Dear Ms. Erica Jackson:

Enclosed is a copy of an ?Af?davit of Complaint? executed by the Of?ce of the Texas Secretary
of State, alleging that you have violated the duties and responsibilities of your of?ce as a Texas
Notary Public. The complaint alleges that you failed to complete a notarial certi?cate at the time
you attached your notarial seal to a document. 

Attaching your seal to a document without attaching a notarial certi?cate constitutes good cause
for the secretary of state to take action against your notary commission. 1 Tex. Admin. Code 
The lack of notarial certi?cate makes it unclear the type of notarial act
performed, for whom the notarization was performed, the date of the notarization and the
location of the notarization. When you respond to the complaint, please include this information.

You must respond to the complaint in writing using the attached Af?davit of Notary Named in
Complaint form and the complaint must be received by this Of?ce within 20 days from the date
this letter is mailed. Tex. Admin. Code ?87.24(c) (2016). Your response must be typewritten
or printed on the enclosed af?davit and veri?ed before an of?cer authorized to administer oaths.

In addition to the information requested in paragraph two above, your response should include
the following:
Any disputed facts;

0 Your current mailing address;
0 Copies of the pages of your notary record book referencing the notarization(s) at issue;
Copies of the pages of your notary record book showing the last 10 notarizations

performed by you; and

- Any additional information relevant to the complaint.
