DATING AND RELATIONSHIP AGREEMENT AND ACKNOWLEDGEMENT OF HARASSMENT-FREE WORKPLACE POLICY The undersigned, [FEMALE] (“[FEMALE]”), is an employee of [COMPANY] (the “Company”). The undersigned, [MALE] (“[MALE]”), is an officer and shareholder of the Company. [FEMALE] and [MALE] have entered into a romantic relationship. Both [FEMALE] and [MALE] understand and acknowledge that neither party wants their relationship with each other to affect their jobs or the Company in any way. It is the policy of [COMPANY] to provide its employees with an equal opportunity in hiring, employment, promotion, compensation and all other employment-related decisions without regard to race, color, age, religion, sex, marital status, national origin, citizenship, veteran status, disability, or any other basis set forth in the applicable laws or regulations relating to discrimination in employment. It is also the Company’s policy to create and maintain a workplace free from harassment, including sexual harassment. This Dating and Relationship Agreement is presented to the parties for signature to document the consensual nature of the relationship, address supervisor-subordinate reporting relationships and provide guidance for appropriate conduct in the workplace. This Agreement is also intended to promote the efficient operation of the Company’s business and avoid any misunderstandings, complaints of favoritism or other problems of security, supervision, safety or morale and possible claims of sexual harassment. We, the undersigned parties, [FEMALE] and [MALE] agree as follows: 1. We understand that [COMPANY] prohibits harassment in the workplace, and have had the opportunity to ask questions about our individual situations within the Company. 2. We desire to engage in a personal relationship. Our personal relationship is entirely voluntary on both of our parts. 3. We agree we will make every effort not to allow our personal relationship to impact our work, and agree that if despite our best efforts, the Company concludes our personal relationship has a negative impact on our work, the Company may address such negative impact as the Company deems appropriate, including but not limited to discipline and/or discharge of either or both of us. 4. All personal aspects of our relationship shall remain outside the workplace and work related functions. 5. We will act professionally towards each other at all times, even if the relationship ends. 6. [MALE] will not directly supervise [FEMALE] at any time during the relationship or after the relationship, if the relationship ends while we are both employed with the Company. 7. [MALE] agrees to recuse himself from any discussion related to the terms of [FEMALE]’s employment. He will not directly or indirectly influence, either positively or negatively any decision related to [FEMALE]’s employment, including but not limited to assignments of customers and/or projects, evaluations, discipline or discharge, compensation, schedule, advancement, promotion, and development. 8. [MALE] agrees not to share with [FEMALE] any Company confidential information that [FEMALE] would not otherwise have access to. 9. We will inform the Company’s Human Resources representative immediately if the relationship ends, or if the conduct of the other person is no longer welcome, provided that both [MALE] and [FEMALE] are still employed with the Company. 10.
