After Of?cers arrived, arrangements were made to have a set of keys delivered to avoid having to force entry into the building. The purpose of the search was to search for additional evidence including other prescription/ non- prescription drugs, and other documents that may assist in explaining where Prince Rogers Nelson, deceased, was acquiring his medications. At approximately 1744 hours, keys for the building were delivered by a representative of Bremer Bank. It is my understanding Bremer Bank is the current representatives of Paisley Park Studios and estate. After - keys were received of?cers were able to gain into the building. Before we entered the building we were told that no one would or should be inside. Revised 05I12 . f: . .. ICR: 16012559 Lust: 0? 1! i new 4. After into the building, Of?ce 5 split mt different groups and conducted a room to room search of the entire building, searching for any persons who may be present on the property. It is noted there was private security personal on the property and their responsibility was to patrol the perimeter and exterior of the building Paisley Park Studios is a large complex and has multiple rooms and storage areas. I was with a group of of?cers that cleared the main level, of?ces, storage rooms, the large concert hall, and part of the 2??1 ?oor including the mechanical rooms. Clearing the building took approximately 30 minutes. 57? At approximately 1821 hours we had briefing regarding the search process, and instructions regarding the items we were searching for. We received instructions if items of interest were located to leave them in place and Detective Tyler Stahn (CCSO) would come and photograph the item before it was moved, and before it was packaged. The item would then be taken to the staging area near the main entry. This is the area where Detective Wagner and Investigative Assistant Nikki Schmidt would log the item. 7. We were also directed, to the best of our ability, to search our assigned areas with partners or in pairs. My search partner for this detail was Detective Amanda Grundeman. 8. We began searching at approximately 1835 hours. Between approximately 1835 hours and 2000 hours DeteCtive Grundeman and I searched the following areas; the office located on the 2m1 ?oor, on the- Northeast side of the building; the master bathroom, and attached mechanical room located on ?oor near Prince? 5 primary bedroom (the green room, room 9. We searched the Studio storage room, which contained a large number of audio and or video tapes. Also located in this room were (2) standard size refrigerators. These refrigerators were running. One Was locked. Detective Grundernan was later able to gain access into the locked refrigeratorand determined it also contained a'_number of audio or video recordings. 10. The second refrigerator was taped shut._ Detective Grundeman was able gain access into this refrigerator. The bottom of this refrigerator contained audio and or video recordings. The freezer contained (2) wrapped spoons. It seemed unusual these spoons would be kept in the freezer. The spoons were collected by Detective Grundeman. 11. We also searched a small of?ce located at the main lobby on the north side of the building. 12. At approximately 2005 hours we were instructed to take a break. Between 2005 and 2100 hours I assisted Detective Chris wagner ?lling out property inventory forms, of items that were collected and seized. 13. At approximately 2105 hours we-again began searching the building. Detective Grundeman was again my search partner. From 2105 to 2205 hours we searched the following areas; the Studio lounge located on the 1% ?oor; a room identi?ed as room lounge which is located near the entry on the Northwest side of the building. 14. We searched room which is located on the ?rst ?oor, off Studio the studio with the drum set. The supervisors were also working with Bremer Bank representatives and attorney?s to get someone to the property the following day to open a large safe door located in the basement of the complex. This door was located in the Vault room. Arrangements were made for deputies to be posted inside the building, holding the property for the night. 19. At approximately 2230 hours I cleared from Paisley Park Studios. 20. On 05-11?16 at approximately 0858 hours I was again at Paisley Park Studios located at 7801 Audubon Road in the city of Chanhassen. 21. At approximately 0900 hours we had a short brie?ng and were instructed to continue the search of the building as we had done the previous day. My search partner was again Detective Amanda Grundeman. 22. Between 0915 and 0930 we ?nished searching the second ?oor near the back stage lounge. This included the Salon located on the 2??1 ?oor. 23. At approximately 0933 hours we began searching the lower level basement and underground garage. We searched the room labeled Vault. This room contained a large number of boxed audio and Video recordings, computer hard drives, photographs, awards, memorabilia, and other items appearing to belong to Prince. There is also a large steel bank vault door located in this room. Currently we are unable to gain access to that room. 24. In the vault room we founda number of smaller plastic storage or shipping containers locked with padlocks. We were unable to locate the keys for the lock, and we were given approval/permission to cut the locks to view the contents. 25. At approximately 1010 hours I cut the lock on those containers. The containers were not damaged. Inside the containers were. hard drives and other recording related equipment. 26'. At approximately 1030 I was back at the staging area where Detective Wagner was collecting and packaging the items to be seized. 27. At approximately 1200 hours we began searching the (.2) Mo dumpsters located on the south side of the building. These dumpsters were relocated by the garbage? hauler to the south east'side of the building, near - the Underground garage access, to limit the potential for people to take photographs of of?cers while searching the dumpsters- The initial search group was Detective Grundeman, Detective .Gramentz, DeteCtive gt. Meier, and myself. 28. During the search of the dumpster a number of items of interest were located and taken to Detective Wagner to be packaged for possible processing. Please see the property inventory form for the items recovered from the dumpster. 29. Note: During the course of executing the search warrant the interior doors of Paisley Park Studios were marked with post it notes, identifying the room by number, letter, or by name. This system was used to help processing, and identify the individual rooms where items were located. 30. At approximately 1326 hours I assisted in removing the post it notes from the interior doors. 31. Additional information: The law enforcement supervisors, Bremer Bank representatives, and attorneys involved have been working to come up with an agreement on how to access the actual vault located in the vault room. - 32. It is my understanding that currently no one has access, or the combination to the vault door. It is also my understanding, arrangements have been made with a company in California to send a lock technician from California to attempt to open the door. 33. Because of paperwork that needed to be completed, and the travel arrangements for the technician, the execution of the search warrant on the vault was going to be temporarily suspended. The representative of - -. I Bremer Bank ill contac aw enforcement supervisors when they have con?rmed arrangements for the date and time for the technician to be at Paisley Park 34. Because the search warrant on the vault was being temporarily suspended there was a need to mark the vault door with evidence tape to verify the door was not opened or tampered with until the lock technician could arrive, and that the interested parties of the contents of the vault, were available and present. 35. At approximately 1350 hours I went to the vault room with Detective Sgt. Meier and Attorney Ruth Rivard, to mark the vault door with evidence tape. Again the purpose of marking the door is to show the door was secured, and is to remain closed until the interested parties from the Sheriff?s Of?ce, the DEA, and representatives of Bremer Bank and Attorneys were available and present. 36. I attached both red and white evidence tapes to different location on the door and door jam of the vault. This was witnessed by Sgt. Meier and Attorney Ruth Rivard, representative of Bremer Bank. 37. I previous veri?ed the Vault door was locked and secure. After the evidence tape was applied I also took photographs with my county issued cell phone showing the locations of the evidence tape, and to show the tape'was not broken. Attorney Ruth Rivard also took photographs with her phone, for her records, also verifying the locations of the tape. 38. The photos taken of the vault will be downloaded and copied to a CD and placed into evidence. Additionally, Detective Tyler Stahn also took photographs documenting the evidence seal on the vault. 39. At approximately 1416 hours I cleared from Paisley Park. Upon arriving, we learned Marco Leone was the driver of the ambulance and did not have any contact with Prince. For that reason we did not interview Leone. At 0826 hours, DEA officer Marc Lombardi and I met with Justin David Fredericksen dob in an office room at the fire hall. Fredericksen said that he was assigned to station 3 and I ambulance 13. Fredericksen said sometime during the shift, they received an "unknown medical problem? atthe airport. Fredericksen said this was unusual because they typically don?t go out to the airport during the night because planes are not scheduled to land overnight. Fredericksen said when they arrived, the plane had already landed and the stairs were already out. Fredericksen said as he grabbed his gear and a cot, heobserved "a guy in a black suit carrying a little guy out of the airplane like you would carry a little kid or a baby?. Fredericksen described the ?guy? who was carrying the person as a "big dude, African American guy wearing a dark suit? and described also seeing a ?pretty younger girl? with lighter colored skin who had also exited the plane. Fredericksen said that at first he thought the person that was being carried was an older female because they were wearing glittery gold clothing and shoes. Fredericksen said he asked the "big dude? (Johnson) how long has she been unresponsive and he said "he just stopped breathing or he?s not breathing good? and that is why they brought the plane down. Fredericksen said as they laid the person down on the cot who was wearing the gold clothing, Fredericksen said he immediately recognized it was Prince and not an older women because Prince was "very distinctive?. 8. Fredericksen said he immediately noticed Prince wasn?t breathing and believed it to be an opiate overdose. When asked why he came to that conclusion, Fredericksen said that he has been a paramedic for 10 years and has treated numerous patients who have overdosed on an opiate. We asked Fredericksen what he had seen during the initial examination of Prince. Fredericksendescribed Prince?s overallpresentation including respiratory arrest, his pulseand described Prince's pupils as "pinpoint." Fredericksensaid he turned to the ?big black dude? (Johnson) and asked what did Prince take. According to Fredericksen,_Johnson told Fredericksen "sometimes Prince takes a Perc'ocet after ashow?. Fredericksen said he asked Kirk ?are you sure he only took one Percocet?? but Kirk didn?t say anything else. Fredericksen said Kirk?s answers were very stoic, short and didn?t volunteer anything unless we asked. 9. Fredericksen said be administered 2mg narcan via IV ?which seemed pretty ineffective". Fredericksen said he was surprised Prince wasn?t coming around because 1mg will usually wake up a casual opiate user, heavier users usually wake up with 2mg narcan. Fredericksen said he was surprised they weren?t getting a response right away and explained how people usually come around in as fast as 10 seconds. 10. Fredericksen said he administered a second 2mg narcan dose via IV for a total of 4mg narcan. Fredericksen said it wasn?t long after the second dose of 2mg of narcan that Prince took a large gasp and woke up. Fredericksen said he asked Prince how he was feeling. Fredericksen said Prince looked up at Fredericksen in almost a confused state and then turned and looked at Kirk Johnson who was sitting on the bench inside of the ambulance. According to Fredericksen, without Prince saying a word, Kirk Johnson replied "Prince feels fine?. Fredericksen said he thought it was strange how Kirk Johnson spoke for Prince-Fredericksen said hispartner Austin Rands asked Prince the'question do you feel Prince?? and thistime Prince answered the paramedics stating, feel all fuzzy?. Fredericksen said that was the only comment Prince ever made during theirinteraction. Fredericksen describedPrince as being very quiet. 11. DEA Officer Lombardi asked Fredericksen who was riding in the back of the ambulance while enroute to the hospital. Fredericksen said it was his partner Austin Rands, Prince, himself and the ?bodyguard?- describing Kirk Johnson as ?your typical bodyguard, big guy in a suit?. Fredericksen said the "young, pretty female "who was on the plane rode to the hospital in the Ambulance?s front seat. Fredericksen . said at one point while they were still at the airport he had asked the female about Prince?s medical history. Fredericksen told us once at the hospital, they requested a private room and transferred Prince to a cot. Fredericksen said Prince never said anything other-than the feel all Fuzzy? comment. asked Fredericksen if they took any samples from Prince for testing while enroute to the hospital. Fredericksen said the only sample they took was a blood glucose reading which came back within the normal limits. Fredericksen said that sample would have been discarded in the biohazard sharps container. Donald Wright, who goes by the name of Ed, was the primary nurse for Prince while Prince was admitted to-the Trinity Rock Island emergency room on 04-15?16. Detective Chris Nelson and DEA task Officer Richard Hand interviewed Pamela Esparza and Cathy Curtis, two additional nurses who treated Prince while at the Trinity Rock Island ER. Doctor Nicole Mancha, the doctor Who treated Prince was not available for an interview at that time. "In speaking with Wright, Wright began by stating that he has worked as an emergency room nurse for the past 3 years. In regards to the night Prince was admitted to the hospital, Wright said that he was the one who answered the radio call from the paramedics around 0100 hours. The call pertained to an unresponsive patient who was getting off a plane at Quad city airport and paramedics administered narcan. Wright said Once they arrived at the hOspital, Wright said he immediately noticed the party being treated was Prince._Wright said Prince was accompanied by a "black male, late 40?s, approximately my age, bald, in a suit, who presented himself as Prince?s manager". Prince was placed . in room 22. Wright said they began the triage process which included screening questions such as do you have allergies? Have you been out Of the coUntry recently? Do you feel like harming yourself? Wright said Prince was the one who answered all of the questions, including the question pertaining to harming himself which Prince answered Wright said P'rin'ce was hooked up to monitors during that time, was responsive and stable but very quiet. Wright said beCause of the patient being Prince, they kept it very private and limited the number of people who had contact with Prince. Wright said other than himself and Dr. Man'cha, there was also RN, Esparza, ConnieBixby a patient care tech and RN Liza Cruz who would have had contact with Prince that evening. Wright said he even got the female a cot so she could lay down in the room with Prince. Wright said when he went in with the EKG, Prince refused the EKG test. Wright said he and Dr. Mancha went into Prince?s room 2-3 times during the night in an effort to obtain permission from Prince to conduct testing but Prince continued to refuse all lab draws and testing. I asked if it was Prince who was answering the questions orthe ?manager? and Wright said that Prince was the one who was speaking and denied the testing. Wright said they explained that they wanted to observe Prince for the 6 hour general monitoring time because. administering the narcan was not always enough to assure there were no other reactions and since they didn?t know exactly what Prince had ingested because he would not allow them to conduct tests, monitoring him was required. Wright said while he was questioning Prince regarding Whathe had taken, Prince told Wright thatf?someone gave it to him to relax? but Prince never said where he got it from. Wright said Prince never told him what it was. Wright could not recallwho gavehim a small ziplock bag which contained a pill that was sent to thepharmacy for testing. Wright said he could not recall how many pills Prince said he took but said that what Princetold him was not enough to warrant the use of narcan. DEA Officer Lombardi asked Wright ifthe paramedics ever disclosed to Wright what Prince may have taken-or that the ?manager? (Kirk Johnson) said Princemay have taken a Percocet. Wright-said "no" he was never told anything about taking Percocet at least not to his recollection. Wright said it_-was possible the paramedics told Dr. Mancha or the charge nurse Pamela Esparza but Wright was never told. Wright said it began to be more of a monitoring situation because Prince refused all testing and explained how Prince had nowhere to go because the pilots had left and Prince had to wait for another plane to come pick them Up. Wright said Prince, his manager (Kirk Johnson) and the female [Judith Hill) just waited at the hospital for the plane. Wright said his shift that night was 1530 hours to 0400 hours but Wrightelected to stay over until 0730 hours to continue care'for Prince and limit the number of people who had contact with Prince for privacy.Wright said RN Cathy Curtis came on at 0730 hours to relieve Wright.
