Contracts Awarded to Strategic Healthcare Solutions, LLC

Mr. Nannis is the owner of Strategic Healthcare Solutions, LLC. Since 2009, Mr.
Nannis?s company has held various contracts with the Health Department to provide
consultation services. The total value of the contracts awarded by the Health
Department to Mr. Nannis?s company from 2004-2018 is $514,863.

The table on page 2 summarizes the contracts awarded to Mr. Nannis?s company from
2004-2018, including the value of the contract, the hourly rate (if specified in the
contract), and the services provided.

Payments to Mr. Nannis and Strategic Healthcare Solutions, LLC

The attached spreadsheet shows the actual payments made by the City to Mr. Nannis
and his company, Strategic Healthcare Solutions, LLC. Payments begin in 2008 and
continue through 2016, totaling $558,788 (including $4,988 in reimbursements for
expenses, primarily travel-related). Payments under columns that start with are
grant-funded,

Lead-Related Services

None of the contracts identified by the Department of Administration Purchasing

Division as having been awarded to Mr. Nannis or his company require services on
lead?related issues or include any reference to lead?related programs.
