?ice
by October 15th.

Parent?Elose Relative Exemption Form: 

Students who are not compliant with the Housing Regulation will be assessed a Housing Rate 3 {$3,249} on their
statement of account for every semester they are in violation of the live?on requirement.
