RISEMAN License No. 36-076506, Respondent ORDER This matter having come before the Medical Disciplinary Board of the Department of Professional Regulation of the State of Illinois, and the Medical Disciplinary Board having approved a Stipulation and Recommendation for Settlement submitted by'the parties, in resolution of alleged violations of the Medical? Practice Act, 225 ILCS 60/22 (A), paragraph (4) (gross negligence in practice under the Medical Practice Act), paragraph (5) (dishonorable, unethical or unprofessional conduct of a character likely to deceive, defraud or harm the public) and (26) (a pattern or practice or other behavior which demonstrates incapacity or incompetence to practice under the Medical Practice Act with reasonable judgment, skill or safety) in the diagnosis of, preparation of, and performance of surgery on pediatric patients, and failure to properly perform a biopsy on an adult patient, as alleged in the formal complaints consolidated under the above?captioned case number, filed originally on May 31, 2001, and amended in the formal complaint filed with the petition for temporary suspension filed on March 15, 2002; NOW, THEREFORE, I, LEONARD A. SHERMAN, DIRECTOR OF THE DEPARTMENT OF PROFESSIONAL REGULATION of the State Of Illinois, Page 1 of 3 ?7do hereby adopt the Stipulation and Recommendation for Settlement approved by the Medical Disciplinary Board in this matter. IT IS THEREFORE ORDERED that the Temporary Suspension of the Physician and Surgeon license of Jay Riseman, License No. 036? 076506, is hereby terminated, and that said License is hereby placed on Probation indefinitely for not less than five (5) years in accordance with the Stipulation and Recommendation for Settlement, which is attached hereto and incorporated herein. IT IS FURTHER ORDERED: A. That Respondent shall be required to complete at least forty (40) hours of Category I Continuing Medical? Education in the area of adult and pediatric trauma surgical procedures, in addition to his yearly statutory requirement, for each year Respondent?s license is on Probation in accordance with the Stipulation and Recommendation for Settlement which is attached hereto and incorporated herein. B. Respondent?s practice as a Physician and Surgeon shall be restricted in accordance with the restrictions, terms and conditions as set forth in the Stipulation and Recommendation for Settlement which is attached hereto and incorporated herein. C. Respondent shall be required to have quarterly reports submitted in accordance with the terms and provisions as set forth in the Stipulation and Recommendation for Settlement, which is attached hereto and incorporated herein.
