When deputies and medical pe1sonnel arrived, they found an unresponsive adult male 1n the elevator. First responders attempted to provide lifesaving CPR, but were unable to revive the victim. He was pronounced deceased at 10:07 am. He has been identi?ed as Prince Rogers Nelson (57) of Chanhassen. An adult male was found unconscious and not breathing inside of his residence. Life-saving measures were attempted and the male was pronounced dead at the scene. DETAILS: 1. On 04f21f2016 at 0945 hours, Deputies were dispatched to a medical call at 7801 Audubon Rd. (Paisley Park). Dispatch advised a male was on the ground and was .not breathing. 2. Deputy Horvath and. I arrived on scene at 0948 hours along with Chanhassen Fire Personnel and Ridgeview "i Paramedics. Emergency Responders were met at the front door of the residence and esgorted to where the male was located. The male was identi?ed as Prince Rogers Nelson N. Nelson was located inside of the elevator that was loCated down the main entrance hallway on the right side. Chanhassen Fire Chief Johnson removed Nelson out of the elevator. Nelson was lying on the ?oor on his back with his feet near the door for the elevator. I could tell Nelson was unresponsive and not responding to any stimulation. I noticed that Nelson?s right arm appeared to be stiff, as it remained in the same position as he was moved on the ?oor. 4. Chief Johnson checked for a pulse and stated that he could not feel a pulse. Chief Johnson stated due to Nelson being warm to the touch that CPR was going to be started. Chief Johnson began chest compressions and was handed the Automated External De?brillator (AED) by Deputy Horvath. As I was removing the pads and activating the AED, Ridgeview Palamedics entered. Ridgeview Paramedics stated that since there were obvious signs of rigor mortis, lifesaving efforts could be stopped. Ridgeview Paramedics pronounced the time Of death to be 1007 hours. 5. I then made contact With Sgt. Breunig, who had arrived on scene. Sgt. Breunig advised he would be making phone contact with investigations and the Midwest Medical Examiner?s Of?ce. 6. At this time, I left the area to obtain statements from the three individuals that were on scene at the time. The three individuals were identi?ed as Kirk Anthony Johnson Meron Bekure (DOB: and Andrew Bateson Kornfeld (DOB: . 7 Deputy Horvath obtained a recorded statement from Johnson and I made contact with Bekure. I asked Bekure if she would be willing to give me a recorded statement in regards to what had happened today. Bekure stated she would. Bekure stated Johnson, Kornfeld and she arrived at Paisley Park around 0900 hours on 04f21i2016 to check on Nelson. Bekure stated that she had not heard from Nelson and he was not responding to email?s and text messages. Bekure stated the last contact anyone had with Nelson was at around 2000 hours on 04/20f2016. Bekure stated the last time she saw Nelson was at 1630 hours. Bekure stated she arrived at Paisley Park on 04K20f2016 to go with Nelson to a doctor?s appointment that was scheduled. Bekure stated Nelson told her that she was not needed and she left. Bekure stated the reason Nelson was going to the doctor was because he was not feeling well. It was at this point that One of the paramedics came and explained that they were leaving and discussed viewing Nelson to everyone in the-room. Bekure again became emotional and was hard to understand while speaking with her. Bekure stated she arrived along with Johnson and Kornfeld at around 0900 hours to check on Nelson. Bekure stated Nelson was found in the elevator while they were looking for him. Bekure stated she did not know of any medical problems that Nelson had and had no known medical history. I explained how the rest of the process would work with Bekure and asked if there was anything else she wanted to add to the statement. Bekure stated she did not have anything further to add and I ended the recording. I then made centact with Kornfeld. I asked Kornfeld if he would be willing to talk to me about what he knew about the incident. Kornfeld 'stated he would. I activated my digital recorder again. The following is a summary of the recording with Kornfeld. I identi?ed Kornfeld and expressed my condolences to Kornfeld. I asked Kornfeld how he knew Nelson. Kornfeld stated that he did not know Nelson. Kornfeld stated he was brought in to help with Nelson?s medical issues and some options for getting assistance. Kornfeld stated his father, who was identi?ed as Howard Kornfeld (No Further Information) had a clinic in California. Kornfeld stated his father?s clinic was asked on 04f20f2016 to come and meet with Nelson. Kornfeld stated he took a red plane to Minnesota from California during the late night of 04f20-04/21. Kornfeld stated he arrived in Minnesota and got a hotel room in Chanhassen. Kornfeld stated he met with Johnson and Bekure during the morning hours of 04/21/2016. While he was speaking with Johnson and Bekure, they mentioned that it was very normal to not check. on Nelson for a while. Kornfeld stated he advised that they should attempt to make contact with Nelson. Kornfeld stated they arrived at Paisley Park at about 0915 hours. Kornfeld stated he was in another room at the residence, while Johnson and Bekure went to go check on Nelson. Kornfeld stated he heard a scream and came into the area of the elevator. Kornfeld stated he called 911 and checked Nelson. Kornfeld stated he decided'not to start CPR as he noted signs of rigor mortis. I asked Kornfeld if he knew any medical conditions or history of Nelson. Kornfeld stated he did not know any further information about Nelson. Kornfeld stated he was just there to explain the medical of?ce?s program and if Nelson wanted to go with him, they would return to California. I advised Kornfeld on what would be happening next and ended the recording. Both of the recordings were later downloaded to a CD at the Chanhassen District Of?ce and secured into evidence. After obtaining the statements, I assisted with scene security. It was also at this time that Carver County Investigators arrived on scene. I was then asked to go with a Carver County Investigator Nucci and attempt to complete the death noti?cation ith Nelsons sister in Minneapolis. Nelson?s sister was identi?ed as Tyka Evene Nelson (DOE-1631 cleared the scene and had begun to drive to Minneapolis. I was soon contacted by phone that Tyka had been noti?ed and I could return to Chanhassen. When I returned to the scene, I assisted with traf?cfcrowd control around Paisley Park and did not go back into the residence. 1 also provided Deputy Horvath a break from approXirnately 1125 hours until 1200 hours. Deputy Horvath was I responsible for the crime scene log at the main gate into Paisley Park. On?04/212?l6 at 0943 hrs, Deputies Horvath and Hastings along with a Ridgeview ambulance and Chanhassen Fire Rescue were dispatched to Paisley Park on a report of male down who was not breathing. Paisley Park is located at 7801- Audubon Rd. in the City of Chanhassen. I (Sgt. Breunig) responded also to assist. 2. Deputy Horvath, Deputy Hastings, Chanhassen CSO Sanchez, members of the Chanhassen Fire I Department, and two Ridgeview paramedics arrived on scene before I did. When I arrived, I entered the building through the main entrance on the north side. I noted three adults in the lobby on the ?rst ?oor. I continued to an elevator that?s to the right, just past the lobby. On the ?oor outside of the elevator, I observed an adult male lying on his side on top of a crumpled rug. The male was dressed in pants, socks (no shoes), and some type of shirt. The male was later identi?ed as Prince Rogers Nelson (DOB- . I Spoke to Chanhassen Fire Chief Johnson (Chief 1) who was one of the ?rst responders to arrive. Chief Johnson told me Nelson was still in the elevator when he arrived. Chief Johnson said he could not detect a pulse on Nelson, but believed he was still warm to the touch. Chief Johnson said he then drug Nelson out of the elevator and initiated CPR. 3. I then spoke to one of the paramedics on the scene. The paramedic told me that he observed signs of rigor mortis on Nelson 3 body which indicated he had not died recently. The paramedic said they used a heart monitor to check for indications of a heart the reading was asystole (?at line). The paramedic then called ?med control? at the Ridgeview Medical Center to consult with emergency room staff. It was after that When the paramedics advised that Nelson was pronounced deceased at 1007 hrs. The paramedics spoke to the three people in the lobby and then cleared from the scene. 4. Chief Johnson wanted to rule out the possibility of there being dangerous levels of carbon dioxide in the building. Chief Jonson said one of his ?re?ghters had used a monitor and advised that carbon dioxide levels in the building were safe. Chief Johnson wanted to check the carbon dioxide levels in the parking garage. I had Deputy Horvath accompany Chief Johnson to the parking garage. They returned a few minutes later and Chief Johnson advised the carbon dioxide levels in the parking garage were safe. All members of the Chanhassen Fire Department then cleared from the scene. 5. I stayed near the body to protect the scene. I instructed Deputies Horvath and Hastings to take initial statements from the people in the lobby and to record them. I then contacted my immediate supervisor, Lt. Kittelson. I advised Lt. Kittelson of the situation and requested that .he respond to the scene. I then contacted Detective. Sgt. Meier and advised him of the situation. Detective Sgt. Meir said he would be responding with a team of detectives. I then requested that Dispatch advise the Medical Examiner. I received a phone call a short time later from a Medical Examiner Investigator. She said she would be responding to the scene after consulting with the pathologist. Meier arrived on scene, he asked that I join him and other law enforcement of?cers in doing a protective sweep of the property. The purpose of this is to ?nd out if anyone else is on the property and to ensure that the scene is safe. An adult male by the name of Kirk accompanied us during the protective sweep. Kirk is a high level staffer who was close to Nelson and had keys for the property. 7. While we were on the second ?oor of the building during the protective sweep, we noticed a black colored glove and a black colored garment, which appeared to be a jacket, lying in the hallway._Ki_rk was asked if that seemed normal. Kirk replied it was not normal and for that reason, he didn?t touch it. I did not observe- anyone or anything suspicious during the rest of the protective sweep of the interior of the building. Detective Sgt. Meir and I then did. a protective sweep of thearea around the outside of the building. I did not observe anything suspicious. 8. During the remainder of my time on the scene, I assisted with interior scene security, assisted in guiding Hennepin County Sheriff?s Of?ce crime scene deputies to various parts of the building, and I left the scene twice for periods of time to get supplies needed at the scene. On 04/21/2016 at 0945 hours, I was dispatched to a medical at 7801 Audubon Road in the city of Chanhassen (Paisley Park). Dispatch advised it was reported an adult male was down and believed to not be breathing. 2. Deputy Hastings and I arrived on scene at 0948 hours. along with Chanhassen Fire Chief Johnson and Ridgeview Paramedics. I entered the front door of Paisley Park with my medical bag and was directed to where the adult male was down. I observed there were three people inside the main entry door/lobby area i as I entered. 3. I walked down the hallway from the front door and to my right I observed a male party down on the ?oor, directly in front of the elevator. I later learned the male party was actually inside the elevator and was pulled out by Chanhassen Fire Chief Johnson. The male party was later identi?ed as Prince Rogers Nelson DOB 4. I observed Prince was lying on the ?oor on his back directly in front of the elevator. Chanhassen Fire Chief Johnson advised that Prince was still warm and began resuscitation efforts. I Observed Chief Johnson begin chest compressions so I got my AED ready from my medical bag. I handed my AED and pads to Deputy-Hastings as he was closer to Prince than I was. Additional Chanhassen rescue personnel arrived on scene at this time, so I allowed them to take over my spot allowing me to speak to the three people in the front lobby area. I 5. The three people in the front lobby area where identi?ed as Andrew Bateson Kornfeld DOB Kirk'Anthony Johnson and Meron NMN Bekure DOB -. I proceeded to activate my digital recorder and spoke tO Kirk about the incident. I was informed that Kirk is an employee of Prince?s and handles most of his daily affairs. The following is a summary of our conversation: 6. Kirk told me about recently having to make an airplane perform an emergency landing due to Prince blacking out in ?ight. Kirk said Prince was rushed to the local hospital but refused to submit to a blood test while he was there to find out whatwas wrong. Kirk said he told Prince that he needed to'take him to the doctor then when they got home so they could get checked out. 7. Kirk said he took Prince to a doctor?s appointment at North Memorial Clinic at 1700 hours yesterday (04/20/2016). Kirk said he could tell Prince was going through withdrawal as he was driving him home from the doctor. Kirk believed the withdrawal were from prescription medications Revised 05x12 10. ll. 12. 13. 14. but did not know what pres01 1ptrons Prince was taking. Kirk also did not know how long Piince has been using prescription drugs but suspected it to be a long time. Kirk speci?cally told me that Prince refused to provide a blood test at the hospital because he did not want people to know about his use of prescription medications. Kirk told me the only thing Prince complained about on the ride back home from the doctor?s of?ce was that he was suffering from anxiety and dehydration. I then made contact with Michael Todd Schulenberg DOB at the scene. Schulenberg IS a dector at North Memorial Clinic and saw Prince the p1ior day. I asked Schulenberg if someone from here requested his presence and he said no. Schulenberg said it was a ?complete coincidence? and that he had showed up at Paisley Park to meet with Prince to discuss substance abuse treatment. - Schulenberg told me he has met with Prince twice at the North Memorial Clinic. Schulenberg reported the ?rst time he met with Prince was this Visit, Schulenberg met Prince at Paisley Park and then transported him to the clinic. Schulenberg told me Prince?s chief complaint on that day was that he generally was not feeling well and being nauseous. Schulenberg said Prince also complained of ninnbness/tingling in one of his legs and both of his hands, Schulenberg said on his Visit yesterday with Prince the main complaint reported to him was that he was ?antsy.? I then was assigned to the north gate entrance on Audubon Road and instructed to start a crime scene log. I kept a crime scene. log beginning at 1027 hours until 1815 hours. Note: I was relieved by Deputy Hastings from 1229 hours until 1250 hours. -I also was relieved by Deputy McKirahan from 1422 hours until 1501 hours. Detective P. Murphy was looking for surveillance video of the hotel area from 04/20/2016 at 2100 hours until 1000 hours on 04/21/2016. 2. I arrived at the hotel and spoke to the front desk manager, Valerie Cheryl Hicks who advised me the hotel does not have exterior or interior surveillance cameras. I then drove around the Country Inn and Suites looking for outside surveillance cameras pointing towards the south side of the hotel. 3. I located one exterior camera at the Americana Bank which is to the south west of the Country Inn and Suites. The camera does not point towards the back entry of the hotel. The camera quality is also very grainy making it hard to even see the back lot of the hotel, especially when zooming. in. I collected the thumb drive of the surveillance video and placed it into evidence locker #5 at the Chanhassen District Of?ce. 4. I then made phone contact with the Hicks at approximately 1400 hours. I inquired if she could provide me any information on when Andrew Kornfeld checked in and out of the hotel. Hicks told me Kornfeld checked into room #102 at 0845 hours on 04/21/2016. Hicks said Kornfeld was still listed as a guest of the hotel and had not checked out. Hicks told me she did not know if Kornfeld was at the hotel currently because she had not seen him all day nor did she know what he looked like. The purpose of this Second follOw up report is to previde additional and clarifying information to the ?rst follow upreport that I submitted on 04/23/16. 2. I put on a pair Of latex gloves when I arrived on the property when the incident was still a medical event. I removed my latex gloves after Nelson had been pronounced deceased as I needed to use my cell phone to make noti?cations. 3. When I was asked to join the team of law enforcement of?cers conducting the protective sweep of the property, I put on a second pair of latex gloves. Paisley Park is a large, diverse property and I did not enter every room of the property while assisting during the protective sweep. I did enter hallways, a residential suite of rooms, storage rooms, utility rooms, recording studios, public restrooms, event concert halls, a hedrOom not associated with the residential suite that Is upstairs from the event 2? concert halls, mixed use or Lmknomi use rooms, parking garage, and a tunnel located under the event concert halls. There were several raised platforms in the event concert halls which had decorative skirting made from cloth. I lifted the skirting on the platforms that I checked to see if anyone was underneath; I did not locate anyone. As stated in my previous report, I also asSisted in the protective sweep of the exterior grounds surrounding the main building. I did not collect any evidence during the protective sweep. 4. I was tasked with purchasing food and beverages for of?cers working at the scene since processing was likely to take a considerable amount of time. Commander Tschida accompanied me. According to the crime scene log, we left the scene at 1308 hrs. and returned at 1340 5. Lt. Kittelson asked that I retrieve his laptop from his of?ce for his use in the command post vehicle and that I update the Chanhassen City Manager as to the crowd and traf?c control concerns surrounding the incident. According to the crime scene log, I left the scene at 1400 hrs. and returned at 1517 hrs. 6. When I was back at the scene, I was asked to show a sergeant from the Hennepin County Sheriffs Of?ce Crime Scene Unit how to access the tunnel. After that, I also showed the HCSO sergeant how- to access the roof, the parking garage, seme storage rooms, and the bedroom that is located upstairs from the event Concert halls. Sgt. Breunig con?rmed it was Prince Rogers Nelson present. I advised the media that they could park on Drive and that they could stay on the On 04-21?16, at appro'ximately 0955 hours, Sgt. Breunig called me on my cellular phone. Sgt. Breunig said that he was at Paisley Park studios on a medical and that I needed to respond. I arrived at Paisley Parkstudios, located at 7801 Audubon Road Chanhassen, MN, at 1011 hours. I drove through the northwest gate and entered the main public entrance on the northeast corner of the building. I walked past the reception area and met with Sgt. Breunig. I saw what appeared to me to be Prince lying on the ?oor in front of the elevator on top of a rug. The body was fully clothed and lying supine and leaning to and that he was dead. I stayed within visual of the body while Sgt. Breunig briefed me on who was present and what Sheriff? 3 Of?ce employees were en route to the scene. Sheriff Olson arrived on scene with Detectives Nelson and Wagner at 1020 hours. Sgt. Meier arrived at 1023 hours. Sgt. Breuni briefed them on the situation. While I was standing near the body, there were two adult females and two adult males standing in the lobby. Also in the lobby was a gentleman I recognized as Kirk Johnson. I have communicated with Johnson in the past regarding concerts at Paisley Park for logistical purposes (parking, etc.) and he was someone I knew to be the event coordinator and responsible for the building and grounds of Paisley Park. Deputy Nucci arrived in the building at 1030 hours. Detective Murphy advised me he was escorting one of the adult males to another area of the building at 1034 hours. Detective Murphy, Detective Nucci and Detective Nelson left the building with all of the parties from the lobby at 1039 hours. I communicated with Chief Deputy Kamerud after 1100 hours to brief him of the situation and advised him that the media was gathering on? Audubon Road. We discussed what information would be released and a possible staging area for the media. I left the studio at 1117 hours and addressed the media that had gathered on Audubon road outside of the northwest gate. The media had numerous trucks, tents etc. I gave my business card to the media that was boulevard on the west Side of Audubon between Drive and MN Highway 5. I advised the media that was present that the Sheriffs Of?ce was conducting an investigation and that the crime lab and medical examiner were on the way. I also advised them to contact our of?ce in Chaska- for any further information. As I was walking away, one of the media stated something to the effect of ?the medical examiner and crime lab, so there has been a fatality?? I responded, ?Yes? and continued back in the building thr0ugh the northwest entrance at 1121 hours. I resrnned a position near the body. Medical Examiner Strobl and the Hennepin County Sheriff?s Of?ce (HCS 0) Crime Lab arrived inside at 1126 hours. Lt. Turpitt, from I-ICSO Crime Lab, had three deputies/sergeants with him. I maintained a position near the body while they were briefed. Revised 05/12 10. l. ICR 16012559 I maintained a pos1tlon near the body until 1345 hours. I left the building, but stayed within the grounds I took a break from 1345-1415 hours on the west side of the building 1n an area I commonly refer to as the employee parking lot. Detective Nelson had returned to the scene and took my place near the body at 1345 hours. From 1435 1442 hours I followed one of the HCSO Crime Lab deputies into an area near the front lobby. The area resembles a business center that would be 1n a hotel with several computers for guests and several small chairs. The HCSO deputy took pictures of the area and checked it. The HCSO Crime Lab deputies were documenting the scene with photographs and also documenting the collection of evidence. A Carver County Sheriff?s Of?ce deputy would follow or escort the HCSO while photographing. After the photographs in the ?computer room?, I followed HCSO Sgt. Magnuson while he photographed. Commander Tschida assisted. The route was clockwise around the building; south on the main ?oor past the of?ces on the east side of the building into the main performance area, up a set of stairs on the southwest corner of the building into a large room that had a bed and night stand in it. I saw on the which was to the left of the bed, an open bag of Halls cough drops and what appeared to be a tube of lipstick. I pointed this out to Sgt. Magnuson, but the items were not collected at that time. I then followed Sgt. Magnuson along the catwalk to the roof door. Sgt. Magnuson opened the door and looked out at the roof top. I gazed out the door as well, seeing nothing remarkable; we proceeded down the center of the upper level and arrived to where the CCSO investigators were primarily concentrating their search in an area that had another bed in it. The upstairs area is near the main entrance, so I proceeded down the stairs and met with Dr. Strobl. I assisted with preparations for transport of the body. I carried the body, which had now been bagged, in the elevator with Sheriff Olson, Sgt. Meier and Lt. Williams. We took the elevator downstairs and placed the body In the transport vehicle which had been parked in the underground garage. I went outside and briefed with staff on how to create 'a traf?c and pedestrian plan for the numerous peOple that had now gathered outside the fence. I cleared the scene and left in my vehicle at 1810 hours. 12. My times for this report were taken by verifying on my iPhone and then recording the times. I did not wear gloves while escorting Sgt. Magnuson, and I did not touch anything. The only time I touched anything was when I used the restroom south of the main lobby after it had been cleared by investigations and the crime lab. I returned to the front of the building and asked Deputy Wagner to interview Johnson. I sent Deputies Murphy, Nucci and Nelson to the Chanhassen District Office to do interviews with some elevator dOOr on the ground floor. This elevator is about forty feet south of the front door, on the west side On 04-21-2016 at about 0900 hours, I received a telephone call from Sergeant Breunig who said that he needed investigati assistance at 7801 Audubon Drive in Chanhassen. He said that the Prince Rodgers Nelson dwas deceased. He said that there were several people at the residence that needed to be interviewed. I asked him to start a 'crime scene log of people entering and leaving the crime scene. I looked for Sheriff Olson but was unable to ?nd him. I found ?Chief Deputy Kamerud and told him what I had learned. He located Sheriff Olson and we spoke for a minute. I requested that a crime lab assist us in processing the death and we settled on the I-Iennepin County Crime lab. I gathered the investigators that were available and we had a short brie?ng. I spoke to the Medical Examiner, Doctor Quinn Strobl by telephone, and explained that I was in need of her personal services at the death scene. She said she would be reSponding and would ?bring one other person from her office with her. . I drove to the residence which doubles as a recording studio for the musicalartist Prince. I arrived and parked my car in front of the studio. It was later moved to the west side. I brought a box of rubber gloves from my car and entered the front doors after putting on a pair of rubber gloves. I was met by Sergeant Breunig who briefed me. I asked about the interior and if it had been searched for subjects and was "told-it had not. - of the ?rst people to arrive and find Prince deceased that morning. Sergeant Breunig, Deputy Wagner and I took Kirk Anthony Johnson with us to open the doors and guide us through the search. Johnson was the chief of security and close associate of Prince. Johnson said that no one lived in the building except Prince. Johnson had knowledge of the studio and apartment we were searching. We searched the interior in areas that could holda subject and did not open anything that was not capable of concealing a human being. We did not locate anyone inside. There was one room in the basement that we were unable to access. Johnson said it was a vault that no one had been in and he did not know where the keys to the locked door were at. Several times during the search, and processing of the building, I tried keys that were found and was unable to open this door. The door is directly in front of the basement elevator when standing inside the elevator. The door is purple in color and has the Prince symbol on it. The door was never opened and the contents of that room remain unknown. I saw that Lieutenant Kittelson had appeared to have remained near the body of Prince during our search. I had observed Prince lying on the ?oor when I entered the building. The body was lying in front of the of the hallway. There is a stairway leading up to the second ?oor and the elevator is tucked under this stairway. This leaves a small covered waiting type area outside the elevator doors. This area is where the Revised 05(06 ICR: 2016012559 body of Prince laid. 10. I saw that Prince was lying on his back with his left arm extended, palm up. I saw he was wearing a' black knit type cap that was off his head, a black Under Armor brand zip up light jacket, black nylon type pants, blue color watch band on his right wrist, and black socks. I saw two white color medical patches on the left arm and right leg. I did not see any signs of a struggle or any obvious trauma on the body. I did not move or touch the body, only observed it. 1. I usually take snap shots with my issue cellular telephone at death scenes to preserve images as they are when I arrive. I did not do this due to'the sensitivity of this investigation. No one else took photos that I am aWare of except the Hennepin County Crime lab. I asked the investigative staff when doing an after action brief if they had any cell phone photos and no one did. 12. The body of Prince was lying on top of a multi colored rug. While searching the building I saw another rug exactly like this rug outside the elevator deer on the landing upstairs. 13. I was called by Deputy Murphy who said that in his interview with one of the subjects he found out that Prince may have been suffering from an addiction to pain medication. Deputy Murphy said one of the subjects he interviewed had been at the residence to try to enroll Prince in a recovery program and had poSsiny some pain medication in a backpack he had been carrying. Based on this information I asked Deputy Murphy to draft a search warrant for the building to look for evidence of pain medications and prescriptions. - 14. I saw that the Hennepin County Crime lab had arrived with three technicians (Sergeant Kevin Schwartz, Sergeant Shane MagnuSon, and Deputy Sarah Buck) and Lieutenant Todd Turpitt. I coordinated our response to searching the living quarters between the two agencies. I informed Sheriff Olson and the rest of the law enforcement that this was the plan and it would be about two hours- before we were ready to serve the search warrant. 15. I asked-if anyone had made a complete check of the exterior of the building and was told it had not been done. I walked one way around the building and Sergeant Breunig went the other. We met in the back and did not ?nd anything to note. . 16. I noticed that the temperature in entrance area we were in was very warm. I spoke to Johnson about the thermostats and he explained they'were located periodically on the east wall of the hallway. Johnson went with me to the ?rst one and showed me how to remove the cover and spin the dial under it to bring the temperatirre down. I asked Hennepin County Sergeant Magnuson to photograph the thermostats. I saw that they were set at 75 degrees and the temperature in the building was reading at 72 degrees. I turned all of the thermostats in the entrance hall way down to 60 degrees.- I asked one of the Hennepin County Deputies to turn off the space heater in the kitchen area to add to reducing the temperature. I checked later-and saw that the heater was off. - 17. I spoke with Dr. Strobel when she arrived and we made a plan for the removal of Prince to her Care after the scene had been processed. - 18. The next of kin arrived and asked to be taken to the remains of Prince. I explained that I would make sure they had a chance to be with Prince before he was removed but that the processing of the scene had to oCcur ?rst. 19. I waited in the entry with others while the search warrant was written and signed. See Deputies Nucci and Murphy?s reports. 20. I spoke to Deputy Murphy by telephone. He said that the assistant to Prince, Meron Vekure, had declined to giVe a statement and wanted to leave. I told him to give her a ride to her room at the County Suites Inn where she stayed and then bring her back to the studios. 21. Deputy Nelson called me and said that he had been waiting for Vekure to come back and she never ICR: 2016012559 22. 23. 24. 25. returned. He said he tried to call her and knocked on the door but she did not answer. I told him to return to assist us. When the warrant was signed I saw the Hennepin Crime lab process the area and the body of Prince. I asked that swabs of Prince?s hands and neck be taken to allow "the family to have contact with Prince. I told Dr. Strobel that we would be ready for the tranSport van to come and take Prince away. The interior of the elevator was constructed of a mostly silver color metal (possibly stainless steel). I saw on the south east wall of the elevator under the hand rail that there was a ?uid splashed on the wall. I made a mental note to have this area photographed and a swab taken of the substance on the wall. I received a telephone call from Deputy Murphy that the search warrant was signed and he was on his way to assist us. The investigators on scene began to search the building. I spoke to the family and explained that they would be allowed very soon to be with Prince. When I was ?nished I received a blanket from Lieutenant Kittelson and I covered all but the left hand of Prince at'the Sister?s request. The family. went to the body of Prince and eventually several asked to see his face. I . uncovered the face after some had the chance to turn away. When the family was done I escorted them 26. 27. 28. investigation. 29. 30. 31. 32. back outside. I helped the ME load Prince inside the body bag and watched the seal put on the zipper. I assisted in carrying the body bag containing Prince (four of us carried him) into the elevator and to the basement garage. I lifted the bag onto a stretcher and saw him secured in place. There had been a second transport van that had left'the building as a decoy prior to us bringing Prince to the garage. Cat from the Of?ce abcompanied Prince in. the transport van to their Of?ce. I rejoined the investigative staff that was searching the living area of the building. I-was called to a room on the north east side of the building by Deputy Wagner. I saw this room contained items that appeared to make this area some sort of a dressing room. Deputy Wagner pointed out a jewelry box that she had opened. She showed me a number of white pills that were without a prescription bottle. The inscriptions on the pills were read and Dr. Strobel used her cell phone to ?nd that they appear to be a controlled substance general prescribed for pain. I saw that the pills were being collected as evidence. I walked through the living area this time with an out for items that Could be important to this I went into a doorway that had a sign on the side asking that only authorized personnel were allowed. The room I entered appeared to be a private room with a bed on the north side of the room. There were couches and chairs in this room with an open suitcase of beauty products. I went into the next room. attaChed to this and which was Prince?s bedroom according to Johnson when I - had search for subjects earlier. I did not disturb anything as I looked around. I saw a bed on the south side of the room and the bedding was disturbed. It appeared that whoever slept in the bed entered and left the bed from the west side. Some of the bedding was pulled onto the ?oor. I saw a white color pill on the ?oor in a fold of the bedding that appeared similar to the pills Deputy Wagner showed me in the dressing room. I pointed this out to Deputy Nelson and continued to look at the scene. I saw an Apple brand laptop computer sitting on the dresser in Prince?s bedroom. This dresser was on the east wall. I saw a charger cord for the computer but did not see any cell phones or cell phone equipment. I did not see anything that was readily apparent as evidence in my survey of the interior of the building. When I returned I was directed to the upper ?oor and a rom that appeared to be an office or business type area. This room was on the east side of the upstairs hallway and toward the north side of the building. Deputy Wagner showed me a white envelope that centained a large number of one hundred dollar bills. These bills were banded together with a paper wrapper. I did not count the bills or any other money found at the scene. Sheriff Olson had spoken to one of the Attorney?s that had arrived and represented the business that Prince Nelson conducted. Sheriff Olson and the Attorney agreed that we would collect any ICR: 2016012559 cash and place it into One of the ?le drawers in the room that appeared to ?serve as an of?Ce. These two file drawers are capable of locking and the keys were-in the looks. I found a brown evidence envelope and retraced my steps. Gathering any cash that the investigations team had uncovered. The areas currency was located in were the bedroom, the sitting room attached to the bedroom, the of?ce area, and the room used as a makeup and bathing area. I put the currency in the drawer and Hennepin County Sergeant Schwartz took photos of the envelope and the drawer. I locked the drawer and gave the key to Sheriff Olson who gave it to the Attorney 33. I later was recheCking the living areas and saw that the kitchen had some currency in small bills lying on the counter next to the door. I gathered this currency and took it up to the of?ce. I put it in the other drawer and locked it inside. Hennepin County Sergeant Schwartz took photos of this drawer and the key missing. I turned the key over to Sheriff Olson who gave this one to the Attorney as well. 34. I spoke with Deputy Wagner about the pills that had been found in the bedroom and the room just outside the "bedroom. The pills, the pill bottles, and the pills from the dressing room, were all seized and inventoried on the search warrant receipt. Dr. Strobel was present when the pills were discovered and inventOried. I 35. Sheriff Olson told me that one of the attorneys who had arrived was in charge of the estate details. Mr. Gary Hanson. was eventually given a copy of the search warrant receipt along with a copy of the search warrant. 36. I spoke with the Crime Scene team and they photographed the entire property again prior to our leaving to show the interior as we were leaving it. They also said that they were going to handle the photographs differently than their standard procedures. They said that a copy of the video and the photos would be delivered by Lieutenant Turpitt the next day and that their reports were locked down so 'no one else could read them. - 37..'The assistant to Prince, .Meron Vekure, came back and spoke to Commander Tschida, who brought her to me. She asked that she be able to enter and retrieve her keys. I escorted her to the west side of the studios and on a desk was a set of black color keys she said were hers. She described them prior to me seeing the keys so I felt assured this was something that was hers-I asked her where she stayedlwhen she did not travel home and she said she had never stayed at the residence overnight. She said she went to the hotel if she was too tired to travel home. She took the keys and I escorted her out of the building. 3 8. I had a round table discussion with my staff and we turned the building over to Attorney Gary Hanson and his partner. - 39. I checked to see that preservation letters and evidence were being processed. I spoke to the Command Staff and we made a plan for the next day. detailed Deputies Nelson and Nucci to attend the autopsy when it was conducted by the ME in Anoka County. 40. fielded ideas and thoughts about'the death investigation. I assigned Deputy Nelson to gather the medical records from Illinois were Prince Nelson had made an emergency landing in his plane and the ones from the hospitals on our area. We discussed the Prescription Monitoring Program (PMP) and I asked Deputy Wagner to start writing search warrants for the records of the people on the staff that surrounded Prince Nelson. - 41. I discussed the sealing of the search warrant that were had served on the residence of Prince Nelson, and a plan was made to see that this happened. 42. I called and discussed the processing of the pill bottles that had been seized. After some discussions and information sharing, I learned that the method of communication between Nelson and his staff was the telephone or emails. I regretted not taking the computer by the bedside of Nelson as I believed that this contained conversations betWeen his staff and Nelson about medications possibly. I directed Deputy Wagner to write a search warrant for the computer that we had not seized. . 2016012559 43. 44. . the security staff. I explained why we were they and some telephone calls were made. I spoke learned that the substance on the wall of the elevator at the recording studio had been photographed but had not been swabbed. On 04-25-2016 at about 1400 hours, I drove to Paisley Park and we were allowed to enter the grounds by former power of Attorney for Prince who said that all inquiries were being-handled by Attorney Gary Hanson. I asked that we be allowed inside the residence to swab the elevator and was told that Hanson was able to arrange this. I knew that Deputy Wagener, who was with me, had a signed search warrant in her possession for the computer that had been by the bed of Prince. I was unable to get any answers to Hanson by telephone and the security staff asked us to leave. I left without serving the warrant or letting anyone i know We had a warrant. When I was back at the of?ce'l received a telephone call from Gary Hanson. He said he was at the airport and'was unable to take my call. I explained I needed to have the wall swabbed and he agreed to meet me at Paisley Park at about 1530 hours the next day. - On 426-2016 at 1530 hours, I went back to Paisley Park with Deputy Wagner and Deputy Stahn. I eventually met Gary Hanson and another attorney who was with him (see Deputy Wagner?s report). I explained that there was some substance on the wall of the elevator that we needed to swab as this had been neglected. He took us to the front doors and I saw that Meron Vekure had opened the doors for him. I sent Deputy Stahn to do the swab of the elevator and asked to meet with Hanson. We went into the of?ce area that is the ?rst door inside the front entrance on the west-side. I explained to Hanson that we needed the computer. I offered him the option of giving us consent and voluntarily? giving me the computer or asking me to serve the search warrant. We discussed the advantages to both alternatives for the estate and he asked for some time to consult. I went to the couches in the front area of the entrance and waited with Deputy Wagner. Hanson told me that he would sign the consent form and give us the computer that was in an of?ce on the first ?oor..He signed consent to search form that Deputy Wagner?lled out and on the form was written that the estate was able to view the information with us when we analyzed it. See the form as it is attached to this report. - Hanson had to call a locksmith to open the locked of?ce the computer was in. I left Deputies Stahn and Wagner remained to collect the device. I received a message later that said the computer was in our custody. I spoke to Medical Examiner Dr. Strobe] and asked that the blood sample that was taken from Prince Nelson the night before his death be analyzed for toxicology to compare: with the sample taken at autopsy. I spoke to BCA Agents Chris Olson and Scott Mueller about doing computer analysis on the laptop that was obtained. They agreed that they would make arrangements to have their best forensic computer technician do the analysis. On 5-03?2106, I was told that our Of?ce would partner with the DEA in determining where the controlled substances came from and that a determination on what charges would be filed and who would charge them would be made when the investigation was complete. I was told that a meeting was going to be set up for 5-04-2016 at about 1130 hours with the US Attorney and the DEA. - On 05-04?2016 I directed Deputy Wagner to print several pictures of the scene and the body of Prince Nelson as we found'Paisley Park on 4-21-2016, The disk of photos was in the care of Deputy Wagner with instructions to not allow anyone to print copies without my permission. I number the photos and brought them to the meeting at 1130 hours. I gave a briefing to the guests that Sheriff Olson had invited and passed the photographs around. I received the photos as they were done being viewed and later double checked the correct number were present and I personally placed them in the shedder for destruction. We entered the guarded gates and waited Of?ce would work together in two investigations that we knew would touch often. The death investigation into Prince Nelson?s death would be one of our priorities and the diversion of the controlled prescriptions would be the other shared by us and the DEA. We agreed to meet at DEA headquarters on 5-05-2016 to iron out some of the details. I explained that We had. the computer that had been by the bedside of Prince Nelson and I had arranged for the BCA to analyze it and do laboratory work on the pill containers that we had seized-from Paisley Park. I Was told that was ?ne and to handle this as we would normally by DEA Supervisor Jon Ryckeley. . - On 05-05?2016 at about 0830 hours, I met BCA Agent Chris Olson at our of?ce and he took the computer that we had obtained ?'om Prince Nelson?s bedside and took it to BCA for analysis. At about 1300 hours, Deputies Nelson and Wagner went with me to the Minneapolis DEA headquarters. We were shown around a ?lled out some paperwork to assist us in working with DEA. A list of priorities was set out and infermation was exchanged. DEA Task Force Supervisor on Ryckeley asked that I get the computer back from and they would have computer specialist come to our of?ce to image the computer under the search warrant We had already obtained. I called BCA Supervisor Scott Mueller who understood and made arraignments for us to pick the- computer back up. Deputies Wagner and Nelson went with me to BCA Headquarters and Deputy Wagner received the computer from Special Agent Olson. The computer was brought back to our Of?ce and entered into evidence again. On 05-06-2016, the computer from Prince Nelson?s bedroom was ?imaged in our lab by the DEA, and three copies were made. Two copies went with the DEA and one went into our evidence room. On 05-07?2016 [received a call-from BCA special Agent Olson who said the lab analysis of the containers given to them was done. He said there were prints suitable for comparison and DNA pro?les On 05-09-0216 at about 0930 hours, I received a telephone call from the Medical Examiner Dr. Strobel. She said that the toxicology results were back from the lab on Prince Nelson?s specimens. She said there were levels of prescription medications (hydrocodone) in the urine but none in the blood. She said the blood contained a high level of the drug Fentanyl. She said the count was 67.8 MCG per liter of blood. In way of reference she said a cancer patient with a Fentanyl patch on would change the patch every three hours and have a blood level of about three to ?ve MCG per liter of blood. I spoke to Chief Deputy Kamerud and Sheriff Olson about the news from the blood analysis. I explained that we needed to return to Paisley Park and search the building again looking for items that could contain the drug Fentanyl. I met with Deputies Nelson and Wagner about the information and I asked Deputy Wagner to begin a search Warrant for returning to Paisley Park. We informed our partners at DEA and the On 5-10-2016 a search warrant was obtained and the DEA and our deputies briefedat 1530 hours. Shortly after 1600 hours we were noti?ed-that the search warrant had been signed. Sheriff Olson made arraignments for a representative from Bremer Bank (the appointed administrator of the estate) to meet us and let us in the building. for the person with the keys to arrive. The keys arrived and I entered the building at 1744 hours. We searched the building until about 0030 hours, and then left uniform Sheriff 3 Of?ce staff on site for the rest of the night, with plans to return the next day at 0900 hours. On 5-11-2016 at about 0900 hours, we returned to ?nish the search of the building. We were unable to get into the vault in the basement without damaging it. She told me that the blood have Sheriff Of?ce staff remain on site and outside the locked building until an expert could be brought in to open the vault. The bank had concerns that the vault contained numerous valuable recordings that could be damaged by unwanted exposure from outside" the vault. When everything was searched except the vault - we took the evidence that had been collected and left. sample taken the night before Prince Nelson died (4-20-2016) had no Fentanyl in it. She asked about nasal sprays at the scene as one of the current methods of abusing Fentanyl is to snort it. I told 'her that there were some nasal sprays and drops at the scene. On 05-13-2016 I received a telephone call from DEA Agent Ryckeley. He said that Bremer bank was ?ying in a specialist to open the vault and he would be at Paisley Park on 5-15-2016 at about 1700 hours. On 05-15-2016 at about 1700 hours, I arrived at Paisley Park. There were numerous people present from the bank and a specialist who was going to open the vault and then put it back together so that the interior was secure. I was present when the vault door was breached and Opened. I saw the time as about 1910 hours. I watched as Deputy Wagner took photos of the interior of the vault. We searched the vault and did not find anything of evidentiary value. I found six zippered bank bags in a white garbage bag on a shelf in the and saw only" cash of different denominations. The bag that said ?opium? on it, I was unable to 'get the zipper to open. I used my poCket knife to slit the seam at the bottom of- the bag to look inside. I saw only cash in the bag. All six of the bags were given by Agent Ryckeley to the Bremer Bank personnel. To my knowledge the cash was never counted by any law enforcement at the scene. I saw Deputy Wagner take photos of the vault and the money bags on the shelf inside the vault as we left the interior; According to my watch the time we exited was about 1938 hours. On 05-17-2016 I spoke Supervisor Scott Mueller and made preliminary arraignments for the evidence we had obtained at Paisley Park to be analyzed for controlled substances including Fentanyl._See Deputy Wagener?s report for the in depth description of the transfer of the evidence to the BCA. I had several conversations with ME Doctor Strobel about a chemical analysis of the stomach ?uids found and collected in the autopsy. Dr. Strobel told me that the stomach acid would be expected to have some low levels of fentanyl in it because there was a signi?cant amount in" the bodyiblood stream. I asked Dr. Strobel to send the contents to the lab any way to see what the levels were asl felt they would be a signi?cant factOr. On 05-23?2016 at about 0800 hours, I spoke to Doctor Strobel by telephone. She said she had the results of the chemical analysis of the stomach ?uids back for the lab. She said that the extremely high level of fentanyl in the stomach contents left no doubt the drug was ingested orally by Prince in pill form. I told him I would have to go with him to collect those items. Andrew told me he was uncertain of the exact location of the Of?ce as this was the first time he had been at Paisley Park Studio?s. Andrew told me he just arrived in Minnesota this, morning. He was supposed to meet with Prince. I asked if he was in the music ?eld. He told me he was in from California representing his father?s Recovery Clinic. He told me the clinic is called Recovery Without Walls. 1 escorted Andrew to an Of?ce he identi?ed. This Office was located on the West side of the building. This is a larger of?ce located off one of the larger recording studios. Andrew told me he was brought to that Of?ce after they arrived at the complex. He told me after they arrived Kirk went to try and locate Prince. Once we were at the of?ce Kirk identi?ed hiscoat, and a green color backpack, which was located on the couch. I allowed Andrew to pick up those items. Note: Before these items were removed I took general overview photographs of the room, with my county issued IPhone, to document the current condition of the room and the locations of the items to be removed. 8. After we collected these items I walked with Andrew back to the lobby where we again met with the other. As we were walking out of the building I explained to Andrew I would need to retain his bag until we talked. 9. I told him I would hold his bag and place it in my squad and would bring it to the Chanhassen city hall. I explained the same to Meron. I told her I would be holding her bag until we met. They both said they understood. I maintained control of both bags, and at no time were Andrew and I separated while we were walking through the building. I was always with Andrew and at no time did I see him remove anything from his backpack. 10. We cleared from Paisley Park at approximately 1045 hours. 11. At approximately 1054 hours I met with Andrew Kornfeld in a private of?ce at the Chanhassen City Hall. At that time I also had Meron?s and Andrews bags in the room where I could see and control them. 12. I explained to Andrew because he was present at the death scene we needed to interview everyone who was there, and talk about any information they may have about Prince. Andrew said he understood. 13. Andrew told me this was the ?rst time he had been to Minnesota. He said he lives in California. He told me he came to meet with Prince as a representative of his father?s recovery center. He told me he arrived at the airport at approximately 6am, this morning. I asked about the recovery center. 14. Andrew provided me a business card from Recovery Without Walls. This business is Operated by his father Dr.HOward Kornfeld. Andrew denied he was a licensed doctor, but admits he was pre-med and admits he also has a medical background. Andrew told me the recovery center specializes in pain management, addictions to medications, and mood disorders. 15. Andrew told me the meeting with Prince was arranged by Prince?s people. Andrew said his father, Dr. Howard Kornfeld, was unable to meet so he came in his father?s place to meet with Prince. The purpose of the meeting was to discuss the concerns about Prince?s health. 16. Andrew told me the meeting was arranged by Kirk (Prince?s friend and security manager), and a girl whom he could not remember her name. He told me the girl was also at Paisley Park this morning. I have identi?ed the girl, an adult, as Meron Bekure, Prince?s assistant. 17. Andrew told me he took a red ?ight and landed this morning about 6am. He told me Kirk and Meron set up his hotel arrangements, at the County Inn and Suites. He told me they checked him in to the hotel but he did not go to his room, he told me he had breakfast with Kirk and Meron before they were going to meet with Prince. 18. Andrew told me they were trying to contact Prince, but he did not answer. Andrew told me they arrived at Paisley Park at approximately 0900 hours. He told me he and Meron went to wait in an of?ce, where we picked up his backpack and Meron? purse. He told me Kirk and Meron then went to look around the building to try and locate Prince. 19. He toldme he heard a scream and went to the area where he heard the scream. He said he observed Prince lying on the ?oor of the elevator. Prince was lying on his left side he told me it looked like he just laid down. He told me he did not see any trauma to Prince?s body. He told me Prince?s body was warm to the ICR: 16-012559 20. He told me he dialed 911, but when the dispatcher answered he did not know where to tell them he was. He said he had never been to the Paisley Park Studio before and did not know the address. 21. I asked about his purpose for coming to meet with Prince. He told me the meeting was arranged by his father by Prince?s people. He said he did not have much information because he had not yet met with Prince to get the background information. He told me he thought it had to do with prescription medications. He denied he had any contact with Prince before Prince was discovered in the elevator. 22. Andrew told me he received information from Kirk, that Prince was struggling with opiate use, and he (Andrew) believed Prince may have been suffering from opiate withdrawal. He told me Prince had also reported not feeling well. - 23. I asked when his meeting with Prince had been arranged. He told me his father was contacted yesterday, the 20th. He admits the meeting was put together at the last minute and he came to Minneapolis early this morning. - 24. Andrew told me he works with his father, but he also has his own company, Practitioner Plus LLC. He told me his company is a medical marketing and consulting company. He said he operates the business from his home residence in California 25. I asked who his main contact was in arranging the meeting. He told me his travel arrangements were made by a woman name Phaedera Ellis-Lamkins He believed Kirk and Meron could tell me about Phaedra. He told me his direct contact was with Kirk when he landed. 26. I explained to Andrew the reason why I wanted to control his bags, was because those items were located inside the scene. I asked if we looked in his bag if there was anything of concern we might ?nd in his bag. 27. Andrew admitted there would be something of concern. He asked if he should get it. I asked if he was willing to tell me what it was. He said yes. Andrew told me he had some pills with him. The time stamp of this conversation is approximately 20 minutes. 28. Andrew told me he had some Buprenorphine which he told me is a drug used for opiate addicts. He told me he wanted me to know he brought these pills of his own accord. He told me ?when he travels with this, these are the types of things? his sentence was incomplete. He told me he had no intention to using them. 29. Andrew told me he told me he also had a ?little? adovan, which he said is like a benzodiazepine, to be used if someone was having a seizure. He said he also had an anti-nausea suppository, if someone were puking. I asked if these were controlled medications. He admitted they were. 30. I asked if these medications were something he had a prescription for, or if he took them from the facility because he was going to meet with a client. He denied he took them from the facility, but admits he brought them of his own accord. He told me he had gotten them from his medicine cabinet. 31. He told me he did not tell his dad he brought them. He told me he did not think about bringing them. He said he just had them. He said he did not think about it, he just said he had them, and now admits it wasn?t a very good idea. 32. I told Andrew I had some concern he was going to meet someone who was a possible prescription drug addict, and he was bringing controlled prescriptions that did not belong to him. I asked if he was qualified to determine what was going on, or able to administer the medications. Andrew denied he would administer the drugs under any circumstances, without ?rst consulting with physician. I asked why he would be carrying the medications. 33. He told me the ?honest answer is? if someone was dying in front of you. I pointed out if he was not qualified, how would he know if he was helping or hurting the circumstances. He told' me he would call a doctor first. ICR: 16- 012559 34. I asked if he could help me understand why he had the medications with him. I pointed out he was bring drugs to a person who possibly had a chemical dependency issue, he wasn ?t quali?ed to administer them, and was possessing medications that were not prescribed to him. Andrew agreed. I told him I was trying to understand the circumstances why he brought the medications. 35. He gave the explanation saying if there was an alcoholic who was having a seizure and he was on the phone with 911, and told them he had some valium, or he had whatever, he would ask if he should give the medication to them. Andrew denied he would ever give the medications without talking to a physician. He also admitted he would be on the phone with his dad. 36. I pointed out our 911 dispatcher, or our medics, are not going to tell him give anyone anything until they arrived. I told Andrew the concern was if someone discovered him (Prince), and things were going had, someone tried to intervene by giving him something. Andrew denied he had any contact with Prince before he was discovered in the elevator. Andrew told me if he had discovered if he (Prince) was having some distress he would call his dad, or he would call 911. 37 . Andrew told melthe suppository are not a controlled substance and the other medications, there was not that much of it. I told Andrew from what he disclosed, the other medications were controlled. I explained, he knew the medications, are neither prescribed to him, nor does he have the authority to administer or control them. 38. I told Andrew the medications were in his bag would be collected and we would be keeping them. Andrew said he understood. I asked if there was anything else in the bag we should know about, or if he had anything-else on his person. He said no. I told Andrew I would be keeping his bag and inventorying the items in his bag. I told him at a later time we could determine what items could be returned. 39. Andrew told me he did not think through the implications of having those medications. Andrew told me they were just in his medicine cabinet and he. did not even remember where they were from. I explained I believed he brought the medications because he was coming to see a client, and if he (Prince) was having problems, he was going to try to help him. Andrew told me he would call a doctor ?rst. 40. I asked if his backpack was a carry-on bag, or was it a checked bag when he was on the airplane. He admits it was a carry?on bag. I told him if airport screen had checked his bag there was the potential he could have been arrested at airport screening because the medications we lose in a bag, and not in an identi?ed bottle prescribed to him. 41. Andrew asked if he was going to be charged. I explained we were looking into the circumstances involving Prince?s death. I told him the issue would have to be address, but I did not know what was going to happen at this time. 42. I again asked when they arrived at Paisley Park. He told me they arrived between 9 or 915. I asked how long he was in the of?ce before Prince was discovered. He thought it was about 10 to 15 minutes. Andrew said Kirk left ?rst, and then came back. He said Kirk seemed nervous like he had interrupted Prince. He believed Kirk found Prince ?rst, and then Meron went with Kirk. He told me Meron was originally with him in the of?ce when Kirk ?rst went to look for Prince. 43. I asked about the other people at Paisley Park. He told me he did not know the others. He said the doctor arrived after he, Kirk, and Meron arrived. He told me the doctor just showed up, and he had no idea the doctor was coming. 44. Andrew seemed to have a hard time remembering if the doctor showed up after Prince was discovered, or arrived after the police. After this inte vi Andrew waited in a conference room which is located in the public lobby of the Chanhassen city Hall. This is a public area, and the conference room was not locked, nor was anyone posted at the door preventing people from coming or going. The others who were at Paisley Park were also waiting in this same conference room, waiting for their turn to be interviewed by either Detective Nelson or myself. 47. At the end of the interview with Andrew I told him I would be willing to drive him back to his hotel if he did not have another ride. - 48. After the interview with Meron Bekure I again brie?y spoke with Andrew, who was still waiting in the conference room. Andrew was sitting in the room by himself. I asked how he was doing. He told me he was just thinking about the things he was doing. I asked if he needed a ride back to his hotel. He said he would like a ride. 49. At approximately 1345 hours I transported Andrew back to the County Inn and Suites located in the city of Chanhassen. During the ride Andrew asked if he was able to return to California. I told him I was not going to keep him from returning home. 50. He asked about his backpack and property. I explained the backpack was going to be held and we would decide at a later time what could or could not be returned. He asked if it was possible the backpack could be shipped to him in California. I told him we could decide at that time, but if he wanted it shipped he would be responsible for the shipping costs. 51. I asked if he was going to arrange a ?ight back today. He said he was going to try to find a ?ight, but when he got back to the hotel he was going to sleep for a while. I dropped Andrew off at the front door to the Country Inn and Suites. The follow up conversation, nor the car ride back to the Hotel, was not audio recorded. 52. Additional information; On 04?21-16 at approximately 1741 and 1746 hours I received calls from Attorney Bill Mauzy Mr. Mauzy was leaving a message telling me he was representing a client Andrew Kornfeld and asked if I could return his call. 53. On 04-21-16 at approximately 1933 hours I spoke with Attorney Bill Mauzy by phone. Mr. Mauzy con?rmed he was representing Andrew Kornfeld. He told me he was calling asking if his clients lap top computer could be returned. 54. I explained to Mr. Mauzy his client?s property would be held until a search warrant could be executed on the bag. I explained to Mr. Mauzy the circumstances his client explained, and that his client disclosed he had controlled prescription medications in his bag that were not prescribed to him. I told him we could decide after the search warrant was executed what items could be returned. 55. Mr. Mauzy told me he understood. He also told me from talking to his client that he understood his client was free to return back to California. I con?rmed he could return and if there was need to get in touch with his client at a later time I could make arrangements with law enforcement in California for any follow up that may be necessary. Mr. Mauzy asked I go through him if there was any other follow-up with his client. 56. Mr. Mauzy asked if there was a possibility his client could be charged with a crime. I told him I was unable to give him an answer at this time. 57. At the end of our phone conversation I asked Mr. Mauzy to send me a letter verifying he was representing Andrew Kornfeld, and I would add that information to my ?le. The recorded messages received from Attorney Bill Mauzy and the follow up phone call was audio recorded and will be downloaded to a CD and placed into evidence. 59. Additional follow up. We have veri?ed Andrews Flight information through Agent Jesse Tabolich, Department of Homeland Security that Andrew KornFeld was on Delta ?ight 0677. The ?ight originated from San Francisco. The ?ight departed San Francisco at 0019 hours on the 215?, and arrived in Minneapolis St. Paul at 0553 hours 60. Additional information; We have also followed up with the County Inn and Suites to con?rm when Andrew checked in to his Hotel room. On 04-22-16 I requested Deputy Horvath to check with County Inn and Suites to verify when Andrew checked into the Hotel. (Deceased) Paisley Park Studios 7801 Audubon Road Chanhassen, Mn 55317 On 04-21-16 at approximately 1425 hours Detective Angela Nucci and I met with the Honorable Judge Eric Braaten. At that time Detective Nucci presented an application and af?davit for a search warrant to search and process the death scene located at 7801 Audubon Road in the city of Chanhassen, also known as Paisley Park Studio?s. This is the location where the body of Prince Rogers Nelson was found. Judge Braaten reviewed and signed the warrant at approximately 1428 hours. After the search warrant was signed Detective Nucci contacted Detective Sgt. Dewitt Meier and informed him the warrant had been signed, and we were on our way to Paisley Park to assist in the execution of the warrant. At approximately 1456 hours I arrived at Paisley Park to assist with the execution of the search warrant. At that time the Hennepin County Crime Scene team was in the process of taking overview photographs documenting the current condition of the complex. During the course of the execution of this search warrant I was in a number of rooms within the complex, the complex was clean and most things appeared organized and in their place. I assisted in searching a number of rooms in the complex. On the second ?oor I assisted in searching the of?ce, dressing room, bedroom (with round bed), a conference room, a bathroom with a white tub located in the center of the room, a mechanical room located off the bathroom with white tub, and a bedroom (the green room) believed the bedroom Prince primarily used when he stayed at Paisley Park. During the search of the of?ce on the second ?oor we found a large amount of cash in an envelope on the top of the desk. There appeared to be a stack of 100 dollar bills bound with an identi?er indicating there was $10,000 in the stack of cash. The seal on the money was not broken, but was photographed as it was found and returned to the envelope. I also found other small amounts of cash in different drawers in the of?ce. When searching the bathroom I found small amounts of cash. I also noticed a number of enema bottles in Revised 0512 10. 11. 12. 13. 14. 15. 16. 17. 18. ICR: 16-012559 the sink cabinet. Although this may not be unusual to ?nd these products in the bathroom but there were multiple boxes. I did notice a number of these empty boxes found during the search of this area. These appear to be an over the counter products. I did not see any names on any of the boxes. I assisted in searching the of?ce located on the mail level near the recording studios. I also walked through a number of the recording studios. I was in the lower level garage where a number of cars were kept. I did search though a number of cars. I did not ?nd anything of any signi?cance in the cars. A number of the cars appeared as though they had not been driven for some period of time, some of them had ?at tires, while others had a signi?cant amount of dust that had settled on them. I found when going through other rooms in the complex it was not unusual to ?nd small amount of cash in these plastic shopping bags, or other small amounts of cash left in drawers. When money was located it was photographed and left in the room it was found, and was later to be collected. We received instruction 'that at the end of the execution of search warrant the money was to be collected by Sgt. Meier and would be provided to a representative of Paisley Park. There were a number of times when going through cabinets and drawers when I would ?nd a plastic shopping and would ?nd small amounts of cash in the bag. Sometimes it was a couple dollars, other times it was 20 dollar or more. It appears whoever went to the store left the change in the bag, and then left the bag in the drawer. I assisted searching the bedroom believed to be the room used by Prince. In the bedroom I searched a black suite case with blue interior. This suite case was originally found on a chair. Inside the suite case were a number of miscellaneous items, including personal hygiene items. I again found cash, $191.00. Inside the suite case I found (2) medications bottles in the case with the name Kirk Johnson. Kirk Johnson was one of the people at Paisley Park when we arrived, and one of the people who discovered Prince in the elevator. When we looked inside the medication bottles it was discovered there appeared there were (4) four different types of medications in the two separate bottles. It appears the medication listed on the outside label was not the same medications as found inside of the bottle. Detective Wagner did some additional research on the contents of both pill bottles. The ?rst prescription bottle was in the name Kirk Johnson and prescribed by Dr. Michael Schulenberg. The bottle labeled Vitamin D2 contained (7) green capsules with number 194 on the pill, and (8) oval yellow pills stamped with G1 on one side, and number 8 on the back. By using the web site drugscom, the green capsule with the number 194 was identi?ed as a Vitamin capsule. The (8) yellow pills stamped with the number 8 was identi?ed as Ondansetron Hydrochloride 8mg. The second bottle also had the name Kirk Johnson?s and was prescribed on 04-07-16 by Dr. Schulenberg was for Ondansetron HC. Inside that bottle was (1) one yellow oval stamped pills with the number identi?ed as Ondansetron Hydrochloride 8mg. There were also (8) whole white round pills stamped with the number Af349, along with (4) halves of the same size pill. Using the web site the drugscom, these pills were identi?ed as Acetaminophen/oxycodone hydrochloride 325 mg. It should be noted a number of of?cers searched different areas of the complex. There were (3) three different Hennepin County Deputies, Crime Scene Technicians, Deputy Sarah Buck, Deputy Kevin Schwartz, and Deputy Shane Magnuson taking photographs, documenting, and assisting in the collection of items of interest. During the execution of this search warrant I discovered the (2) two prescription bottles with the name Kirk Johnson on the outside label. These items were later taken as evidence by Detective Wagner. The content of these pill bottles will be veri?ed at a later time. Any cash I found was left in the room where it was discovered. These people were requested to come from Paisley Park Studio?s to city hall. Meron got a ride from one of the others who were at Paisley Park. I asked Meron what she did at Paisley Park Studio?s. She told me she was Prince?s personal assistant. She told me she has worked for Prince for approximately the last 1 1/2 yrs. I told Meron we were trying to gather some background information to explained Prince?s death, including the last time people saw or spoke with him. Meron said she last saw Prince the previous night. She told me she, Kirk, and Prince were at Paisley Park Studio?s. She said about 8pm Prince went tO his side of the building, she called it his side of town, and she and Kirk stayed at the Studio until approximately 2230 hours. Meron told me when she is working in Chanhassen she sometimes stays at the Country Inn and Suites located in Chanhassen. She told me she has a full time room, number #127, she said is paid for by Prince. She told me she arrived back at her hotel room at approximately 23 00 hours. I asked if she could provide any background about what was happening with Prince lately, or how he was acting the previous day. Meron told me she had not seen Prince since yesterday at 2000 hours. She said they tried to call him this morning, but he did not answer. I asked if there was anything unusual about yesterday. Meron told me she only saw Prince from a distance. Revised 05/12 ICR: 16- 012559 She told me she thought she was supposed to accompany him to a doctor 5 appointment at 1630, but she was told Kirk was going to go and she did not need to go. She again told me she only saw Prince from a distance. 7. I asked if anyone else was on the property yesterday. She told me Prince has a personal chef, Ray Roberts?s Cat" who prepares his meals. She told me Chef Roberts made dinner for Prince, but she did not believe he ate last night. She believed Roberts was at Paisley Park until approximately 2030 hours. 8. I tried to get additional information from Meron to try and understand if there was anything unusual about yesterday, or if there were any other concerns for Prince. Meron told me she was uncertain what information she could share with me. She told me when she was hired she signed a con?dentiality agreement with Prince, and she did not know what she could say. She told me she would like to talk with someone or talk to an attorney, before she would be able to answer any of my questions. 9. I explained to Meron we were only trying to understand if there were any issues or concerns that may help us understand or explain why Prince was found dead. Again Meron told me she was uncertain what questions she could answer because of the con?dentiality agreement. She told me she saw Prince the previous day from a distance. She did not provide any other information. 10. During our conversation I asked Meron if she could make a general diagram of the complex so 1 could understand where Prince living quarters were, and where the studio was located. She told me the building 1s divided 1n half. She told me Prince stays on the North side of the building where the kitchen is located. She told me his bedroom, the green room, is located on the second floor. 11. I asked about the business structure for Paisley Park Studio?s. I was trying to understand the business structure and who may have information about Prince, or who controls Paisley Park Studios. Meron told me she and Kirk control the Paisley Park Studio part of the building, and the recording studio is operated by NPG (New Power Generation). Meron did not-provide any information about any other person who may-have information about Prince. She denied she was aware of anyone else being on the property the previous night. 12. I explained to Meron because her purse was located within the scene of a death investigation, there was a necessity to control her bag. I asked if there was anything of concern in her bag. She told me there was not. I asked if she would consent to the search or her bag. She agreed. 13. I did search the contents of her bag. The only item of note was she was carrying what could be considered a signi?cant amount of cash. In a separate envelope. she had $3 630.00 in cash. She told me that money was from the sale of what she called ?Tour Books?. She said these are merchandise books Prince put out containing photographs. She said the books are similar merchandise you would get if you bought a concert T-shirt. In a second envelope she had $200.00 cash which she said was hers. I did not note any other items of signi?cance. After the search, the purse was returned to Meron. 14. At the end of the interview I asked how long she would need to consult to determine if she would be willing to provide any other information about Prince. We agreed she would take a half hour to make phone calls she needed. We also agreed we would meet back in the main lobby at Chanhassen City Hall, near the conference room, where she had previously been waiting, at approximately 1300. I ended the interview with Meron at approximately 1223 hours. Meron was escorted back out to the public lobby. 15. At approximately 1230 hours I was asked to do some additional follow up related to this investigation. Because I had to leave the building I asked Detective Nelson if he could meet with Meron in the lobby approximately 13 00 hours. 16. I checked with Detective Nelson at approximately 1300 hours. He told me he checked the lobby and Meron had not yet returned to the city hall. He told me he would wait until approximately 1315 hours to see if she returned. Andrew told me he arrived in Minnesota this morning to meet with Prince Rogers Nelson. This backpack was recovered by me at Paisley Park Studio?s after Andrew pointed out where the back pack was located. 3. The backpack was controlled by me until the interview with Andrew. During the interview Andrew made disclosures for his reason to meet with Prince, and what was in his backpack. The backpack was seized on 04-21-16 at approximately 1129 hours. Please see the attached follow up report regarding the interview with Andrew Kornfeld. 4. On 04?21-16 at approximately 1820 hours the backpack was locked into the cabinet at my desk. 5. On 04-22-16 at approximately 1600 hours the backpack was taken from my locked cabinet. On 04-22-16 at approximately 1614 hours the backpack was entered as evidence and placed into evidence locker 15 at the LEC, pending the execution of a search warrant. 6. On 04-26-16 at approximately 0917 hours I met with the Honorable Judge Eric Braaten and presented an application and af?davit for a search warrant, to search the content of the backpack seized from Andrew Kornfeld. Judge Braaten reviewed and signed the warrant. 7. On 04-2646 at approximately 1330 hours Detective Tyler Stahn and I executed the search warrant at the Sheriffs Of?ce on the green backpack seized from Andrew Kornfeld. During the Search of the backpack Detective Stahn and I both wore latex gloves. Within the backpack we located a number of items including small sandwich size zip loc bag containing a number Of pills. Revised 05712 8. Located 1n the zip loc bag were the followmg pills, (2) two suppos1tor1es still 1n their package labeled as, Promethazine HCL .25mg, (11) individual pills with the number on the pill, (5) ?ve pills with the number ?5441 1? on the pill, and (6) six pills with the name ?Mylan? and the number ?457? on the pill. 9. I have preliminarily identi?ed these pills, through a google search, using a web site called Drugscom, and identi?ed the mediations as the following: The pills identi?ed as are believed to be ?Suboxone? (2mgf.05 mg) a schedule 3 controlled substance. The pills identi?ed as 54411 are believed to be Buprenorphine (sublingual 8mg) a schedule 3 controlled substance. The pills Mylan 457 are believed to be Lorazepam (1mg) a schedule 4 controlled substance, and the suppository is identi?ed as Promethazine 25mg. - 10. I have also contacted a pharmacist at the Target Pharmacy in Chaska to verify my research. With the information I provided to the pharmacist (Emily), she was able to con?rm my research. 11. The only pill she was unable to con?rm was Suboxone tablet with the marking stamping. Emily believes this could be an older manufacture of this medication. Emily was able to con?rm all these pills are controlled medications, and would require a prescription to obtain. She con?rmed none of ?iese medications are available over the counter. 12. Also located were a number of other miscellaneous items including what appeared to be (2) two medical devices. These were ?Homedics? devices. One item was a blood pressure cuff and the other was a ?ngertip pulse monitoring device. 13. Also found was a soft cover book, or information booklet with the title The Emerging Role of Buprenorphine In The Treatment of Pain?, Selection of Medical Publications by Howard Kornfeld, At the bottom of the booklet identi?ed the author; Howard Kornfeld, MD, Howard Kornfeld, MD Associates, Recovery Without Walls?. 14. The signi?cance of this book was-Andrew Kornfeld was to meet with Prince Rogers Nelson, representing Recovery Without Walls, and one of the medications found in his backpack was preliminarily identi?ed was ?Buprenorphine?. 15. Also in the backpack, in a blue color carrying case was a silver! gray color Apple Laptop Mac Book Pro- with black plastic cover, serial 16. I believed there was a computer in the backpack, but was unable to con?rm that information until the warrant was executed. I did not look through, document or search any of the contents of the backpack until the signed search warrant was received. 17. Note: I had previously been contacted by Andrew Kornfeld Attorney, Bill Mauzy, requesting the return of his client?s computer. Mr. Mauzy was told no items would be returned until the execution of the search warrant. After the warrant was executed a decisions would be made, what if any items could be returned. 18. Detective Stahn photographed all the items locate in the backpack. The pills recovered from the backpack were repackaged and will be entered as separate evidence items. I then observed Prince Rogers Nelson lying on his backthe ground, in front of the elevator. I was then requested by Det. Wagner to interview all ?ve persons that were on the premises at the time the incident was reported, and to conduct the interviews at the Chanhassen City Hall (7500 Market Blvd. in Chanhassen, MN). Det. Wagner advised me that Johnson, who is the ?grounds keeper?, will'remain on the premises in order to assist and guide detectives through a protective sweep of the building. 4. At this time, Det. Murphy, Det. Chris Nelson and I asked Kornfeld, Bekure, Schulenberg, Hannah, and Joshua to follow us to the Chanhassen City Hall so we could speak further about what occurred. Kornfeld then requested if he could retrieve his backpack from the building and take it with him. Bekure then requested if she could also retrieve her backpack, and Det. Murphy assisted both Bekure and Kornfeld in retrieving their bags from the property. - 5. Upon arrival at Chanhassen City Hall, Det. Murphy explained that we would be speaking with everyone individually. Det. Murphy advised, him and I will begin speaking with Kornfeld, while Det. Nelson would be speaking with Schulenberg. I then guided Hannah, Joshua, and Bekure to a conference room where they could sit and wait for Det. Murphy, Nelson or me to get them. I then asked ifI could bring anyone anything, and they (Bekure, Hannah, and Joshua) requested water. I then requested Chanhassen CSO Jazmin Sanchez, who was present at the Chanhassen City Hall, to assist in providing water. CSO Sanchez was able to Obtain water and provide it to them as requested. 6. I then returned to the of?ce where Det. Murphy was conducting his interview with Kornfeld. During the interview, Kornfeld disclosed that he was from California and was sent to Minnesota to meet with Prince Rogers Nelson. Komfeld explained that he was not given much information about the meeting, other than his father (Howard Kornfeld, M.D.) requesting him to come over to Minnesota and meet with Prince?s staff. KOmfeld explained that he works with his father in?a rehabilitation program called Recovery Revised 05(12 10. 'll. 12. 13. ICR: 16- 12559 Wlthout Walls Kornfeld said that they are known to handle high pro?le cases with celebrities and that Kornfeld was supposed to meet with Prince Rogers Nelson on 04/21/ 16 1n order to determine whether or not ?the program? would be of value to him (Prince Rogers Nelson). Kornfeld explained that the program focuses in recovery of substance abuse and chemical dependency. Det. Murphy asked if Kornfeld had any specifics or information as to why or how ?Recovery Without Walls? was contacted or made aware of Prince Rogers Nelson?s situation, and how travel was arranged. Kornfeld said that the meeting was not arranged with Prince Rogers Nelson directly, and that he (Kornfeld) had never spoken or met Prince Rogers Nelson before. Kornfeld said everything was arranged through Johnson and a female by the name of Phaedra. Kornfeld said that he was supposed to meet with Prince Rogers Nelson, and get additional information about his ?issue/ condition? but that he ?inferred?, given the work he does with his father at Recovery Without Walls, that Prince Rogers Nelson was going through opioid withdrawals and or was needing assistance with chemical dependency recovery/ rehabilitation. Det. Murphy then advised Kornfeld that he had retained control of Kornfeld?s back pack due to it being inside the residence at the time of the incident. Det. Murphy explained that anything leaving the scene needed to be accounted for, as this is an ongoing death investigation. Kornfeld said he understood. Det. Murphy then asked Kornfeld if there would be anything of concern inside the back pack that we should be aware of. Kornfeld said there was, and explained he was in possession of controlled substances. Kornfeld said he had Ativan, which is a benzodiazepine used for when a person is having a seizure, and buprenorphine, which is medication given to patients addicted to opiates. Kornfeld said he also had an anti-nausea suppository used to prevent and help with vomiting, but that it was not a controlled substance. Kornfeld advised that he brought these controlled substances on his own accord and without consent or knowledge from his father or the medical practice. Kornfeld said that he does not have a prescription for the medication, nor has the ability to prescribe these himself. Kornfeld said that he would never administer these medications without medical advice. Kornfeld then reiterated that he did not obtain these medications from his father? 5 practice and that bringing the medications was his own decision, and his father had no knowledge of this. Kornfeld said he neVer administered any of this medication to P1 ince Rogers Nelson. Shortly after sitting in the interview, I received a call from Sgt. Meier requesting that I, along with Deputy Adam Hastings, respond to the city of Minneapolis in order to notify Tyka Evene Nelson (DOB: Prince Rogers Nelson?s sister, of his death. Please see Det. Murphy?s report for more information about his interview with Kornfeld. I met with Deputy Adam Hastings at the Chanhassen City Hall, and requested that we drive to the Sheriff? 3 Of?ce to retrieve my unmarked squad to then drive to Minneapolis. Upon arriving at the Sheriff?s Of?ce in Chaska, MN, Deputy Hastings received a call from Det. Wagner advising that Tyka had been notified by Johnson of Prince Roger Nelson?s death, and Tyka was already in route to Paisley Park. I then returned to the Chanhassen City Hall, and met with Det. Murphy who was just concluding his interview with Bekure. Det. Murphy was ?nishing inventorying Bekure?s property, and I assisted in counting the currency located inside Bekure?s bag. All of the currency, including all the property inside Bekure?s bag was returned to her. See Det. Murphy?s report for more information about Bekure?s interview. Det. Murphy then advised that a search warrant needed to be drafted in order to process the scene at Paisley Park. Det. Murphy explained that Sgt. Meier, Det. Wagner and other Deputies were securing the scene at Paisley Park and were waiting for us to draft a search warrant for the premises. 15advised Det. I wrote a Search Warrant for the premises located at 7801 Audubon Rd. in Chanhassen, MN also known as Paisley Park Studios. The Search Warrant was read, reviewed and signed by the Honorable Judge Braaten at l428hrs. At approximately 1434hrs, I made phone contact with Det. Wagner and advised her that I had obtained a Search Warrant for the premises, and the warrant had been signed by Judge Braaten. Det. Wagner advised they will begin the processing of the scene and requested I respond to assist in executing the warrant. At approximately 1501hrs, I arrived at the scene and reported to Det. Wagner. I was advised to proceed to the second ?oor east side of the building and assist in searching the rooms. During my search of the premises I did not locate any items of interest to the investigation. I then assisted Det. Wagner in packaging evidence found by Det. Nelson and Det. Murphy during the search. I completed the Receipt, Inventory, and Return form noting the following items collected: 0 15 white capsules with inscription ?Watson 853? found in second ?oor dressing room on east side of the building. 0 CVS Pharmacy bottle under the name of Kirk olmson, with said name Vitamin D2, which contained 7 green capsules with the imprint ?194? and 8 orange oval pills. The prescription bottle was located inside a suitcase in the mirror room joining the green room on the second ?oor. 0 Bayer bottle with 64 1A: white pills with inscription ?Watson 853?, located on the night stand near the bed in the Green Room on the second ?oor. 0 CVS Pharmacy bottle under the name of Kirk Johnson with said name Ondansetron HCL 8mg, containing 10 white round pills with inscription and 1 orange pill with inscription Aleve bottle containing 20 1A white pills with inscription ?Watson 853?, located on the night stand near the bed in the Green Room on the second ?oor. 0 ?Recovery Without Walls? pamphlet recovered from Purple Rain room, which is located in the main level of the building. 0 Items 1, 2 and 3 from Hennepin County Crime Scene Team containing a swab from Prince Rogers Nelson?s neck, left hand and right hand. A copy of the search warrant, and Return, Inventory, and receipt form was left at the premises. Prior to leaving the scene, I released the evidence collected to Det. Chris Nelson, to be entered into evidence at the Sheriff?s Of?ce Evidence Room in Chaska, MN. I then cleared from the scene at approximately 1800hrs. On 04I22/l 6 at approximately O900hrs, Det. Murphy that the date of birth for Prince Rogers Nelson on the warrant was incorrect. I reviewed the original warrant and noticed that I mistakenly typed Prince Roger Nelson?s date of birth as 0607/1978, and instead it should have been?. I advised Det. Murphy I will be contacting Judge Braaten and inform him of the ?typo? in order to correct it. 25. I attempted to contact Judge Braaten on 04/25fl6; however I was advised he was in the city of Glencoe, but should be back on 04f26/ 16. On 04f26x? 16 at approximately 1100hrs, I met with Judge Braaten in his chambers and explained the typo on Prince Roger Nelson?s date of birth on the original search warrant. Judge Braaten witnessed me correcting the date of birth and we both initialed the change on the warrant. 26. On 04f27!16 at approximately 105 Ohm, I met with Carver County Attorney Mark Metz. On 4-2146 at approximately 1000 hrs, I was requested by Sgt. DeWitt Meier to respond to Paisley Park, 7801 Audubon Rd in the city of Chanhassen, to assist with a male down at the complex. I was advised the male down was Prince Rogers Nelson (DOB I gathered my investigative equipment and responded to the scene. I arrived at approximately 1015 and I entered the front entrance facing to the north and Observed some people gathered in a lobby area inside Paisley Park. I ploceeded to meet with Lt. Eric Kittelson, Sgt. Jason Breunig, Detective Christina Wagner and Deputy Adam Hastings. I met with the three deputies' 1n the hallway by the elevator I was advised that Prince Rogers Nelson was deceased and observed him lying on the ?oor In front of the elevator. 2. Deputy Hastings gave me a brief scenario of what actions were taken when Prince was located. Deputy Hastings advised Prince was found inside the elevator by the Chanhassen Fire Chief and pulled out of it. CPR was started by Chanhassen Fire but Ridgeview Paramedics arrived, advising ?re could they could stop CPR. Ridgeview Paramedics called the time of death at 1007 hrs. Meier arrived on scene and Detective Wagner and I met with him. Shortly after Detectives Pat Murphy and Angela Nucci arrived on scene and we all met in the front lobby area to discuss the incident. It was determined that I Detective Murphy and Detective Nucci would take the parties that were at Paisley Park back to the district Of?ce 1n Chanhassen to conduct interviews. Detective Murphy advised the partiesthe interviews would be held at the Chanhassen office and we walked out with the five parties. The parties were not identi?ed yet but the group consisted of three males and two females. 3. exited out of Paisley Park at 1045 with Detectives Murphy and Nucci. The other ?ve parties followed us to the Chanhassen office because they had vehicles at Paisley Park. One female who was distraught was escorted with Detective Murphy in his patrol car. We all arrived at the Chanhassen City Hall and the Carver County District Of?ce at 1050 hrs. The parties were escorted in and I started my ?rst interview with Dr. Michael Todd Schulenberg who arrived on scene at Prince?s residence. LEAD 5 Dr. Michael Todd Schulenberg (Home) (Cell) 4. I conducted my interview with Schulenberg in the Chanhassen gt.?s room? I received Schulenberg?s information and ?lled out my recorded statement form and advised Schulenberg that was asking for a voluntary statement from him. Schulenberg agreed to speak to me and understood the conversation was beingrecorded and he was not under arrest. 5. I learned the following from Schulenberg during my interview with him. Schulenberg advised he knew Prince from a professional standpoint. I asked Schulenberg how he became involved with treating Prince for medical issues. Schulenberg said he has known Kirk Anthony Johnson [u for approximately Revised 05106 ICR: 16012559 10. 11. the past 16 years because Kirk was a patient of Schulenberg. Schulenberg said last fall Kirk contacted him to see if Schulenberg would be open to helping Prince, Kirk?s boss. Schulenberg said at that time, the idea was to just have a physical and to check Prince out. Schulenberg said nothing came to light at that time until about 2 weeks ago. On April 7th 2016 Schulenberg received a text message from Kirk at 0545 asking if Schulenberg would be able to help out by administering some I.V. ?uids to Prince. Schulenberg talked with Kirk and at approximately 0645 hrs, Schulenberg arrived at Paisley Park. Schulenberg met Prince and completed a physical exam on Prince. Schulenberg then asked about Prince?s past medical history. Since Schulenberg could not administer the LV. ?uids to Prince at Paisley Park, Schulenberg arranged for Prince to meet Schulenberg at his of?ce. Schulenberg said his of?ce is located at15450 Hwy 7, Minnetonka, MN 55345. Schulenberg said later that morning, Kirk brought Prince to the North Memorial Clinic and Meron, (later identi?ed as Meron NMN Bekure (DOB met them at the of?ce. Schulenberg said for privacy reasons, Prince, Kirk, Meron and Dr. Schulenberg entered through the back of the building and came in the Urgency part of the clinic because it was not open for patient care. Schulenberg placed an I.V. in Prince and gave Prince a liter of normal I.V. saline. Once the LV. of saline was completed on Prince, Prince advised he was feeling better and left with Meron. Schulenberg said two days later (April Prince called to let Schulenberg know that he was feeling better. Schulenberg then received a text message from Meron (around lunch time) asking Schulenberg if Prince could call him. Schulenberg said Prince did call him and they had about a 15 minute conversation. Schulenberg reviewed with Prince Some of the and some did improve. Prince then complained about some new because the previous night (April 7th) a chiropractor had worked on Prince and new developed. Schulenberg then offered to see Prince again and offered to do some blood work if Prince felt it was necessary. According to Schulenberg, his next contact with Prince was on the 21St of April. NOTE: Prince passed on 4-21-16 and Schulenberg says in the recording that it was yesterday which would have been 4-20-16. Schulenberg said he was contacted again via text message from Kirk. Schulenberg says he talked with Kirk on Wednesday about having Prince come in for some blood work at 7pm. Schulenberg said Kirk got in touch with him again on April 20th and Kirk brought Prince to the Hwy 7 medical building at 5pm for blood work. Schulenberg then says, ?In the interim I had learned from Kirk, who stated to me, that he had just learned this, ah, concern that, ah, Prince was using, ah, opiates.? I asked if Kirk told him how much Prince was using and Schulenberg said Kirk did not know. Schulenberg said Kirk had told him that Kirk had been in contact with a lot of people that knew Prince about this but Kirk advised he was not aware. I then asked Schulenberg about the that Prince was experiencing during his ?rst contact with Prince on April 7th. Schulenberg said Prince was complaining of numbness, tingling in one of his legs (right), numbness and tingling in his hands and that he had some vomiting the night before. Schulenberg said Prince attributed the vomiting to some soup he had eaten. I asked Schulenberg if Prince mentioned anything about taking any opiates. Schulenberg said he asked Prince if he was on any prescription medications and Prince responded no. Schulenberg then asked Prince again if he was taking anything for his hands and Prince told Schulenberg he had a medication but did not know what it was. I asked Schulenberg if he prescribed anything to Prince and Schulenberg said he did not on April 7th but did on April 20th for opiate withdrawal I Schulenberg then goes back to explain what procedures were completed on Prince on April 20th. Schulenberg said Prince came in at 5pm and explained that time worked because there were no patients in the clinic. Schulenberg met Prince, took Prince?s vitals and did a medical history. Schulenberg also said he completed a physical examination on Prince. Schulenberg asked Prince about doing blood work and Prince consented to have blood work completed. Schulenberg had a nurse come in for the blood draw and to place another LV. Schulenberg then gave Prince another liter of saline. Schulenberg said he asked ICR: 16012559 12. 13. 14. 15. Prince if he was taking any medication and Prince told Schulenberg he had taken Tylenol. Schulenberg asked Prince if there were any other ingredients in the Tylenol and Schulenberg advised he believed Prince said no or that he did not know. Prince told Schulenberg that he had taken the Tylenol earlier that morning. Schulenberg said Prince?s main complaint was that he was feeling antsy. Prince denied any nausea, vomiting or diarrhea but was asking about opiate withdrawal I asked Schulenberg if the blood test results had come back for Prince and Schulenberg con?rmed they did. Schulenberg then starts to talk to me about how he arrived at Paisley Park the morning of Prince?s death. Schulenberg said he had administered the liter of LV. ?uids and sent a prescription to the Walgreens ?lled on 4-20?16. Schulenberg said the prescription was for Clonidine. Schulenberg said Kirk then left to go and pick up the Clonidine. Schulenberg said he had given Prince two doses of Clonidine before Prince had left along with one dose of Hydroxyzine. Schulenberg noticed that I was having a hard time spelling Clonidine and Hydroxyzine and asked if I- needed him to spell them. I said ?yes? and Schulenberg spelled Clonidine and provided me with an explanation as to what it does. Schulenberg described Clonidine as a blood pressure medication that has for many years been used to treat opiate withdrawal Schulenberg described Hydroxyzine as an antihistamine similar to Benadryl. Schulenberg gave Prince the dose of Hydroxyzine because Prince felt like he needed to relax. Prince asked if Hydroxyzine could be given through the I.V. and Schulenberg did net want to make ?the problem worse by giving some other control medication intravenously in the of?ce.? I asked if the medication was given in pill form and Schulenberg said ?yes,? Clonidine 0.1 and the Hydroxyzine was 25 milligrams. I asked Schulenberg when Prince left if - Prince seemed to be in good spirits. Schulenberg said ?yeah, he appeared comfortable and he left with Kirk? and that was around 1830 hrs. - - I then asked Schulenberg why someone would want to have an LV. of ?uids. Schulenberg said most - people just feel better and Prince was mildly dehydrated saying Prince?s mouth was dry. Schulenberg stated he gave Prince a liter of normal saline because that is what Prince wanted. Schulenberg advised the potential for harm performing this procedure is very low. I asked Schulenberg if the prescriptions were ?lled and Schulenberg said the he only knew the prescription for Clonidine Was ?lled. Schulenberg said the other two prescriptions were printed and did not know if they were ?lled. Schulenberg advised one prescription was for the Hydroxyzine; Schulenberg then researched Opiate withdrawal, and issued the second prescription for valium. Schulenberg said he had a brief discussion with Prince about Kirk being a long time patient of his did not want to" violate trust. Prince asked what the medications were and Schulenberg told him. Schulenberg said it was Kirk?s preference not to have the valium and Kirk wanted the valium to be administered in a supervised setting. Schulenberg said it was his responsibility to be a good doctor and didn?t want to substitute one problem for another.? Prince said, ?Understood, I will use the other two prescriptions ?rst.? I asked Schulenberg what effects the valium would have on a person?s system. Schulenberg said the type of in Prince?s settings was primarily for anxiousness, jitteriness, and ?it?s widely used as a tranquilizer and muscle relaxant.? Schulenberg said he prescribed what he felt was reasonable short?term. Schulenberg prescribed Prince 30 capsules of the Clonidine and 30 capsules of the Hydroxyzine and for the Valium it was just 15 tablets. I asked if you were to mix all three of the pills together if it could cause health problems. Schulenberg said only if you intentionally overdosed on them. Schulenberg said all three are routinely taken together with Opiate withdrawal Schulenberg said on 4-20-16 at 8:17 pm that he had received a phone call from Phaedra Ellis-Lamkins and had an eleven minute conversation with her. Phaedra wanted to know if Schulenberg would talk with Dr. Kornfeld. Schulenberg said either Kirk or Prince had reached out to Phaedra to arrange chemical dependency Phaedra called Schulenberg to be the doctor of record on the issue. Phaedra wanted to know if he would be willing to talk to Dr. Kornfeld. Schulenberg gave Phaedra information about the of?ce visit and the medications that Schulenberg prescribed. Schulenberg was told that potentially Dr. Kornfeld may reach out to a Minnesota doctOr who could prescribe anti?withdrawal ?5m I ICR: 16012559 16. . 17. 18. 19. Schulenberg agreed to try and set up chemical dependency treatment for Prince. Schulenberg said he has medications. Schulenberg said the medication was Suboxone but he cannot prescribe it because he has not gone through the training for prescribing it. Schulenberg said Dr. Kornfeld was out in California and is an addiction medicine specialist. Phaedra said she was going to reach back out to Dr. Kornfeld and wanted to know if Schulenberg would speak with Dr. Kornfeld. Schulenberg then received a second phone call from Phaedra at 1047 pm, and Schulenberg, Phaedra and Dr. Kornfeld had a conference call. Schulenberg advised he talked brie?y with Dr. Kornfeld and explained what medications Schulenberg had given to Prince. Dr. Kornfeld agreed that the medications were appropriate medications to manage withdrawal. Dr. Kornfeld told Schulenberg that Dr. Kornfeld was sending his son to Minnesota to try and convince Prince to go to California for Opiate withdrawal treatment. Schulenberg said he then went back to bed and then started to receive text messages from Kirk at 1112 pm. Schulenberg adviSed he did not see the text messages until morning. Schulenberg got up and went to his of?ce at approximately 0815 am and ?nished his documentation on Prince?s notes. Schulenberg then contacted Kirk to ask if he was still in town. Kirk responded, I?m here waiting for him (Prince) to wake up to give him something to take the edge off. Kirk then told Schulenberg he was waiting for Andrew (Kornfeld) for help. Schulenberg then texted that he had Prince?s medical records and test results and wanted to know if he should fax them to Paisley Park. Schulenberg said Prince had given him verbal permission to communicate all test with Kirk. Kirk then wanted Schulenberg to e~mail the medical but Schulenberg said he couldn?t because he did not have a secure e-mail. Schulenberg said I?ll just drive over and Kirk said that would be good just to let Kirk know when Schulenberg was coming. Schulenberg texted Kirk at approximately 0930 am saying he would arrive at Paisley Park in approximately ten minutes. Schulenberg did not receive a text back and arrived on scene when rescue units were on scene. I asked Schulenberg if I could take pictures of the text messages between him and Kirk. Schulenberg consented andl took pictures later of the text messages with my work cell phone to document. I then asked Schulenberg if there was anything else he felt was important. Schulenberg said Kirk had contacted him on 4-14-16 asking if Prince could have some pain medication because his hip was bothering him. Schulenberg said he felt comfortable prescribing Prince a medication for 15 Percocet because he had seen Prince the week before. Schulenberg then heard about the emergency landing in Moline, IL. 011 4-18-16 Kirk had texted Schulenberg asking for Schulenberg to call him. During the phone conversation, Kirk told Schulenberg about the concerns he had for Prince and Opiate use. Schulenberg said Kirk apologized for asking Schulenberg to issue the Percocet for that and had only recently found out about Prince?s Opiate use. Schulenberg said Kirk told him that Kirk and a group had discussed the issue and learned from an ex- wife that Prince had Opiate issues for many years. I asked about medical records and if I would have to go through North Memorial and I was advised I would. Schulenberg said there is ?virtually nothing there.? Schulenburg said they did do lab tests and the only thing remarkable was mild anemia. At the end of their meeting at the Clinic, Prince asked to use the bathroom and Schulenberg received a urine sample from Prince. Schulenberg then ran a urine drug screen on Prince?s urine and it tested positive for Opiates, nothing else. Schulenberg said he set the sample out to con?rm what Opiate it was. Schulenberg said he has only had two contacts with Prince and was trying as best he could to help. everything documented on prescriptions and then said, take that back.? Schulenburg said he prescribed Zophrane and vitamin on 4-7-16. The prescription was in Kirk?s name because they are not controlled. Schulenberg then says, ?The only thing I regret, and I?ll be perfectly honest is that ah, Kirk said it would be better if I provided the Percocet prescription under his name.? Schulenberg said he did document the prescription in the electronic medical record under Kirk?s name. Schulenberg documented that the Percocet was being used for back pain. Schulenberg then tells me that he has known Kirk for sixteen years and said it was the ?rst time that he has ever prescribed a controlled medication under Kirk?s name. I then tried to con?rm that the prescription was on 4-14?1 6 and Schulenberg advised he thought it was but it ICR: 16012559 20. 21 22. 23. Joshua said Hannah was discovered on YouTube by Prince and was asked to play in his band. Joshua said she was married to Hannah so he moved with Hannah to Minnesota. Joshua said Prince was religious as would not be under Prince chart. Schulenberg then stated that he regretted doing that. Schulenberg then said, ?Butl don?t do things like that, actually it?s probably the first time I have ever done anything like that.? I again asked Schulenberg how many Percocet pills it was. Schulenberg said the prescription was Percocet 5-325 and 15 pills. Schulenberg said the prescription was given to Prince before he knew Prince had a problem. I was advised by Hannah that they had a doctor?s appointment and needed to get going soon. Hannah was pregnant and it was a doctor?s appointment for the baby. I decided to interview them together because of the appointment. I read Hannah and Joshua the recorded statement form and asked for a voluntary statement from them at 1145 on 4-21-16. Hannah and Joshua agreed to. provide a statement to me. 1 was Joshua and that is how they got along so good with each other. Joshua advised that he and Hannah met Prince in the year of 2012. Joshua said he produced a record with Prince later and worked with him on other projects over the years. I asked Joshua what had brought him and Hannah to Paisley Park today (4-21-16) and Joshua said Meron called him. Joshua said that basically he, Hannah, Kirk and Meron are the only ones that had been around with Prince lately. Joshua went to the movies on the previous Monday with Prince and the group. They went out to eat and watched a jazz performance at the Dakota. oshua and Hannah said that was the last time they saw Prince. I asked Joshua and Hannah if Prince made comments about the way he was feeling to them. Joshua, since he was close to Prince, felt like Prince was just ?cruising.? Joshua said ?you know when someone is vibrant and you know when they are ?low.? Joshua said Prince was towards the middle calling it cruising. I asked about if they knew if Prince had any issues with drugs. Joshua and Hannah said that their core values were not to be part of Prince?s band if Prince was into that. Joshua and Hannah stated they did not see any drug- use or know of any drug past with Prince. Joshua said the main way he communicated with Prince was through e-mail, almost daily. I asked Joshua and Hannah if they knew anything about the emergency landing that occurred in Atlanta, GA. Joshua said they only heard about the emergency from a friend in Atlanta but by the time the friend called, Prince was back in Minnesota. Joshua said Kirk had told them that Kirk made the call to land cause Prince was not himself or responsive. When they landed the paramedics came and Prince ?snapped back into it.? Prince wanted to get back home but another plane had to come and pick Prince up because of the pilot?s 12 hour rule. I asked what company the ?ight was booked through and was advised it was Signature by the airport. 24. I then asked about the phone call they received from Meron on 4-21-16. Joshua said Meron was 16012559 {my hysterical and just sard 25. 26. 27. 28. 29. 6? come to Palsley.? Joshua sald Kirk sent him a text message that said ?just come to Paisley.? I asked Hannah and Joshua who was at Paisley Park when they arrived. Hannah said Meron and Kirk. Joshua then said the doctor, who has not met, and Andrew who Joshua said he has never. met. Joshua included Kirk and Meron and then a bunch of Paramedics. I asked again about any drug use and Hannah and Joshua said they never observed anything other than Prince taking an Advil. Hannah and Joshua said from time to time Prince would consume alcohol but not to the point of intoxication. Hannah and Joshua said it was hard to understand what was going on. Hannah and Joshua said they were concerned about protecting Prince?s property and his legacy. I asked Joshua about security cameras and Josh said they were not working because Prince did not live in ?fear.? Joshua and Hannah advised they had no other information on the incident and I ended our conversation. I showed Joshua and Hannah the way out so they could leave for their doctor?s appointment. I then met with Detective Pat Murphy and asked if there was anyone else at the of?ce to interview. Detective Murphy advised he had talked with Meron but Meron did not want to provide a statement. Meron said it was because she had signed a con?dentiality agreement with Prince and did not want to violate the agreement. Detective Murphy said he gave Meron until 1300 to contact her attorney to see if she was willing to provide a statement. Detective Murphy asked if I would stay at the of?ce until 1300 to see if Meron would provide a statement. Detective Murphy advised Meron had walked outside the Chanhassen District Office to make the phone call via cell phone. I advised Detective Murphy I would wait and he and Detective Nucci cleared the of?ce. At approximately 1255 hrs, I walked outside to ?nd that Meron had left the area and was nowhere to be found. I called Meron?s cell phone but she did not answer. I left a voicemail and advised Meron if she wished to provide a statement to call me back. Meron did not call me back. I then contacted Sgt. Meier and advised him I was ?nished with the statements Sgt. Meier advised for me to return to Paisley Park because a search warrant was being drafted for Paisley Park. I returned to Paisley Park and while on scene, Meron arrived at Paisley Park. Sgt. Meier noticed Meron and stopped and talked with her. Sgt. Meier then advised Meron would give a statement and asked for me to conduct it. I escorted Meron into an of?ce area just off from the main entrance inside of Paisley Park. I asked Meron what she? did for Prince and she said she was his assistant. Meron said she did whatever Prince asked her to do, ?pretty much simple things.? I asked Meron if she observed any health issues with Prince over the past couple of weeks and Meron said he had the Meron said this 13 why Prince cancelled the show' 111 Atlanta on April 7th. I asked Meron what kind of schedule she had working as Prince?s assistance. Meron said she would come to work in the afternoon and has worked for Prince the last year and a half. Meron said Prince just talked about not feeling good because of the Meron said that the last couple of weeks Prince has been quiet and that was not normal. Meron did not know if Prince was on any medication and Meron said she had never bought him anything. Meron said she only bought Prince items for constipation and that was it. Meron said over the past couple of weeks the only people who were in Paisley Park Were the inner circle (Kirk, Meron, Hannah and Joshua). Meron said the last time she saw Prince was on 4?20-16 at approximately 1630 hrs. Meron arrived to go with Prince and Kirk to his doctor?s appointment but Prince told her not to come and he would just go with Kirk. Meron left, going to her room at the hotel, and came back to Paisley Park around 1900 to open the doors for the chef. Meron does not know if Prince ate his meal and she said she left Paisley Park around 2230 hrs. Meron said she did not have anything else to provide so I stopped the recorded conversation. I exited the room and met with Sgt. Meier advising him that Meron did not offer much information. At 1428 hrs, I Was advised'the search warrant for property was signed. At that time, the body of Prince was photographed and DNA swabs were taken from various parts of his body. I did not conduct any searching of the body and that action was performed by the Hennepin County Crime lab. Once Prince?s body was removed from the area by the elevator, Hennepin County Crime Lab began to photograph the entire I 30. 31. 32. 33. complex. All the detectlves and command staff on scene then had a meeting to discuss the best practice to search the large complex It was decided that detectives would start on the 2nd ?oor after it was photographed and then move to ?oor. On the 2"d ?oor, it appeared to have the bedrooms for Prince along with dressing rooms. The ?rst areal searched was what appeared to be the master suite of Paisley Park. Before entering into the room, I placed black latex gloves on my hands to not contaminate anything. The room had a dressing room area and a wood ?oor. The dressing room area was white with a blue ?sky? trim on the top. The room had a small like stage area and three steps leading'to the top of it. The dressing room area had a white vanity with white light bulbs all around the mirror. The counter was relatively clean but did have some clear drinking glasses on them. I did not start my search in the dressing room area. I moved from the dressing room area up the small white carpeted stairs (5) leading to the master suite. Inside the room I noted it was clean with a round bed with a top. The walls in the room were white with a purple trim toward the ceiling. In this room, I searched the closet in the far southeast wall. Inside the closet were ?le cabinets which were unlocked and a brief case. I opened the unlocked black brief case and inside was nothing to note for evidence. Inside. the ?le cabinets were pictures and some ?les. Nothing to note for evidence. I then moved along the south wall and searched behind the drapes. I searched a couch, coffee table and then another set of drapes. Nothing to note for evidence. I found a glass door that lead out to a patio area but it appeared it had not been used in some time. I came to an opening in the wall for a small area with a sink and cabinets. I was advised the area was already searched along with the bathroom and hallway leading to the bathroom. I returned to the northeast corner of the room and searched the closet in that area. Inside the closet was stereo equipment and nothing for evidence. I cleared the master suite and moved to the next room to the north on the eastside of the 2nd ?oor. This room was another dressing room that had clothing, shoes, hats and shelves with jewelry The room had a wood ?oor with white walls. When I entered the room, other detectives were inside starting their search. I asked if anyone had searched the white shelves and was advised no one had. I started my search in.this room with the shelves. The ?rst shelf had hats on it and was checked. I moved to the second shelf which had jewelry and a white box. I opened the white box and inside I could see it contained jewelry. I moved some of it because I observed white tissue paper, used for gifts, inside. I picked up the tissue paper and opened it. When I opened the paper, I located a number of white pills with the stamp of Watson 853 on them. I advised Detective Wagner of the ?nd and she looked up what the pills were on Drugs.com. Detective Wagner also had Dr. Quin Strobl (the Medical Examiner) look at the pills. The pills were identi?ed as Hydrocodone by Detective Wagner using the web page called Drugs.com. I requested that a Hennepin County Crime Scene Deputy come and photograph the pills. A Hennepin County Deputy came and the pills were photographed. I then counted the pills and there were 15 of the Hydrocodone pills. Detective Wagner and Dr. Strobl also counted and con?rmed there were 15 pills. The pills were then collected and placed into an evidence bag. I continued my search of the shelves and did not locate anything else. This was the only area in the dressing room that I searched and I cleared. The next room I was directed to search was what we labeled as the ?green room? on the second ?oor. I was advised by Detective Wagner that this was the room Prince used to sleep on a regular basis. When I entered this room a shower was 'to the right. Inside the room was a glass top desk against the wall on the left hand side with a chair. Then there was a bed in the middle with a night stand to the right of the bed. It appeared that someone had been sleeping in the bed at some time because the comforter was pulled back and open. On the far wall was another glass table, garbage can and then a small plug in heater. There was a T.V. that was hanging on the wall with an entertainment table underneath the TV. A bathroom was in the northwest corner of this bedroom that had a toilet and sink area. While in this room, Sgt. Meier advised me that he had located a small white pill lying on the ground. The pill was on the bed skirting and appeared to be the same as I had found in the dressing room. I located the pill and advised for a Hennepin County crime tech to photograph the pill. The pill was photographed and I collected it as evidence. On the small white pill, I observed the same marking of Watson 853 16012559 34. 35. 36. 3?7. (Hydrocodone) stamped on it. I then located a black hand carrying bag lying on the bed. I began to search the black bag and located a Passport for Prince Rogers Nelson inside. I also located a bunch of $100.00 dollar bills. The bills were removed and placed into an envelope to be placed in safe keeping by Sgt. Meier. Sgt. Meier counted the money and I continued to search. I continued to search the bag and did not locate anything else except jewelry/cosmetics and a nasal spray bottle. I moved my search over to the night stand on the right side of the bed. I located on the top of the stand two over the counter pain relievers. One was a Bayer container and the other Aleve. I opened the Bayer container and located Watson 853 (Hydrocodone) pills inside the container. I advised Detective Wagner and she counted the pills. Detective Wagner counter 64 1/2 pills and I con?rmed the count. I then looked into the Aleve bottle and located more white pills with the stamp of Watson 853 (Hydrocodone). I counted the pills and inside were 20 pills. Detective Wagner con?rmed the count of the pills. I contacted a Hennepin County Deputy to photograph the two bottles and contents. Detective Wagner collected the items and placed them into evidence bags. I continued to search the room and did not locate anything else. I continued to search other areas of the complex and did not locate anything else of interest. The search warrant on Paisley Park was completed at 1755 and detectives cleared. After we cleared Paisley Park, detectives returned to the Sheriff? Of?ce and looked at the cell phone numbers of the individuals we had taken statements from. Detective Pat Murphy went to the internet to search each phone number to locate which carrier provided service. It was determined that Schulenberg, Tyka Nelson and Joshua Welton all had Verizon Wireless as cell phone providers. DeteCtive Murphy sent Verizon Wireless preservation letters for the call history and text messages associated with the numbers. After this was completed, I cleared the Sheriff?s Of?ce and was out of service. Autopsy for Prince Rogers Nelson - On 4-22?16 at approximately 0850 hrs, Detective Angela Nucci and I arrived at the Midwest Regional Corner?s Of?ce in Ramsey, MN. Detective Nucci and I met with Medical Examiner Dr. Quin Strobel who was going to perform the autopsy of Prince. At approximately 0900 hrs, Dr. Strobel started the autopsy and noted that Prince was Wearing his clothes backwards. Dr. Strobel said Prince?s pants and shirts were on backwards. Prince was wearing a out long sleeve black shirt and a grey shirt with the sleeves cutoff under. Dr. Strobel said Prince was wearing boxer briefs but were on the correct way. A black stocking hat was also removed from Prince?s head. Dr. Strobel asked if the Sheriff 5 Of?ce wanted Prince?s clothes as evidence and I advised we did. The clothing was gathered and placed into evidence bags by Dr. Strobel and sealed. Dr. Strobel continued her autopsy and advised she did not see any trauma to the body of Prince. Dr. Strobel completed the autopsy at approximately 1300 and said it would be numerous weeks to determine a cause of death. Dr. Strobel had no other information to provide at the time and we cleared. Detective Nucci and I took custody of the evidence bags that contained Prince?s clothing at the time of death. Detective Nucci entered the items into evidence at the Sheriff?s Of?ce. Video from Walgreens Lead 14 On 4-22-16 at approximately 1330 hrs, I drove to the Walgreen?s on Hwy 101/ Hwy 7 to check if they had video of Kirk Johnson picking up medications or of Prince at the scene. I was advised by the Store Manager and Loss Prevention District Manager that they could not release the video. They advised they did have video but Management had looked the video so no associate could view or release it. I was given phone numbers of management to request the video for our investigation. I returned to the of?ce and called the phone numbers. I called a Phil Caruso and Emily Hartwig and left them messages as they were the media of?cial?s with the video. Neither of them answered so I left phone messages. I then sent an email to Phil Caurso and Emily Hartwig. At approximately 1944 hrs, I received an email from Terry Reiland and a phone call. I talked with Reiland and advised him I was seeking the video. I gave Reiland MET ICR. 16012559 the information I was requesting and Reiland asked for an Administrative Subpoena. 38. On 4-25-16, I wrote the Subpoena and it was signed by Asst. County Attorney David Hunt. I then faxed the Subpoena requesting all the information/video associated with the Prince Rogers Nelson, Meron NMN Bekure and Kirk Anthony Johnson on 4?20-16 or other dates. I was then contacted by Timothy Belka and advised that Walgreens had received the Subpoena and asked other questions. I answered Belka?s questions and he advised he would look for the video?s and would send them to me. On 4-27-16, I talked with Belka via phone and learned he had found three video?s from Walgreen stores. Belka advised. he would email me the videos with descriptions. 39. I received the email and the ?rst video was from a Walgreens located at 7845 Portland Ave S, in the city of Bloomington, MN 55420. The date of the video is from 4-14-16, the date the Percocet was filled and issued to Kirk Johnson via Dr. Schulenberg for Prince. The second video is from a Walgreens located at 4950 County Rd 101, Minnetonka, MN 55345 on 4- 20- 16. In this video, Kirk Johnson ?lls prescriptions, during Prince 5 doctor?s visit with Dr. Schulenberg, at 1749 and 13 by himself. The last video is from the same Walgreens location in Minnetonka, MN and hour later at 1904 after Prince?s doctor visit. I thanked Belka for the video and downloaded all videos to a CD, placing the CD into evidence at the Sheriff?s Of?ce. Prince?s Medical Records Ridgeview Medical Center Lead #28 40. On 4-26-16, I wrote a search warrant for Prince?s medical records located at the Hwy 212 Medical Center in Chaska, MN. On 427- 16, I met with the Honorable Judge Eric Braaten at approximately 1325 and he reviewed the search warrant. Judge Braaten reviewed the warrant and signed it I then called the Ridgeview Medical Center and talked with a Jennifer in records. I advised Jennifer I had a signed search warrant for Prince?s records and she advised she had already pulled the records. I drove to the hospital and served the warrant on 4-27?16 at approximately 1615 hrs. 1 received all of Prince?s medical records . from his visits with Ridgeview. 41. I received records that showed Prince?s medical emergency on 4-21-16. paramedics checked a box on their felm that showed Prince had vomit in his airway and his eyes were closed. In the narrative history on the paramedic? 5 record it reads, Pt. was taken yesterday by family to clinic doctor for? ??nervous and anxious. Pt. given meds to relax and per family Pt. stated ?it?s not enough.? Pt. last seen at his home last night and today found supine, not breathing. Fire arrived and pulled Pt. out of elevator and started CPR. Upon our arrival CPR in progress. Pt. limbs are stiff, arms bent and unable to straighten. CRP stopped due to rigor mortis. Pt. has vomit in mouth. Leads applied and asystole con?rmed. Med control contacted and TDD (time of death) con?rmed at 1007. Clear scene. I continued to read the record from 4-21-16 and learned that no AED was used and the death was unwitnessed. 42. I then found a medical record for Prince from 10-23-13. This record shows Prince was admitted into the emergency department at 1958 on 10-23-13 after being transported by ambulance. In this report, Prince complains about vomiting and dehydration. Prince was assisted to the ambulance on the gurney and transported to the emergency room. While at the hOSpital, the doctor treating Prince wrote in her report the following, Patient very with orthostatic vital signs and has elevated WBC (white blood count) with left shift. Bicarb is only 19 suggesting he has not been eating for some time and potassium low at 3 .4. Declined the Zofran offered as he would prefer to be as natural as possible. Liter of normal saline given and will give liter of D5 1/2 normal with of potassium. Instructed patient he will need to be able to keep at least some water down so will try a PO challenge when ready. Prince was then discharged from the hospital at 23 02 on 10-23-13. ICR: 16012559 .v?m'h - Medical Records from North Memorial Hospital Robbinsdale. MN Lead# 12 43. On 4-26-16, I called North Memorial records and asked to talk with a supervisor. I made contact with a 44. 45. 46. 47. 48. supervisor and I advised I had written a search warrant for the medical records of Prince. The supervisor advised he would pass the information onto the attorney who represents North Memorial. A short-time later I received a phone call from Attorney Joseph Dixon with Fredrikson and Byron. Dixon said he was representing North Memorial and was willing to help with my request. On 4-27-16, I arrived at the Hennepin County Government Center to meet with the Honorable Judge Daniel Mabley. I advised Judge Mabley I had a search warrant for the medical records for Prince at North Memorial. Judge Mabley read the application and signed the warrant. I swore the information was true to the best of my knowledge. I thanked Judge Mabley and cleared the courthouse. I met with Dixon on 4- 28-16 at approximately 0930 and discussed what information I was looking for at North Memorial. I showed Dixon a copy of the search warrant that asked for the records. Dixon and I talked for about an hour and agreed to what till the following week to execute the warrant so he could collect all the information requested. I cleared North Memorial. On 5-4-16, I received a phone call from Dixon advising he had all the records ready for pick-up. Dixon and I set a meeting for 5?5-1 6 at 1330 at North Memorial in Robbinsdale. On 5-5-16 at 1330 hrs, I served Dixon the search warrant. Dixon said he was able to find all the records for Prince and placed it into a ?le folder. Dixon also advised he located video of Prince, Kirk. and Dr. Schulenberg .at the North Memorial Clinic in Minnetonka. Dixon placed the video on a jump drive and gave it to me along with all the records. I continued to talk with Dixon and thanked him for his time. I returned to the Sheriff?s Office and started to review the video that was provided by Dixon of Prince?s last visit with Dr. Schulenberg on 4?20?16 just after 1700 hrs. In the video, Dr. Schulenberg is seen walking out the doors of the clinic and into the parking lot. Kirk and Prince are then seen walking with Dr. Schulenberg into the clinic. Prince is wearing black tight pants, black shirt, black coat and a stocking hat. Prince, Kirk and Dr. Schulenberg enter the clinic at approximately 1706 and walk out of sight. At 1738 hrs, Kirk is seen walking out of the clinic and then returns at 1805 hrs. At 1829 hrs, Dr. Schulenberg walks Prince and Kirk out of the clinic. . I reviewed the medical records for Prince and found a letter from Dixon. In the letter dated May 5, 2016, Dixon explains that North Memorial changed Prince?s name in their system to a ?Peter Vang.? Dixon advised North Memorial also changed Kirk ohnson?s name in the system to John Pellegrino. The names were changed to protect other employee?s from looking into the ?les. I then started to review the medical records for Prince. I started by looking at Dr. Schulenberg?s notes from the April 7th, 2016 visit. I learned the following from Dr. Schulenberg?s report. It reads as follows, Pt is 57 yo. year old male who is seen at his home for recent onset of a variety of complaints. He has been vomiting over the night and thought it might be related to something he ate. No fever is noted. No chills but did feel cold over night. No diarrhea. Not really and - abdominal pain and nausea is not prominent. Was concerned that he might need some IVF as these have helped in the past. Over night as well has noted numbness and tingling in the right leg below the level of the knee. N0 injury noted and has not had like this in the past. Feels like pins and needles but does not seem to be waking up like he would expeCt. Similar are noted in both hands. He is supposed to'leave town today for a show as well. Denies any back pain. Has history of hip pain. No changes in bowel or bladder function are noted. No neck pain. No other concerns were noted at the time of the home visit. He was brought to the office for IVF and during that time complained of feeling some odd sensation in the left eye, as if something was present in it. Feels he is out of balance at this time. Was ICR: 16012559 49. 50. 51. '52. 53. 54. 55. not Interested 1n any blood testing The notes for 4-7?16 say that Prince later came to the clinic and was given 1 liter of normal saline and it was administered through an IV in the right forearm. Dr. Schulenberg checked with Prince the next day and Prince said the numbness and tingling in both leg were resolved and clear. I then reviewed the medical records from 4-20-16. I learned the following from Dr. Schulenberg?s notes as he wrote them. ?Pt. is a 57 y.o. year old male who presents with a friend today (Kirk Johnson) for feeling antsy. started today. He thinks it might be related to stopping Tylenol, the last of which he took this morning. When questioned if the Tylenol contains any other ingredients he stated that he did not know. Independent sources of history have informed me the TylenOl also contains hydrocodone. There has been no nausea or vomitingthe neurologic he had the last time I had seen him. Denies any pain except some mild discomfort in the hip. No diarrhea. Just feeling antsy, anxious and in need of something calming. Feels some may help to restore balance; He is open to having some labs done today and expressed concerns about his adrenal glands due to persistent fatigue. Some hoarseness and weakness to his voice. Mild shakiness but no overt tremors. No fevers. No URI No cough. No chest pain. No shortness of breath. No palpitations.? I continued to read the notes and learned that Dr. Schulenberg ordered medications for Prince and they were Clonidine, Hydroxyzine Pamoate and diazepam (Valium). Dr. Schulenberg also had Prince complete a urine test which resulted 1n a positive test result for Opiates. The test was sent off and confirmed with the following results on 4- 25- 16. Positive for Hydrocodone 253 ng/ml and Hydromorphone 87 ng/ml for levels. On 4- 21? 16, Dr. Schulenberg added progress notes that stated the following, ?Will check some labs today. If the Tylenol indeed contains hydrocodone there 18 the concern of opiate withdrawal. Pt. did have 1 liter of NS infused 1n the office at his request and tolerated well without any complications?? "1 then looked at the medical records for Kirk Johnson pertaining to what Dr. Schulenberg admitted to me his statement. Dr. Schulenberg advised Kirk called wanting to get something for Prince cause his hip Was hurting. Dr. Schulenberg admitted to ordering Percocet for Prince but placing the medication in Kirk?s name. I looked at the report for Kirk and it showed that on 4-14-16 that Kirk did make contact with Dr. Schulenberg and medication of Percocet was approved. The following is Dr. Schulenberg?s medical notes pertaining to the phone call he had with Kirk on 4?14-16. ?Pt called and is leaving town for Atlanta for a concern shortly and hurt his back lifting equipment. Was wondering if he could get something. Reviewed of low back axial pain without alarm One time script for Percocet that he could pick up. FYI. I have known pt for 15 years and this IS a very rare request. Pt IS aware he will need to come and pick up script.? I ?nished looking through the medical records and entered them into evidence at the Carver County Sheriffs Of?ce. Medical Examiner found a Lethal amount of Fentanvl in Prince?s system On 5-9~l6, Sgt. Meier asked Detective Wagner and I to meet'in his of?ce. Detective Wagner and I arrived at Sgt. Meier?s office and learned that Prince had a very high amount of Fentanyl in his blood that was taken at autopsy. Sgt. Meier instructed Detective Wagner to start working on a new search warrant and to relay the information to Marc Lombardi who is working the case with us from the D.E.A. We met with Lombardi and discussed the information with him at our meeting which was at 1300 hrs. On 59-1 6 at 1610 hrs, Sgt. DeWitt Meier asked me if I had requested the blood for Prince when it was drawn on 4?20-16 at North Memorial by Dr. Schulenberg. I advised I did not but would call North Memorial. I called North Memorial?s Attorney, Joseph Dixon, and asked him about the blood and learned the blood was Sent to the Medical Examiner?s Office. I contacted Sgt. Meier via cell phone and relayed the information that the blood taken from Prince on 4-20-16 was at'the Medical Examiner?s Office. Sgt. ICR. 1601i559 Meier advised he would call the Medical Examiner to" "have the blood sent for further testing for Fenianyl. Sgt. Meier worked the details out with Dr. Strobel and the blood would be further tested for the Fentanyl. Search Warrant at Paisley Park with DEA 56. On 5?10-16, I was noti?ed by Detective Wagner that a new search warrant was drafted for Paisley Park 7801 Audubon Rd., Chanhassen, MN to look for the source of the entanyl. I was advised the DEA and our staff were going to meet at the Sheriff? Of?ce at approximately 1600 hrs. The DEA agents arrived at the Sheriff?s Of?ce and we had a brie?ng about the warrant and process. On 5-10-16 at 1744 hrs, Carver County Detectives and DEA agents arrived on scene at Paisley Park and served the warrant to Bremer Bank which was is the trustee for Paisley Park. The bank trustee looked at the warrant and unlocked the doors to Paisley Park. 57. Once inside Paisley Park, the DEA went ahead and started to number the rooms so we knew where items of signi?cance came from. When the DEA was ?nished numbering the rooms, we started our search of Paisley Park. I started off in room number #6 (The green room), which was where Prince slept. On the left side of the room, I located three small clear plastic bags. It was unknown if it contained anything but was collected for testing. I packaged the bags and brought them to Investigations assistant Nikki Schmidt to enter the items as evidence. I returned to room #6 and continued to search. I then found a red notebook with some writing on it of names and phone numbers of people I did not know. I found the red notebook on the left side of the bed on a chair. I asked Detective Wagner if she wanted to collect the red notebook and she advised ?yes? because of the phone numbers. The item was collected and placed into an evidence bag. - 58. I again returned to room number #6 and had DEA agent Marc Lombardi assist me in the search of the room. I found the bedding that was on Prince?s bed during the ?rst search warrant was already taken off the bed and placed into plastic bags. In another bag I located towels that used to be in the bathroom of Prince?s bedroom (Room 6). I inventoried the towels as such, 1 cream hand towel, 1 white hand towel, 1 tan bath towel, 1 peach towel, and one white bath towel. The towels were packaged and entered into evidence along with the bags of the bedding. DEA agent Lombardi was searching a black suitcase that was located on the ?oor of room #6 when he located in a CVS vitamin bottle 27 white oblong pills with the stamp Watson 853 on them. Agent Lombardi then located another, CVS vitamin bottle, with 49 white oblong pills with the stamp Watson 853 on them. Agent Lombardi also located 3 half pills that were of the white oblong type. The items'were collected by Agent Lombardi and packaged and placed into evidence. I continued searching and remembered from the ?rst search warrant that there were some Kleenexes on the right side of the bed on the The Kleenexes were still on the night stand so I collected them and packaged them. I turned them into evidence. I then ?nished in the room and moved to the lower level of Paisley Park to Room I. 59. I searched this small room (I) with DEA Agent Terrence Olstad. The only item that was collected was a copy of a con?dentiality agreement between Prince and a party named Evan Bakke. The paper was collected and placed into evidence. Agent Olstad and I then cleared the room and continued to search other areas. 60. Since the start of the search started earlier in the evening, it was decided to stop the search for the night and have two deputies arrive on scene to hold the residence. At approximately 2330 hours all agents and detectives cleared except Sgt. Meier and I. We stayed on scene and waited for Sgt. Michael Wollin and Deputy Carly Barrett to arrive to hold the scene. Sgt. Wollin and Deputy Barrett arrived at approximately 0000 hrs. The scene was taken over by them until the following morning. On 5-11-16, Sgt. Dave McDonald and Cpl. Kurt Schoening relieved Sgt. Wollin and Deputy Barrett at 0600 and were on scene when we arrived. On 5-11-16, DEA agents and detectives from the Carver County Sheriff? Office arrived on scene at approximately 0900 and continued our search of Paisley Park. Once we completed the search it was determined that ?the vault? needed to be searched. It was determined that the Sheriff?s Of?ce and DEA would continue to hold the scene until the safe was Searched. Numerous deputies were contacted and worked 24 hours a day to secure and hold Paisley Park in our control until the safe was opened. 61. On 5-12-16, the DEA and Sgt. Meier, Detective Wagner and I arrived on scene. We were advised the person who was going to open the safe had arrived and was getting ready to start working on the vault. We arrived and the adult male started drilling into the safe. Attorney?s that represented the brothers and sisters were in attendance along with the US. Attorney?s Of?ce and Bremer bank of?cials handling the trust. Once the vault was opened, Detectives and DEA agents entered and searched the vault. Inside the vault were numerous music recordings and personal items. I did not locate anything of evidence inside the vault. At approximately 1744 hours, keys for the building were delivered by a representative of Bremer Bank. It is my understanding Bremer Bank is the current representatives of Paisley Park Studios and estate. After keys were received Of?cers were able to gain into the building. After entry into the ilding, Of?cers split into different groups and conducted a room to room search of the entire building, searching for any persons who may be present on the property. It is noted there was private security personal on the property and their responsibility was to patrol the perimeter and exterior of the building 5. Paisley Park Studios is a large complex and has multiple rooms and storage areas. I was with a group of of?cers that cleared the main level, of?ces, storage rooms, the large concert hall, and part of the 2"d ?oor including the mechanical rooms. Clearing the building took approximately 30 minutes. 6. At approximately 1821 hours we had brie?ng regarding the search process, and instructions regarding the items we were searching for. We received instructions if items of interest were located to leave them in place and Detective Tyler Stahn (CCSO) would come and photograph the item before it was moved, and before it was packaged. The item would then be taken to the staging area near the main entry. This is the area where Detective Wagner and InvestigatiVe Assistant Nikki Schmidt would log the item. 7. We were also directed, to the best of our ability, to search our assigned areas with partners or in pairs. My search partner for this detail was Detective Amanda Grundeman. 8. We began searching at approximately 1835 hours. Between approximately 1835 hours and 2000 hours Detective Grundeman and I searched the following areas; the of?ce located on the 2??1 ?oor, on the Northeast side of the building; the master bathroom, and attached mechanical room located on the 2nd ?oor near Prince?s primary bedroom (the green room, room 9. We searched the Studio storage room, which contained a large number of audio and or video tapes. Also located in this room were (2) standard size refrigerators. These refrigerators were running. One was locked. Detective Grundeman was later able to gain access into the locked refrigerator and determined it also contained a number of audio or video recordings. 10. The second refrigerator was taped shut. Detective Grundeman was able gain access into this refrigerator. The bottom of this refrigerator contained audio and or video recordings. The freezer contained (2) wrapped spoons. It seemed unusual these spoons would be kept in the freezer. The spoons were collected by Detective Grundeman. 11. We also searched a small of?ce located at the main entry! lobby on the north side of the building. 12. At approximately 2005 hours we were instructed to take a break. Between 2005 and 2100 hours I assisted Detective Chris Wagner ?lling out property inventory forms, of items that were collected and seized. 13. At approximately 2105 hours we again began searching the building. Detective Grundeman was again my search partner. From 2105 to 2205 hours we searched the following areas; the Studio lounge located on the ?oor; a room identi?ed as room lounge which is located near the entry on the Northwest side of the building. 14. We searched room which is located on the ?rst ?oor, off Studio the studio with the drum set. We searched room located on the ?oor, the lounge located off Studio we searched room located on the floor, the piano room. 15. We searched room ?Al which is the large concert?hall located in the Southwest corner of the building. This is a large open area with at least (3) three performance areas and different seating areas. Located behind the main stage is a large lounge. 16. We searched the lounge. This area also has access to a kitchen, and two different sets of stairways that lead to the second ?oor. One stairway leads to a second floor bedroom. The second stairway leads to a large mechanical! furnace room and roof access. During the search of this area we did not ?nd anything of signi?cance. 17 . At approximately 2205 hours we were back at the main lobby staging area for a brie?ng. The decision by the supervisors was to suspend the search for the night and that we would continue the search the ICR: 16-012559 a. -.. following morning at 9am. 18. The supervisors were also working with Bremer Bank representatives and attorney?s to get someone to the property the following day to open a large safe door located in the basement of the complex. This door was located in the Vault room. Arrangements were made for deputies to be posted inside the building, holding the property for the night. 19. At approximately 2230 hours I cleared from Paisley Park Studios. 20. On 05-11?16 at approximately 0858 hours I was again at Paisley Park Studios located at 7801 Audubon Road in the city of Chanhassen. 21. At approximately 0900 hours we had a short brie?ng and were instructed to continue the search of the building as we had done the previous day. Mysearch partner was again Detective Amanda Grundeman. 22. BetWeen 0915 and 0930 we ?nished searching the second ?oor near the back stage lounge. This included the Salon located on the 2"Cl ?oor. 23. At approximately 0933 hours we began searching the lower level basement and underground garage. We searched the room labeled Vault. This room contained a large number of boxed audio and video recordings, computer hard drives, photographs, awards, memorabilia, and other items appearing to belong to Prince. There is also a large steel bank vault door located in this room. Currently we are unable to gain access to that room. 24. In the vault room we found a number of smaller plastic storage or shipping containers locked with padlocks. We were unable to locate the keys for the lock, and we were given approval/permission to cut the locks to view the contents. 25. At approximately 1010 hours I cut the lock on those containers. The containers were not damaged. Inside the containers were hard drives and other recording related equipment. 26. At approximately 1030 I was back at the staging area where Detective Wagner was collecting and packaging the items to be seized. 27. At approximately 1200 hours we began searching the (2) two dumpsters located on the south side'of the building. These dumpsters were relocated by the garbage hauler to the south east side of the building, near the underground garage access, to limit the potential for people to take photographs of of?cers while searching the dumpsters. The initial search group was Detective Grundeman, Detective Gramentz, Detective Sgt. Meier, and myself. 28. During the search of the dumpster a number of items of interest were located and taken to Detective Wagner to be packaged for possible processing. Please see the property inventory form for the items recovered from the dumpster. 29. Note: During the course of executing the search warrant the interior doors of Paisley Park Studios were marked with post it notes, identifying the room by number, letter, or by name. This system was used to help processing, and identify the individual rooms where items were located. 30. At approximately 1326 hours I assisted in removing the post it notes from the interior doors. 31. Additional information: The law enforcement supervisors, Bremer Bank representatives, and attorneys involved have been working to come up with an agreement on how to access the actual vault located in the vault room. 32. It is my understanding that currently no one has access, or the combination to the vault door. It is also my understanding, arrangements have been made with a company in California to send a lock technician from California to attempt to open the door. 33. Because of paperwork that needed to be completed, and the travel arrangements for the technician, the execution of the search warrant on the vault was going to be temporarily suspended. At approximately 1350 hours I went to the vault room with Detective Sgt. Meier and Attorney Ruth Rivard, to mark the vault door with evidence tape. Again the purpose of marking the door is to show the door was secured, and IS to remain closed until the interested parties from the Sheriff?s Of?ce, the DEA, and representatives of Bremer Bank and Attorneys were available and present. I attached both red and white evidence tapes to different location on the door and door jam of the vault. This was. witnessed by Sgt. Meier and Attorney Ruth Rivard, representative of Bremer Bank. I previous verified the Vault door was locked and secure. After the evidence tape was applied I also took photographs with my county issued cell phone showing the locations of the evidence tape, and to show the tape was not broken. Attorney Ruth Rivard also took photographs with her phone, for her records, also verifying the locations of the tape. Meier explained that the Drug Enforcement Administration (DEA) was Obtaining the search warrant, and we would join the search and assist. 2. At approximately l400hrs, I was requested by Sgt. Meier to drive to Paisley Park and wait in the area. Sgt. Meier requested that I observe the area for any persons, and or vehicles leaving the premises. 1 took the time to walk around the fenced area, and noted several vehicles inside the premises. NO vehicles left the premises, except for security guards going from the south to the north gate and vice versa. 3. At approximately l700hrs, I observed a group Of unmarked squads approach Paisley Park I followed the vehicles inside the premises. Upon arrival, I met with CCSO Detectives Wagner, Stahn, Murphy, Grundeman, Gramentz, Kuhnau, Nelson, Sgt. Meier, and a group of DEA Agents. While waiting for keys 'tO the building, we were briefed in how we would coordinate efforts to search the premises. It was also explained that we were looking for any prescription medication, control substance and or any documentation that may indicate or contain information about Prince Roger Nelson?s use of controlled substances. 4. During the brief it was explained that there was reason to believe that Prince Rogers Nelson may have been using Pentanyl. Fentanyl is an Opioid designated as a Schedule 11 controlled substance. Fentanyl is commonly found in transderrnal patch form. Persons can soak the patch in a liquid solution such as water, juice and other liquids in order to have its properties transfer from the medication gel contained within the patch, into the solution. Users can ingest, snort, or have the fentanyl solution administered via suppository. Fentanyl could also be found as a pill form, powder and or injection. 5. At approximately l744hrs, we made entry to the Paisley Park Studios building, and began clearing the . premises of any persons. Once it was con?rmed that there were no persons inside, we all reconvened at the center of the building to coordinate the search. I teamed with DEA Controlled Substances Technical Specialist Ginny Mckenna to conduct our search. - 6. Wagner at Paisley Park. Det. Stahn advised to also bring a laptop and evidence bags. I then removed SW Item #1 from the evidence garage, and transported it to Paisley Park. 11. There was no answer and Detective Wagner was unable to leave a voice mail due to the mail box not being set up. 4. On 05/04/2016 at approximately 1500 hours I went to Larry Graham?s house, 2110 Clover Ct, in the City of Chanhassen to attempt to meet with Larry Graham. While standing outside a house I notice a van drive by the address two times. The driver of the van appeared to be Larry Graham, but didn?t stop as I attempted to wave at him to stop. I got in my vehicle to leave and the van stopped near me and asked if they could help me. I explained to the driver that I was looking for Larry Graham. The driver stated he could help me contact Larry Graham. I pulled over to the shoulder of the road and went back to the van and spoke to the male. The male then stated he was Larry Graham. Graham stated at ?rst he didn?t recognize that I was with the sheriff?s of?ce. Graham apologized for driving past the ?rst two times. Graham stated he was willing to talk to me so I followed Graham back to his house. Graham let me into the house and we talked while we sat on the couch in the living room area. 5. I recorded the conversation that I had with Graham. Graham was identi?ed by MN DL as Larry Graham asked Graham to explain to me his relationship with Prince. Graham stated he has known Prince for 20 years. Graham stated his relationship with Prince started out as musicians that met while on tour. I completed an administrative subpoena for the phone records and faxed the subpoena to Integra Telecom on 04/27/2016 at 0907 hours. I asked Graham how often he had seen Prince 1n the last six months. Graham started talking about seeing Prince on March 23?. Graham explained that he had received a text/email about meeting Prince' 1n Canada. Graham stated Prince was on tour in Canada. On 04/27/2016 at approximately 1022 hours I called the phone number for Ray and spoke with him by phone. Ray agreed to meet with me on 04/28/2016 at 1000 hours at his place of employment, Galleria, in Edina, MN. 28. On 04/28/2016 2111028 hours I met with Raymond Russell Roberts DOB "at his restaurant, Peoples Organic 3545 Galleria, Edina, MN. I recorded my conversation with Roberts. 1 spoke with Roberts in his of?ce. 29. I asked Roberts what his relationship was with Prince. Roberts stated he has been Prince?s personal chef since August of 2013. Roberts explained Roberts would daily make dinner (an appetizer, an entree, and desert) for Prince and stock his refrigerator. Roberts stated when Roberts wasn?t there Prince would have someone bring Prince food or Roberts would leave food at Prince?s house. 30. I asked Roberts if he was at Prince?s studio on Wednesday, April 20, 2016. Roberts stated Prince?s assistant, Meron Bekure, told Roberts to bring Prince?s food, packed up. Roberts explained that from time to time Prince would request his food be brought prepared, and packaged. Roberts. stated on other nights he would prepare/cook Prince?s meals at his house. Roberts stated he was at the studio around 1930 hours. Roberts stated he got to the'studio early and was waiting to get into ICR: 16012559 the st11d1o. Roberts stated K1rk Johnson and Prince arrived at the studio and pulled into the studio. They waved Roberts 1n. Roberts stated Johnson and Prince went to the rear entrance. Roberts stated Johnson walked through the studio and let Roberts into the front entrance. 31. Roberts stated he left Prince?s food in a kitchen that Roberts described as ?Roberts? kitchen per ohnson?s request. Roberts explained that there is a public side to the studio and Prince?s private area. Roberts stated that while he was at the studio, Bekure and Johnson were sitting in the media room around a table. Roberts stated they were talking. Roberts stated he never went to Prince?s private living area on that night. Roberts stated he normally would go to Prince?s private living area. Roberts stated that night Prince ?didn?t seem good?. Roberts stated Prince looked, ?distressed? when Roberts saw Prince arrive with Johnson. Roberts stated Prince looked like he wasn?t feeling good. When Prince and Johnson arrived, they were in a black colored suburban. Roberts stated Prince was in the front passenger seat and when they pulled up Prince?s window was next to Roberts?s driver?s window. I asked Roberts about Prince 3 health. Roberts stated Prince 3 behavior was up and down. Roberts Stated Prince would be happy and energetic and then he would be sleeping or not feeling well. During the conversation Ramadan stated she wanted to verify who I was because she was leery of phone calls because media was calling her. I told Ramadan that she could obtain the sheriffs office phone number on the website and call the dispatch number to con?rm that I was a detective with the sheriff of?ce. I asked Ramadan when the last time Prince had communicated with Ramadan was. Ramadan stated she had received emails from other people after Prince?s plan had made an emergency landing on April 15, 2016. Ramadan stated she had emailed Prince saying that she was ?sending love? and checking on Prince. Ramadan stated Prince replied by saying, ?love received? and ?sending my love?. Ramadan stated Prince didn?t elaborate on what had happened to cause the emergency landing. At approximately 1512 hours Ramadan returned my phone call. I asked Ramadan about any health issues. Ramadan wasn?t aware of any health issues that Prince had. I asked Ramadan about any drug use and Ramadan stated she wasn?t aware of any drug use. I asked Ramadan if Prince was using any prescription medications and Ramadan stated that she wasn?t aware of any medications Prince was using. Ramadan stated Prince would use vitamins from ?Lakewind? grocery store. I asked Ramadan who else I should talk to about Prince. Ramadan suggested that I talk to Kirk Johnson, Meron Bekure and Phaedra Ellis- Lamkins. After the phone conversation I provided Detective Wagner With the email address that Ramadan stated Prince had been using to communicate with Ramadan. Assist on Search Warrant 05f10/2016 On 05/10/2016 at approximately 1744 hours, I assisted on an execution of a search warrant at Paisley Park, 7801 Audubon Rd, Chanhassen, MN 5317. Prior to searching an investigator labeled the rooms with a'Post-it note. The rooms were labeled by number, alphabet, and number alphabet combination. During the search warrant Detective Ben Gramentz and I searched rooms together. The ?rst room we searched was Conference Room on the top ?oor; the room was labeled as ?Room An Aquea Hydrate PH9 blue transparent bottle that was half full. of a clear liquid was located on the bottom shelf on the middle cabinet in the room. Detective Stahn photographed the item. Detective Gramentz and I collected the bettle and brought it to Investigative Assistant Nikki Schmidt to be logged into evidence. On 4?21-16 at approximately 1000 hrs, I was requested by Sgt. DeWitt Meier to respond to Paisley Park, 7801 Audubon Rd in the city of Chanhassen, to assist with a male down at the complex. I was advised the male down was Prince Rogers Nelson I gathered my investigative equipment and responded to the scene. I arrived at approximately 1015 and I entered the front entrance facing to the north and observed some people gathered in a lobby area inside Paisley Park. I proceeded to meet with Lt. Eric Kittelson, Sgt. Jason Breunig, Detective Christina Wagner and Deputy Adam Hastings. I met with the three deputies in the hallway by the elevator. 1 was advised that Prince Rogers Nelson was deceased and observed him lying on the ?oor 1n front of the elevator. - - 2. Deputy Hastings gave me a brief scenario of what actions were taken when Prince was located. Deputy Hastings advised Prince was found inside the elevator by the Chanhassen Fire Chief and pulled out of 1t. CPR was started by Chanhassen Fire but Ridgeview Paramedics arrived, advising ?re could they could stop CPR. Ridgeview Paramedics called the time Of death at 1007 hrs. Sgt. Meier arrived on scene and Detective Wagner and I met with him. Shortly after, Detectives Pat Murphy and Angela Nucci arrived on scene and we all met in the front lobby area to discuss the incident. It was determined that I, Detective Murphy and Detective Nucci would take the parties that were at Paisley Park back to the district of?ce in Chanhassen to conduct interviews. Detective Murphy advised the parties the interviews would be held at the Chanhassen Of?ce and we walked out with the ?ve parties. The parties were not identi?ed yet but the group consisted of three males and two females. 3. I exited out of Paisley Park at 1045 with Detectives Murphy and Nucci. The other ?ve parties followed us to the Chanhassen of?ce because they had vehicles at Paisley Park. One female who was distraught was escorted with Detective Murphy 1n his patrol car. We all arrived at the Chanhassen City Hall and the Carver County District Of?ce at 1050 hrs. The parties were escorted 1n and I started my ?rst interview with Dr. Michael Todd Schulenberg who arrived on scene at Prince? 3 residence. LEAD 5 Dr. Michael Todd Schulenberg (Home) (Cell) 4. I conducted my interview with Schulenberg in the Chanhassen Sgt.?s room. I received Schulenberg?s information and ?lled out my recorded statement form and advised Schulenberg that I was asking for a voluntary statement from him. Schulenberg agreed to speak to me and understood the conversation was being recorded and he was not under arrest. 5. I learned the following from Schulenberg during my interview with him. Schulenberg advised he knew Prince from a professional standpoint. I asked Schulenberg how he became involved with treating Prince for medical issues. Schulenberg said he has known Kirk Anthony Johnson ~for approximately Revised 05/06 10. 11. I the past-lo years because Kirk was a patrentof Schulenberg Schulenberg said last fall Kirk contacted him to see if Schulenberg would be open to helping P1ince, Kirk?s boss. Schulenberg said at that time, the idea was to just have a physical and to check Prince out. Schulenberg said nothing came to light at that time until about 2 weeks ago. On April 7th 2016 Schulenberg received a text message from Kirk at 0545 asking if Schulenberg would be able to help out by administering some I. V. ?uids to Prince. Schulenberg talked with Kirk and at approximately 0645 hrs, Schulenberg arrived at Paisley Park. Schulenberg met Prince and completed a physical exam on Prince. Schulenberg then asked about Prince?s past medical history. Since Schulenberg could not administer the I.V. ?uids to Prince at Paisley Park, Schulenberg arranged for Prince to meet Schulenberg at his of?ce. Schulenberg said his of?ce is located 'at15450 Hwy 7, Minnetonka, MN 55345. Schulenberg said later that morning, Kirk brought Prince to the North Memorial Clinic and Meron, (later identi?ed as Meron NMN Bekure met them at the of?ce. Schulenberg said for privacy reasons, Plince, Kirk, Meron and Dr. Schulenberg entered through the back of the building and came in the Urgency part of the clinic because it was not open for patient care. Schulenberg placed an I. V. in Prince and gave Prince a liter of normal I V. saline. Once the I. V. of saline was completed on Prince, Prince advised he was feeling better and left with Meron. Schulenberg said two days later (April 8th) Prince called to let Schulenberg know that he was feeling better. Schulenberg then received a text message from Meron (around lunch time) asking Schulenberg if Prince could call him. Schulenberg said Prince did call him and they had about a 15 minute conversation. Schulenberg reviewed with Prince some of the and some did Ilmprove. Prince then complained about some new because the previous night (April 7 h) a chiropractor had worked on Prince and new developed. Schulenberg then offel ed to see Prince again and offered to do some blood work if Prince felt it was necessary. According to Schulenberg,'his next contact with Prince was on the 21St of April. NOTE: Prince passed on 4-21?16 and Schulenberg says in the recording that it was yesterday which would have been 4-20-16. Schulenberg said he was contacted again via text message from Kirk. Schulenberg says he talked with Kirk on Wednesday about having Prince come in for some blood work at 7pm. Schulenberg said Kirk got in touch with him again on April 20th and Kirk brought Prince to the Hwy 7 medical building at 5pm fo1 blood work. Schulenberg then says, ?In the interim I had learned from Kirk, who stated to me, that he had justlearned this, ah, concern that, ah, Prince Was using, ah, opiates.- I asked if Kirk told him how much'Prince was using and Schulenberg said Kirk did not know. Schulenberg said Kirk had told him that Kirk had been 1n contact with a lot of people that knew Prince about this but Kirk advised he was not aware. I then asked Schulenberg about the that Prince was experiencing during his ?rst contact with Prince on April 7th. Schulenberg said Prince was complaining of numbness, tingling in one of his legs (right), numbness and tingling in his hands and that he had some vomiting the night before. Schulenberg said Prince attributed the vomiting to some soup he had eaten. I asked Schulenberg if Prince mentioned anything about taking any opiates. Schulenberg said heasked Prince if he was on any prescription medications and Prince responded no. Schulenberg then asked Prince again if he was taking anything for his hands and Prince told Schulenberg he had a medication but did not know what it was. I asked Schulenberg if he prescribed anything to Prince and Schulenberg said he did not on April 7th but did on April 20th for opiate withdrawal Schulenberg then goes back to explain what procedures were completed on Prince on April 20th. Schulenberg said Prince came in at 5pm and explained that time worked because there were no patients in the clinic. Schulenberg met Prince, took Prince?s vitals and did a medical history. Schulenberg also said he completed a physical examination on Prince. Schulenberg asked Prince about doing blood work and Prince consented to have blood work completed. Schulenberg had a nurse come in for the blood draw and to place another Schulenberg then gave Prince another liter of saline. Schulenberg said he asked 12@3er warn Prince if he was taking any medication nberg he had taken Tylenol. Schulenbel asked Prince if there were any other ingredients 1n the Tylenol and Schulenberg advised he believed Prince said no or that he did not know. Prince told Schulenberg that he had taken the Tylenol earlier that morning. Schulenberg said Prince?s main complaint was that he was feeling antsy. Prince denied any nausea, vomiting or diarrhea but was asking about opiate withdrawal I asked Schulenberg if the blood test results had come back for Prince and Schulenberg con?rmed they did. Schulenberg then starts to talk to me about how he arrived at Paisley Park the morning of Prince?s death. Schulenberg said he had administered the liter of IV. ?uids and sent a prescription to the Walgreens 011 Hwy 101 and Hwy 7 to be ?lled on 4-20-16. Schulenberg said the prescription was for Clonidine. Schulenberg said Kirk then left to go and pick up the Clonidine. Schulenberg said he had given Prince two doses of Clonidine before Prince had left along with one dose of Hydroxyzine. Schulenberg noticed that I was having a' hard time spelling Clonidine and Hydroxyzine and asked ifI needed him to spell them. I said ?yes? and Schulenberg spelled Clonidine and provided me with an explanation as to what it does. Schulenberg described Clonidine as a blood pressure medication that has for many years been used to treat opiate withdrawal Schulenberg described Hydroxyzine as an antihistamine similar to Benadryl. Schulenberg gave Prince the dose of Hydroxyzine because Prince felt like he needed to 1elax. Prince asked if Hydroxyzine could be given through the I. V. and Schulenberg did not want to make ?the problem worse by giving some other control medication intravenously 1n the office.? I asked if the medication was given in pill form and Schulenberg said ?yes,? Clonidine 0.1 and the Hydroxyzine was 25 milligrams. I asked Schulenberg when Prince left if Prince seemed to be in good spirits. Schulenberg said ?yeah, he appeared comfortable and he left with Kir and that was around-1830 hrs. I then asked Schulenberg why someone would want to have an IV. of Schulenberg said most people just feel better and Prince was mildly dehydrated saying Prince?s mouth was dry. Schulenberg stated he gave Prince a liter of normal saline because that is what Prince wanted. Schulenberg advised the potential for harm performing this procedure is very low. I asked Schulenberg if the prescriptions were ?lled and Schulenberg said the he only knew the prescription for Clonidine was ?lled. Schulenberg said the other two prescriptions were printed and did not know if they were ?lled. Schulenberg advised one prescription was for the Hydroxyzine; Schulenberg then researched Opiate withdrawal, and issued the second prescription for valium Schulenberg said he had a brief discussion with Prince about Kirk being a long time patient of his did not want to violate trust. Prince asked what the medications were and Schulenberg told him. Schulenberg said it was Kirk?s preference not to have the valium and Kirk Wanted the valium to be administered 1n a supervised setting. Schulenberg said it was his responsibility to be a good doctor and didn?t want to substitute one problem for another.? Prince said, ?Understood, I will use the other two preseriptions ?rst.? I asked Schulenberg what effects the valium would have on a person 3 system. Schulenberg said the type of in Prince? settings was primarily for anxiousness jitteriness, and ?it?s widely used as a tranquilizer and muscle relaxant.? Schulenberg said he prescribed what he felt was reasonable short-term. Schulenberg prescribed Prince 30 capsules of the Clonidine and 30 capsules of the Hydroxyzine- and for the Valium it was just 15 tablets. I asked if you were to mix all three of the pills together if it could cause health problems. Schulenberg said only if you intentionally overdosed on them. Schulenberg said all three are routinely taken together with Opiate withdrawal Schulenberg said on 4-20-16 at 8:17 pm that he had received a phone call from Phaedra Ellis-Lamkins and had an eleven minute conversation with her. Phaedra wanted to know if Schulenberg would talk with Dr. Kornfeld. Schulenberg said either Kirk or Prince had reached out to Phaedra to arrange chemical dependency Phaedra called Schulenberg to be the doctor of record on the issue. Schulenberg was told that potentially Dr. Kornfeld may reach out to a Minnesota doctor who could prescribe anti-withdrawal medlcatlons Schulenberg said the medlcat1on was Suboxone but he cannot prescribe it because he has not 16. 17. 18. would. Schulenberg said there is ?virtually nothing there.? Schulenburg said they did do lab tests and the 19. gone through the training for prescribing it. Schulenberg said Dr. Kornfeld was out in California and IS an addiction medicine specialist. Phaedra said she was going to reach back out to Dr. Kornfeld and wanted to know if Schulenberg would speak with Dr. Kornfeld. Schulenberg then received a second phone call from Phaedra at 1047 pm, and Schulenberg, Phaedra and Dr. Kornfeld had a conference call. Schulenberg advised he talked brie?y with Dr. Kornfeld and explained what medications Schulenberg had given to Prince. Dr. Kornfeld agreed that the medications were appropriate medications to manage withdrawal. - Dr. Kornfeld told Schulenberg that Dr. Kornfeld was sending his son to Minnesota to try and convince Prince to go to California for Opiate withdrawal treatment. Schulenberg said he then went back ?to bed and then started to receive text messages from Kirk at 1112 pm. Schulenberg advised he did not see the text messages until morning. Schulenberg got up and went to his of?ce at approximately 0815 am and ?nished his documentation on Prince?s notes. Schulenberg then contacted Kirk to ask if he was still 111 town. Kirk responded I here waiting for him (Prince) to wake up to give him something to take the edge off. Kirk then told Schulenberg he was waiting for Andrew (Kornfeld) for help. Schulenberg then texted that he had Prince?s medical records and test results and wanted to know if he should fax them to Paisley Park. Schulenberg said Prince had given him verbal permission to communicate all test results/concerns with Kirk. Kirk then wanted Schulenberg to e-mail the medical records/test but Schulenberg said he couldn?t because he did not have a secure e-mail. Schulenberg said I?ll just drive over and Kirk said that would be good just to let Kirk know when - Schulenberg was coming. Schulenberg texted Kirk at approximately 0930 am saying he wOuld arrive at Paisley Park- in approximately ten minutes. Schulenberg did not receive a text back and arrived on scene when rescue units were on scene. - I asked Schulenberg if I could take pictures of the text messages between him and Kirk. Schulenberg consented and I took pictures later of the text messages with my work cell phone to document I then asked Schulenberg if there was anything else he felt Was important. Schulenberg said Kirk had contacted him on 4- 14- 16 asking if P11nce could have some pain medication because his hip was bothering him. Schulenberg said he felt comfortable prescribing Prince a medication for 15 Percocet because he had seen Prince the week before. Schulenberg then heard about the emergency landing in Moline, IL. On 4-18-16 Kirk had texted Schulenberg asking for Schulenberg to call him. During the phone conversation, Kirk told Schulenberg about the concerns he had for Prince and Opiate use. Schulenberg said Kirk apologized for asking Schulenberg to issue the Percocet for that and had only recently found out about Prince?s Opiate use. Schulenberg said Kirk told him that Kirk and a group had discussed the issue and learned from an ex- wife that Prince had Opiate issues for many years. I asked about medical records and if I would have to go through North Memorial and I was advised I only thing remarkable was mild anemia. At the end of their meeting at the Clinic, Prince asked to use the bathroom and Schulenberg received a urine sample from Prince. Schulenberg then ran a urine drug screen on Prince?s urine and it tested positive for Opiates, nothing else. Schulenberg said he set the sample out to con?rm what Opiate it was. Schulenberg said he has only had two contacts with Prince and was trying as best he could to help. Schulenberg agreed to try and set up chemical dependency treatment for Prince. Schulenberg said he has everything documented on prescriptions and then said, take that back.? Schulenburg said he prescribed Zophrane and vitamin on 4~7-l6. The prescription was in Kirk?s name because they are not controlled. Schulenberg then says, ?The only thing I regret, and I?ll be perfectly honest is that ah, Kirk said it would be better if I provided the Percocet prescription under his name.? Schulenberg said he did document the prescription in the electronic medical record under Kirk?s name. Schulenberg documented that the Percocet was being used for back pain. Schulenberg then tells me that he has known Kirk for sixteen years and said it was the first time that he has ever prescribed a controlled medication under Kirk?s name. I again asked Schulenberg how many Percocet pills it was Schulenberg said the prescription was Percocet 5- 325 and 15 pills. Schulenberg said the prescription was given to Prince before he knew Prince had a problem. Hannah'Jeanne Walton .303? Joshua Aaron?Michael welton (DOB- Joshua Cell ?5 Hannah Cell a After I completed my follow up with Dr. Schulenberg, I entered the conference room andmet with Hannah and Joshua Welton. I was advised by Hannah that they had a doctor?s appointment and needed to get going soon. Hannah was pregnant and it was a doctor?s appointment for the baby. I decided to interview them together because of the appointment. I read Hannah and Joshua the recorded statement form and asked for a voluntary statement from them at 1145 on 4-21-16. Hannah and Joshua agreed to provide a statement to me. Joshua said Hannah was discovered on YouTube by Prince and was asked to play in his band. Joshua said he was married 10 Hannah so he moved 'with Hannah to Minnesota. Joshua said Prince was religious as was Joshua and that is how'they got along so good with each other. Joshua advised that he and Hannah met Prince in the year of 2012. Joshua said he produced a record with Prince later and worked with him on other projects over the years. I asked Joshua what had brought him and Hannah to Paisley Park today (4?21?16) and Joshua said Meron called him. Joshua said that basically he, Hannah, Kirk and Meron are the only onesthat had been around with Prince lately. Joshua went to the movies on the previous Monday with Prince and the group. They went out to eat and watched a jazz performance at the Dakota. cshua and. Hannah said that was the last time they saw Prince. I asked Joshua and Hannah if Prince made comments about the way he was feeling to them. Joshua, since he was close to Prince, felt like Prince was just ?cruising.? Joshua said ?you know when someone is vibrant and you know when they are ?low.? Joshua said Prince was towards the middle calling it cruising. .I asked about if they knew if Prince had any issues with drugs; Joshua and Hannah said that their core values were not to be part of Prince?s band if Prince was into that. Joshua and Hannah stated they did not see any drug use or know of any drug past with Prince. Joshua said the main way he communicated with Prince was through e-mail, almost daily. I asked. Joshua and Hannah if they knew anything about the emergency landing that occurred 111 Atlanta, GA. Joshua said they only heard about the emergency from a friend in Atlanta but by the time the friend called, Prince was back in Minnesota. Joshua said Kirk had told them that Kirk made the call to land cause Prince was not himself or responsive. When they landed the paramedics came and Prince ?snapped back into it.? Prince wanted to get back home but another plane had to come and pick Prince up because of the pilot?s 12 hour rule. I asked what company the ?ight was booked through and was advised it was Signature by the airport. 25. 26. 27. 28. '29. 1. ?35 3 g1 i5"? ICR: 16012559 hysterical and just sa1d come to Paisley Joshua said Kirk sent him a text message that said? just come to Paisley.? I asked Hannah and Joshua who was at Paisley Park when they arrived. Hannah said Meron and Kirk. Joshua then said the doctor, who has not met, and Andrew who Joshua said he has never met. Joshua included Kirk and Meron and then a bunch of Paramedics. I asked again about any drug use and Hannah and Joshua said they never observed anything other than Prince taking an Advil. Hannah and Joshua said from time to time Prince would consume alcohol but not to the point of intoxication. Hannah and Joshua said it was hard to understand what was going on. Hannah and Joshua said they were concerned about protecting Prince?s property and his legacy. I asked Joshua about security cameras and Josh said they were not working because Prince did not live in ?fear.? Joshua and Hannah advised they had no other information _on the incident and I ended our conversation. I showed Joshua and Hannah the way out so they could leave for their doctor?s appointment. I then met with Detective Pat Murphy and asked if there was anyone else at the of?ce to interview. Detective Murphy advised he had talked with Meron but Meron did not want to provide a statement. Meron said it was because she had signed a con?dentiality agreement with Prince and did not want to violate the agreement. Detective Murphy said he gave Meron until 1300 to contact her attorney to see if she was willing to provide a statement. Detective Murphy asked if I would stay at the of?ce until 1300 to see if Meron would provide a statement. Detective Murphy advised Meron had walked outside the Chanhassen District Of?ce to make the phone call via cell phone. I advised Detective Murphy I would wait and he and Detective Nucci cleared the of?ce. At approximately 1255 hrs, I walked outside to ?nd that Meron had left the area and was nowhere to be found. I called Meron? 3 cell phone but she did not answer. I left a voicemail and advised Meron if she wished to provide a statement to call me back. Meron did not call me back. I then contacted Sgt. Meier and advised him I was ?nished with the statements. Sgt. Meier advised for me to return to Paisley Park because a search warrant was being drafted fer Paisley Park. I returned to Paisley Park and while on scene, Meron arrived at Paisley Park. Sgt. Meier noticed Meron and stopped and talked with her. Sgt. Meier then advised Meron would give a statement and asked for me to conduct it. I escorted Meron into an of?ce area just off from the main entrance inside of Paisley Park. I asked Meron what she did for Prince and she said she was his assistant. Meron said she did whatever Prince asked her to do, ?pretty much simple things.? I asked Meron if she observed any health issues with Prince over the past couple of weeks and Meron said he had the Meron said this is why Prince cancelled the show 1n Atlanta on April 7th. I asked Meron what kind of schedule she had working as Prince?s assistance. Meron said she would come to work in the afternoon and has worked for Prince the last year and a half. Meron said Prince just talked about not feeling good because of the Meron said that the'last couple of 'weeks Prince has been quiet and that was not normal. Meron did not know if Prince was on any medication and Meron said she had never bought him anything. Meron said she only bought Prince items for constipation and that was it. Meron said over the past couple of weeks the only people who were in Paisley Park were the inner circle (Kirk, Meron, Hannah and Joshua). Meron said the last'time she'saw Prince was on 4-20-16 at approximately 1630 hrs. Meron arrived to go with Prince and Kirk to his doctor?s appointment but Prince told her not to come and he would just go with Kirk. Meron left, going to her room at the hotel, and came back to Paisley Park around 1900 to open the doors for the chef. Meron does not know if Prince ate his meal and she said she left Paisley Park around 2230 hrs. Meron said she did not have anything else to provide 301 stopped the recorded conversation. I exited the room and met with Sgt. Meier advising him that Meron did not offer much information. At 1428 hrs, 1 was advised the search warrant for property was signed. At that time, the body of Prince was photographed and DNA swabs were taken from various parts of his body. I did not conduct any searching of the body and that action was performed by the Hennepin County Crime lab. Once Prince?s body was removed from the area by the elevator, Hennepin County Crime Lab began to photograph the entire . 30. 31. .in this room with the shelves. The ?rst shelf had hats on it and was checked. I moved to the second shelf . and the pills Were photographed. Ithen counted the pills and there were 15 of the Hydrocodone pills. anything else. This was the only area in the dressing room that I searched and I cleared. 32. 33. ICR: 16012559 complex. All the detectives and command staff on scene then had a meeting to discuss the best practice to search the large complex. It was decided that detectives would start on the 2nd floor after it was photographed and then move to 1St floor. 0n the 2nd ?oOr, it appeared to have the bedrooms for Prince along with dressing rooms. The ?rst area I searched was what appeared to be the master suite of Paisley Park. Before entering into the room, I placed black latex gloves on my hands to not contaminate anything. The room had a dressing room area and a wood ?oor. The dressing room area was white with a blue ?sky? trim on the t0p. The room had a small like stage area and three steps leading to the top of it. The dressing room area had a white vanity with white light bulbs all around the mirror. The counter was relatively clean but did have some clear drinking glasses 0n them. I did not start my search in the dressing room area. I moved from the dressing room area up the small white carpeted stairs (5) leading to the master suite. Inside the room I noted it was clean with a round bed with atop. The walls in the room were white with a purple trim toward the ceiling. In this room, I searched the closet in the far southeast wall. Inside the closet were ?le cabinets which were unlocked and a brief case. I opened the unlocked black brief case and inside was nothing to note for evidence. Inside the ?le cabinets were pictures and some files. Nothing to note for evidence. I then moved along the south wall and searched behind the drapes. I searched a couch, coffee table and then another set of drapes. Nothing to note for evidence. I found a glass door that lead out to a patio area but it appeared it had not been used in some time. I came to an opening in the wall for a small area with a sink and cabinets. I was advised the area was already searched along with. the bathroom and hallway leading to the bathroom. returned to the northeast corner of the room and searched the closet in that area. Inside the closet was stereo equipment and nothing for evidence. I cleared themaster suite and moved to the next room to the north on the eastside of the 2H ?oor. - This room was another dressing room that had clothing, shoes, hats and shelves with jewelry. The room had a wood ?oor with white walls. When I entered the room, other detectives were inside starting their search. I asked if anyone had searched the white shelves and was advised no one had. I started my search which had jewelry and a white box. I opened the white box and inside I could see it contained jewelry. I moved some of it because I observed white tissue paper, used for gifts, inside. I picked up the tissue paper and opened it. When I Opened the paper, I located a number of white pills with the stamp-of Watson 853 on them. I advised Detective Wagner of the ?nd and she looked up what the pills were on Drugscom. Detective Wagner also had Dr. Quin Strobl (the Medical Examiner) look at the pills. The pills were identi?ed as Hydrocodone by Detective Wagner using the web page called Drugscom. I requested that a Hennepin County Crime Scene Deputy come and photograph the pills. A Hennepin County Deputy came Detective Wagner and Dr. Strobl also counted and con?rmed there were 15 pills. The pills were then collected and placed into an evidence bag. I continued my search of the shelves and did not locate The next room I was directed to search was what we labeled as the ?green room? on the second ?oor. I was advised by Detective Wagner that this was the room Prince used to sleep on a regular basis When I entered this room a shower was to the right. Inside the room was a glass top desk against the wall on the left hand side with a chair. Then there was a bed in the middle with a night stand to the right of the bed. It appeared that someone had been sleeping in the bed at some time because the comforter was pulled back and open. On the far wall was another glass table, garbage can and then a small plug in heater. There was a T. V. that was hanging on the wall with an entertainment table underneath the T.V. A bathroom was in the northwest corner of this bedroom that had a toilet and sink area. While 1n this room, Sgt. Meier advised me that he had located a small white pill lying on the ground. The pill was on the bed skirting and appeared to be the same as I had found in the dressing room. I located the pill and advised for a Hennepin County crime tech to photograph the pill. The pill was photographed and I collected it as evidence. 0n the small white pill, I observed the same marking of Watson 853 34. 35. .36. 37. On 4?22?16 at approximately 1330 hrs, I drove to the Walgreen?s on Hwy lOlf Hwy 7 to check if they had counted the pills and inside were 20 V2 pills. Detective Wagner con?rmed the count of the pills. I . under. Dr. Strobel said Prince was wearing boxer briefs but were on the correct way. A black stocking hat ICR: 16012559 (Hydrocodone) stamped on it. I then located a black hand carrying bag lying on the bed. I began to search the black bag and located a Passport for Prince Rogers Nelson inside. I also located a bunch of $100. 00 dollar bills. The bills were removed and placed into an envelope to be placed in safe keeping by Sgt. Meier. Sgt. Meier counted the money and I continued to search. I continued to search-the bag and did not locate anything else except jewelry/cosmetics and a nasal spray bottle. I moved my search over to the night stand on the right side of the bed. I located on the top of the, stand two over the counter pain relievers. One was a Bayer container and the other Aleve. I opened the Bayer container and located Watson 853 (Hydrocodone) pills inside the container. I advised Detective Wagner and she counted the pills. Detective Wagner counter 64 1/2 pills and I con?rmed the count. then looked into the Aleve bottle and located more white pills with the stamp of Watson 853 (Hydrocodone). I contacted a Hennepin County Deputy to photograph the two bottles and contents. Detective Wagner collected the items and placed them into evidence bags. I continued to search the room and did not locate anything else. I continued to search other areas of the complex and did not locate anything else of interest. The search warrant on Paisley Park was Completed at 1755 and detectives cleared. After we cleared Paisley Park, detectives returned to the Sheriffs Of?ce and looked at the cell phone numbers of the individuals we had taken statements from. Detective Pat Murphy went to the internet to search each phone number to locate which carrier provided service. It was determined that Schulenberg, Tyka Nelson and Joshua Welton all had Verizon Wireless as cell phone providers. Detective Murphy sent Verizon Wireless preservation letters for the call history and text messages associated with the numbers. After this was completed, I cleared the Sheriffs Of?ce and was out of service. Autopsy for Prince Rogers Nelson On 4-22-16 at approximately 0850 hrs, Detective Angela Nucci and I arrived at the Midwest Regional Corner?s Of?ce in Ramsey, MN. Detective Nucci and I met with Medical Examiner Dr. Quin Strobel who was going to perform the autopsy of Prince. At approximately 0900 hrs, Dr. Strobel started the autopsy and noted that Prince was wearing his clothes backwards. Dr. Strobel said Prince?s pants and shirts were on backwards. Prince was Wearing a out long sleeve black shirt and a grey shirt with the sleeves cut off was also removed from Prince head. Dr. Strobel asked if the Sheriff Of?ce wanted Prince?s clothes as evidence and I advised we did. The clothing was gathered and placed into evidence bags by Dr. Strobel and sealed. Dr. Strobel continued her autopsy and advised she did not see any trauma to the body of Prince. Dr. Strobel completed the autopsy at approximately 1300 and said it would be nirmerous weeks to determine a cause of death. Dr. StrObel had no other information to provide at the time and we cleared. Detective Nucci and I took custody of the evidence bags that contained Prince?s clothing at the time of death. Detective Nucci entered the items into evidence at the Sheriff?s Of?ce. Video from Walgreens Lead 14 video of Kirk Johnson picking up medications or of Prince at the scene. I was advised by the Store Manager and Loss Prevention District Manager that they could not release the video. They advised they did have video but Management had locked the video so no associate could view or release it I Was given phone numbers of management to request the video for our investigation. I returned to the of?ce and called the phone numbers. I called a Phil Caruso and Emily Hartwig and left them messages as they were I the media Of?cial with the video. Neither of them answered so I left phone messages. I then sent an email to Phil Caurso and Emily Hartwig. At approximately 1944 hrs, I received an email from Terry Reiland and a phone call. I talked with Reiland and advised him I was seeking the video. I gave Reiland 40. 41. 42. the information I was requesting and Reiland asked for an Administrative Subpoena. 38. On 4?25?16, I wrote the Subpoena and it was signed by Asst. County Attorney David Hunt. I then faxed. the Subpoena requesting all the information/video associated with the Prince Rogers Nelson, Meron NMN Bekure and Kirk Anthony Johnson on 4-20-16 or other dates. I was then contacted by Timothy Belka and advised that Walgreens had received the Subpoena and asked other questions. I answered Belka?s questions and he advised he would look for the video?s and would send them to me. On 4-27-16, I talked with Belka via phone and learned he had found three video?s from Walgreen stores. Belka advised he would email me the videos with descriptions. I received the email and the ?rst video was from a Walgreens located at 7845 Portland Ave S, in the city of Bloomington, MN 55420. The date of the video is from 4?14-16, the date the Percocet was ?lled and issued to Kirk Johnson via Dr. Schulenberg for Prince. Thesecond video is from a Walgreens located at 4950 County Rd 101, Minnetonka, MN 55345 on 4-20?16. In this video, Kirk Johnson ?lls prescriptions, during Prince?s doctor?s visit with Dr. Schulenberg, at 1749 and is by himself. The last video is from the same Walgreens location in Minnetonka, MN and hem later at 1904 after Prince?s doctor visit. I thanked Belka for the video and downloaded all videos to a CD, placing the CD into evidence at the Sheriff? Office. Moline, IL Quad City International Airport Public Safety Lead #19 On 4-25?16 at 1622 hrs, I wrote an email to the Public Safety Manager, Jeffrey Patterson, advising that I was seeking information about the emergency landing with Prince. I did not disclose much in the email and asked for Manager Patterson to give me a call. On 4?26-16, I made phone contact with Patterson who advised he'would check into all the records for the emergency landing. Patterson asked that I send him a request on of?cial Carver County letterhead to prove who I was and what information I was seeking. I told Patterson I-would complete a letter and email it to him. I emailed the letter to Patterson and he advised he had received it, Patterson advised he had the information and would put all reports and video into a packet. Patterson said he was going to check with the Legal Department for the Airport to make sure the information could be released and contact me at a later date. 0114-29?16, I received a phone call from Patterson who advised me that I would need to serve a search warrant to obtain the records from Quad City Airport. Patterson advised it was what the legal division - with the Airport had requested. Patterson told me that he had all the information ready "and packaged for pick-up when the warrant was serVed. I thanked Patterson and advised him that I would be contacting Sgt. Meier to see when I would be going down to Moline, IL. Administration and Detectives were currently in discussion about how and when to serve the search warrant. I thanked Patterson for his time and advised I would be in contact with him in the future. On 5-3-16 at approximately 1100 hrs, Sgt. Meier, Detective I Wagner and I had a meeting with the DEA. At that time it was deCided this case would be a joint investigation. It was determined that I would not send a search warrant and a Federal Grand Jury Subpoena" would be drafted by the DEA for the Quad City Airport information. On 5 ?31-16, Detective Wagner received a copy of a CD with the information sent to the DEA from the Airport. Detective Wagner and I reviewed the information and Detective Wagner documented the ?ndings in her report. Prince?s Medical Records Ridgeview Medical Center Lead #28 On 4-26-16, Iwrote a search warrant for Prince?s medical records located at the Hwy 212 Medical Center in Chaska, MN. On 4-2746, I met with the Honorable Judge Eric Braaten at approximately 1325 and he reviewed the search warrant. Judge Braaten reviewed the warrant and signed it. I then called the Ridgeview Medical Center and talked with a Jennifer in records. I advised Jennifer I had a signed search warrant for Prince?s records and she advised she had already pulled the records. I drove to the hospital and served the warrant on 4- 27- 16 at approx1mate1y 1615 I received all of Prince? 3 medical records from his visits with Ridgeview. 43. I received records that showed Prince?s medical emergency on 4-2146. The paramedics checked a box on their form that showed Prince had vomit in his airway and his eyes were closed. In the narrative history on the paramedic?s record it reads, Pt. was taken yesterday by family to clinic doctor for ?nervous? and anxious. Pt. given rneds to relax and per family Pt. stated ?it?s not enough.? Pt. last seen at his home last night and today found supine, not breathing. Fire arrived and pulled Pt. out of elevator and started CPR. Upon our arrival CPR in progress. Pt. limbs ale stiff, arms bent and unable to straighten. CRP stopped due to rigor mortis. Pt. has vomit in mouth. Leads applied and asystole con?rmed Med control contacted and T. (time of death) con?rmed at 1007. Clear scene. I Continued to read the record from 4? 21- 16 and learned that no AED was used and the death was unwitnessed. 44. I then found a medical record for Prince from 10-23-13. This record shows Prince was admitted into the emergency department at 1958 on 10?23- 13 after being transported by ambulance; In this report, Prince complains about vomiting and dehydration. Prince was assisted to the ambulance on the gurney and transported to the emergency room. While at the hospital, the doctor treating Prince wrote in her report the following, Patient very with orthostatic vital signs and has elevated WBC (white blood count) with left shift. Bicarb is only 19 suggesting he has not been eating for some time and potassium low at 3. 4. Declined the Zofran offered as he would prefer to be as natural as possible. Liter of normal saline given and will give liter of D5 1/2 normal with of potassium. Instructed patient he will need to be able to keep at least some water down so will try a PO challenge when ready. Prince was then discharged from the hospital at 23 02 on 10-23-13. Unity Point Health-Trinity. Rock Island, IL. Lead#13 45.1;On 4?26-16, I contacted the Moline, IL Police Department to try and ?gure out what hospital Prince was taken to on 4?15-16 during the emergency landing. I was put in contact with Sgt. Eli Soliz and I had a phone conversation with him. Sgt. Soliz said he did not know what hospital it was but would check into it. On 4- 26- 16 at approximately 1507 I received an e-mail from gt Soliz. I was informed that his agency did not have anything to do with the medical event but found out Prince was taken to Unity Point Health-Trinity, 2701 17th St, Rock Island, IL 61201. I called Unity Point Health and talked with the Director of Risk Management, Steve Gross. I advised Gross I was looking to serve a search warrant on the hospital for Prince?s medical records from the emergency plane landing. Gross advised he has the information but to believe I was a Detective, asked if I would send him a letter on Of?cial Letterhead because of the sensitivity of the information. I drafted a letter and e?mailed a copy of it to Gross. I also printed a copy of the letter to keep for this ?le. I advised Gross I would be in contact with him in the future as to a date when I would come down to Illinois for serve the warrant. Again, we met with the DEA and decided to work together and we will decide together when to drive down to Rock Island, IL to serve the warrant. We are going to interview staff at the hospital and Airport Police. The information with be sought serving a Federal Grand Jury Subpoena. "On 5?31-16, Detective Wagner met with the DEA at their Minneapolis of?ce and received a CD containing information about the medical emergency and medical records for Moline, IL. Please refer to her report for the details. Medical Records from North Memorial Hospital Robbinsdale. MN Lead# 12 46. On 4-26-?16, I called North Memorial records and asked to talk with a supervisor. I made contact with a supervisor and I advised I had written a search warrant for the medical records of Prince. The supervisor advised he would pass the information onto the attorney who represents North Memorial. A short-time later I received a phone call from Attorney Joseph Dixon with Fredrikson and Byron. Dixon said he was 47. the best of my knowledge. -I thanked Judge Mabley and cleared the courthouse. I met with Dixon on 4- information requested. I cleared North Memorialmeeting for 5-5-16 at 1330 at North Memorial in Robbinsdale. On 5-5-16 at 1330 hrs, I 49. . Dr. Schulenberg into the clinic. Prince 15 wearing black tight pants, black shirt, black coat and a stocking 50. 51. - onset of a variety of complaints. He has been vomiting over the night and thought it might be related to 52. 53 J.- ICR: 16012559 representing North Memonal and was w1llmg to help with my request. 0n 4-27-16, I arrived at the Hennepin County Government Center to meet with the Honorable Judge Daniel Mabley. I advised Judge Mabley I had a search warrant for the medical records for Prince at North Memorial. Judge Mabley read the application and signed the warrant. I swore the information was true to 28?16 at approximately 0930 and discussed what information I was looking for at North Memorial. I showed Dixon a copy of the search warrant that asked for the records. Dixon and I talked for about an hour and agreed to what till the following week to execute the warrant so he could collect all the on 5?4?1 6, I received a phone call from Dixon advising he had all the records ready for pick-up. Dixon served Dixon the search warrant. Dixon said he was able to ?nd all the records'for Prince and placed it into a file folder. Dixon also advised he located video of Prince, Kirk and Dr. Schulenberg at the North Memorial Clinic in Minnetonka. Dixon placed the video on a jump drive and gave it to me along with all the 1ecords. I continued to talk with Dixon and thanked him for his time. I returned to the Sheriff?s Of?ce and started to review the video that was provided by Dixon of Prince last visit with Dr Schulenberg on 4-20-16 just after 1700 hrs. In the video, Dr. Schulenberg is seen Walking out the doors of the clinic and into the parking lot. Kirk and Prince are then seen walking with hat. Prince, Kirk and Dr. Schulenberg enter the clinic at approximately 1706 and walk out Of sight. At 1738 hrs, Kirk is seen walking out of the clinic and then returns at 1805 hrs. At 1829 hrs, Dr. Schulenberg walks Prince and Kirk out of the clinic. I reviewed the medical records for Prince and found a letter from Dixon In the letter dated May 5, 20l6, Dixon explains that North Memorial changed Prince? name in their system to a ?Peter Vang.? Dixon advised North Memorial also changed Kirk Johnson name in the system to John -Pellegrino The names were changed to protect other employee from Icoking into the ?les. I then started to review the medical records for Prince. I started by looking at Dr Schulenberg 3 notes from the April 7th, 2016 visit. I learned the following from Dr. Schulenberg? 3 report. It reads as follows, Pt' 13' 57 y. 0. year old male who 1s seen at his home for recent something he ate. No fever is noted. No chills but did feel cold over night. No diarrhea. Not really and abdominal pain and nausea is not prominent. Was concerned that he might need some IVF as these have helped in the?past. Over night as well has noted and tingling in the right leg below the level of the knee. No injury noted and has not had like this in the past. Feels like pins and needles but does not seem to be waking up like he would expect. Similar are noted in both hands. He is supposed to leave toWn today for a show as well. Denies any backpain. Has history of hip pain. No changes in bowel or bladder function are noted. No neck pain. No other concerns were noted at the time of the home visit. He was brought to the office for and during that time complained of feeling some odd sensation in the left eye, as if something was present in it. Feels he 15 out of balance at this time. Was not interested in any blood testing. The notes for 4?7-16 say that Prince later came to the clinic and was given 1 liter of normal saline and it was administered through an?IV in the right forearm. Dr. Schulenberg checked with Prince the next day and Prince said the numbness and tingling in both hands/right leg were resolved and clear. I then reviewed the medical records from 4-20-16. I learned the following from Dr. Schulenberg? 5 notes as he wrote them. ?Pt. is a 57 y. 0. year old male who presents with a friend today (Kirk Johnson) for feeling antsy. started today. He thinks it might be related to stopping Tylenol, the last of which he took this morning. When questioned if the Tylenol . contains any other ingredients he stated that he did not know. Independent sources of history have informed me the Tylenol also contains hydrocodone There has been no nausea or vomiting He has not 54. were Clonidine, Hydroxy'zine Pamoate and diazep-am (Valium). Dr. Schulenberg also had Prince complete 55. 57. 53. Mild shakiness but no overt tremors. No fevers No URI No cough. No chest pain. No . at the Carver County Sheriff?s Of?ce. 56. Prosecutor 3 Request to seal the warrant and showed him it was signed by Judge Mabley. The male said 5; I .3 m?zn??g I r" had any of the neurolo g1c he had the last time I had seen him. Denies any pain except some mild discomfort 1n the hip. No diarrhea. Just feeling antsy, anxious and 1n need of something calming. Feels sOme IVF may help to restore balance. He IS open to having some labs done today and expressed concerns about his adrenal glands due to persistent fatigue. Some hoarseness and weakness to his voice. shortness of breath. No palpitations.? I continued to read the notes and learned that Dr. Schulenberg ordered medications for Prince -and_they a urine test which resulted in a positive test result for Opiates. The test was sent off and con?rmed with the following results on 4?25-16. _Positive__fo_r Hydrocodone 253 ng/ml and Hydromorphone 87 ng/ml for levels. On 4-21-16, Dr. Schulenberg added progress notes that stated the following, ?Will check some labs today. _If the Tylenol indeed contains hydrocodon'e there is the concern of opiate withdraWal. Pt. did have 1 liter of NS infused in the of?ce at his request and tolerated well without any complications.? I then looked at the medical records for Kirk Johnson pertaining to what Dr. Schulenberg admitted to me in his statement. Dr. Schulenberg advised Kirk called wanting to get something for Prince cause his hip was hurting. Dr. Schulenberg admitted to ordering Percocet for Prince but placing the medication-in Kiik? 5 name. I looked at the report for Kirk and it showed that on 4-14? 16 that Kirk did make contact with Dr. Schulenberg and medication of Percocet was approved. The following 13 Dr. Schulenberg? 3 medical notes pertaining to the phone call he had with Kirk on 4- 14- 16. ?Pt called and 18 leaving town for Atlanta for a concern shortly and hurt his back lifting equipment. Was wondering if he could get Something. Reviewed of low back axial pain without alarm One time script for Perco'cet that he could pick up. FYI. I have known pt for 15 years and this is a very rare request. Pt is awa1e he will need to come and pick up script.? I ?nished looking through the medical records and entered them into evidence On 5-6-16, at approximately l100 hrs, DEA Agent Lombardi and I met with Assistant County Attorney Amy Sweasy with the Hennepin County Attomey? 3 Of?ce to have the North Memorial Hospital medical records warrant sealed. Agent Lombardi and I met with SweaSy and she wrote the Prosecutor?s request to seal the search warrant pursuant to rule 33. 04. Sweasy wrote the request and then signed it. I then called Judge Mabley who advised he was in his chambers and would review the request. Agent Lombardi and I walked to the Government building in Minneapolis and met with Judge Mabley in his chambers. Judge. Mabley reviewed the request and signed the request to have the warrant sealed on 5-6-16. I asked Judge Mabley?s secretary to make a copy of the Prosecutor?s Request to seal the warrant. The secretary made a copy and provided it to me. Agent Lombardi and I then went down to level 3 1n the Government Center to ?le the search warrant. I met with an employee and explained to him the warrant was to be sealed. I showed the male worker the ok? and walked over to a desk and grabbed a manila envelope which said sealed search warrant. The male collected the signed request to seal the warrant from Judge Mabley and the original signed search warrant. All the items were paper clipped together. The male took all the items and placed them into the envelope and pressed down a piece of tape closing the warrant. The male then took the envelope to a desk and the warrant was ?led. Agent Lombardi and I cleared the Government Center and returned to the DEA of?ce. On 5-11-16, I was asked to speak with Chief Deputy Jason Kamerud in regards to the search warrant that was ?led in Hennepin County. Chief Deputy Kamerud advised me that the search warrant was released and asked me about the process I completed while ?ling it. I advised Chief Deputy Kamerud on the process that Agent Lombardi and I. folloWed for the process of sealing the warrant. Chief Deputy Kamerud advised he would look into the incident and would follow up with me later. I learned from Chief ICR. 16012559 I Deputy that the release of the warrant was a mistake on Hennepin County releasing the sealch warrant. I was advised it Was employee error at the Hennepin County Courthouse. 59. Medical Examiner found a Lethal amount of Fentanvl 1n Prince?s system 60. On 5-9-16, Sgt. Meier asked Detective Wagner and I to meet in his of?ce. Detective Wagner and I arrived at Sgt. Meier of?ce and learned that Prince had a very high amount of Fentanyl in his blood that was taken at autopsy. Sgt. Meier instructed Detective Wagner to start working on a new search warrant and to relay the information to Marc Lombardi who is working the case with us from the D. E. A. We met With Lombardi and discussed the information with him at our meeting which was at 1300 hrs1610 hrs, Sgt. DeWitt Meier asked me if I had requested the blood for Prince when it was drawn on 4- 20- 16 at North Memorial by Dr. Schulenberg. I advised I did not but would call North Memorial. I called North Memorial? Attorney, Joseph Dixon, and asked him about the blood and learned the blood was sent to the Medical Examiner?s Of?ce. I contacted Sgt. Meier via cell phone and relayed the information that the blood taken from Prince on 4-20-16 was at the Medical Examiner?s Of?ce. Sgt. Meier advised he would call the Medical Examiner to have the blood sent for further testingfor Fentanyl. Sgt. Meier worked the details out with Dr. Strobel and the blood would be further tested for the Fentanyl. 2nd Search Warrant at Paisley Park with DEA 62. On 5?10-16, I was noti?ed by Detective Wagner that a new search warrant was drafted for Paisley Park 7801 Audubon Rd, Chanhassen, MN to look for the source of the Fentanyl. I was advised the DEA and our staff were going to meet at the Sheriff?s Of?ce at approximately 1600 hrs. The DEA agents arrived at the Sheriff?s Of?ce and we had a briefing about the warrant and process. On 5-10?16 at 1744 hrs, Carver County Detectives and DEA agents arrived on scene at Paisley Park and served the warrant to Bremer Bank which was is the trustee for Paisley Park. The bank trustee locked at the warrant and unlocked the doors to Paisley Park. 63. Once inside'Paisley Park, the DEA went ahead and started to number the rooms so we knew where items of signi?cance came from. When the DEA was ?nished numbering the rooms, we started our search of Paisley Park. I started off in room number #6 (The green room), which was where Prince slept. On the left side of the room, I located three small clear plastic bags. It was unknown if it contained anything but was collected for testing. I packaged the bags and brought them to Investigations assistant Nikki Schmidt to enter the items as evidence. I returned to room #6 and continued to search. I then found a red notebook with some writing on it of names and phone numbers of people I did not know. I found the red notebook on the left side of the bed on a chair. I asked Detective Wagner if she wanted to collect the red notebook and she advised ?yes? because of the phone numbers. The item was collected and placed into an evidence bag. 64. I again returned to room number #6 and had DEA agent Marc Lombardi assist me in the search of the room. I found the bedding that was on Prince?s bed during the ?rst search warrant was already taken off the bed and placed into plastic bags. In another bag I located towels that used to be in the bathroom of Prince?s bedroom (Room 6). I inventoried the towels as such, 1 cream hand towel, 1 white hand towel, 1 tan bath towel, 1 peach towel,?and one white bath towel. The towels were packaged and entered into evidence along with the bags of the bedding. DEA agent Lombardi was searching "a black suitcase that was located on the ?oor of room #6 when he located in at CVS vitamin bottle 27 white oblong pills with the stamp Watson 853 on them. Agent Lombardi then loCated another, CVS vitamin bottle, with 49 white oblong pills with the stamp Watson 853 on them. Agent Lombardi also located 3 half pills that were of the white oblong type. The items were collected by Agent Lombardi and packaged and placed into evidence. I continued searching and remembered from the ?rst search warrant that there were some Kleenexes on the right side of the bed on the The Kleenexes were still on the night stand so .I 67. collected them and packaged them. I turned them into ev1dence I then ?nished 1n the room and moved to 65. 66. arrived on scene at approximately 0900 and continued our search of Paisley Park. Once we completed End of Report Detective Chris Nelson #83 9 ICR: 16012559 the lower level of Paisley Park to Room I. I searched this small room (I) with DEA Agent Terrence Olstad. The only item that was collected was a copy of a con?dentiality agreement between Prince and a party named Evan Bakke. The paper was collected and placed into evidence. Agent Olstad and I then cleared the room and continued to search other areas. Since the start of the search started earlier in the evening, it was decided to stop the search for the night and have two deputies arrive on scene to hold the residence. At approximately 2330 hours all agents and detectives cleared except Sgt. Meier and I. We stayed on scene and waited for Sgt. Michael Wollin and- Deputy Carly Barrett to arrive to hold the scene. gt. Wollin and Deputy Barrett arrived at approximately 0000 hrs. The scene was taken over by them until the following morning. On 5-11-16, Sgt. Dave McDonald and Cpl. Kurt Schoening relieved Sgt. Wollin and Deputy Barrett at 0600 and were on scene when we arrived. On 5-11-16, DEA agents and detectives from the Carver County Sheriff?s Of?ce the search it was determined that ?the vault? needed to be searched. Bremer Bank advised they were ?ying in a person who could drill into the safe but he was coming on Sunday. It was determined that the Sheriff?s Office and DEA would continue to hold the scene until the safe was searched. Numerous deputies were contacted and worked 24 hours a day to secure and hold Paisley Park In our control until the safe was opened. On 5-12-16, the DEA and Sgt. Meier Detective Wagner and I arrived on scene. We were advised the person who was going to open the safe had arrived and was getting ready to start working on the vault. We arrived and the adult male started drilling into the safe. Attorney?s that represented the brothers and sisters were in attendance along with the US. Attorney?s Of?ce and Bremer bank of?cials handling the trust. Once the vault was opened, Detectives and DEA agents entered and searched the vault. Inside the vault were numerous music recordings and personal items. I did not locate anything of evidence inside the vault. Once the vault was searched, the vault door was repaired and turned over to Bremer bank. Detectives and DEA agents then cleared Paisley Park.
