Home in Quincy (IVHQ) from August 2015 to date. The purpose of this submission is to correct the record regarding the IVHQ notification process to our employees and to correct much of the testimony that was given at the February 2018 hearing. In that hearing, testimony was provided that was inaccurate and misleading. The following documents provide evidence to clarify the actions ta ken by the leadership of the IVHQ during the onset of the legionnaires outbreak in 2015. The first document provided in the attachments is the transcript from the February hearing. We have highlighted the inaccurate and/or misleading statements and have provided a rebuttal in the margins of each paragraph. This format was used purposefully to provide clarity on all aspects of the testimony that were erroneous or misleading. By commenting in the margins of the transcript we illustrate that our rebuttal specifically addresses the verbatim comments made by the hearing participant. We believe that it is imperative that we highlight these inaccuracies with supporting evidence in order to best represent the ha rd-working and dedicated employees of our agency. My PHD is in animal nutrition and for several years I have been involved in food safety issues in for the animal feed and pet food industry reduction on salmonella. There are many similarities in how you control legionella outbreak in water sources. I will keep my comments brief as a more detailed document has already been submitted. Thank you to all the members for your service to the people and the state of Illinois. American Legion Post 37 supports Senator Cullertons audit of the events of the outbreak of legionaries disease at the Quincy veterans home and appreciates this committees dedication of improving and providing high quality health care to our veterans and a safe work environment to the staff. We support the current administrator at the veterans home. We mourn the lost of our veterans. We do not support the calls to close the veterans home. Legionella is a world wide health issue with CDC estimating 18,000 cases per year average and others estimating as many as 50,000 cases. Legionella needs to be approached as a health epidemic not just a Quincy veterans home issue. It is well established that legionella is an issues found commonly in older plumbing which a majority of the untied states and world have. If you test for legionella you will more than likely find it. It was reported by SJR that the capitol tested positive for legionella bacteria just this year. Legionella bacteria can not be eradicated from a biological point of view. With significant monetary investment the incident of legionaries can be reduced but it never truly can be eliminated. If elimination was achievable then we wouldn?t have antibiotic resistant bacteria. The CDC report states that only QVH was testing for legionella why were not other elderly care facilities? Further it mentions that Blessing hospital was only testing QVH residents for legionaries thus biasing the data against them. Why did local health care limit the testing? The second CDC report states that there were recommendations made by public health officials but practices of local health providers hindered there implementation. It further states that the QVH staff expressed concern that legionaries testing was not being provide by private care facilities to people outside the veterans home. This report recommended a review of all cases of Legionaries in the state. The legionaries testing is now available at blessing hospital which will speed the identification of the disease. It should not have taken several CDC requests and a visit from the Governor for this to happen. They need to utilize this testing for all residents not just for QVH residents. It is our opinion that the lack of maintenance under the previous administrator of the home and were was the budget for the updates and maintenance and how was that money used. We thank this committee as the impact it can have can cause a great difference not only in this state but nationwide. Nettie Smith, President of local AFSCME 1787 in Quincy LPN at Veteran?s Home since 1993. bmith: Today I want to focus on how the Department of Veterans Affairs can do better at communicating with staff about legionella, as well as what the delayed communication meant to my coworkers and I when information wasn't shared in 2015. In 2015 I learned about 1e legionella outbreak on Facebook when an employee shared a posted by our local news station. [Not long after my supervisor wants us to know we had some cases of legionella and there would be some articles in the news about it that evening. Management held an all-staff meeting on August 26, 2015 informing staff of the legionella outbreak. These meeting are not truly all-staff meetings. Some staff have to stay back and care for the members, some may not be working that day, and some may not be working that shift. Ilt?s also important to note that most of the staff at the Quincy Veterans Home most of the staff, (spelling) dietary, housekeeping, lamented Infection Control Coord'mator (ICC) issued an ernai to all (Erect are licensed nus'ng staff of an UNCONFIRMED use of lo. understandably, the ICC asked staffto be discreet aboutthe mconfimedmse unti further 'lrformation was made available. The read rece'qn confirm that Nettie Smith read the ema'l at 1429 on August 25", 2015. (Exhbit A) This negates hertest'lnonythatshefist learned ofthe issue from the local news station?s Facebook postings. Those shared postings re?ect a date of Augwt 27?, 2015, two days after she read the email. (Exfibit B) Cemented [112]: A mandatory all-staff meetig was conducted twice on August 26, 2015. Nettie Smith attended one ofthe two meetings C), gain negating casesfrom Facebook scheduled meet'mgsonAlgust26, 2015, minutesofthe meetigweretistributedtoeach unitanddepartmenton August28, 2015. All staff were requied to verifytheyhad read the minutes and so on do not have email. We still rely on charge nurses and supervisors to spread information by word of mouth making it quite possibly for employees to remain uninformed. As soon as they had one confirmed case of legionella, which happened in August, the Superintendent should have sent a physical letter to all staff and posted that letter in work sites. Because information wasn't shared quickly and efficiently, one of my co-workers got sicker than she should have from legionnaires disease. I'm not going to share her name today out of respect for her privacy but I am going to tell her story. On August 15, 2015 my co-worker left work ill, returning August 24. During the time she was off she was misdiagnosed twice, prescribed three different antibiotics and had a 24 hour stay in the hospital because her were not responding to treatment. She had a high fever and was experiencing shortness of breath. After her return and a discussion about her I suggested maybe she had legionella. The CDC spoke with her that day and suggest she had testing, which came back positive for legionnaires disease. Because she had been about sick with no information from the Quincy Veterans Home, she had no knowledge of the legionnaire outbreak. We wonder if she had known about the legionella outbreak, if the Department of Veterans Affairs had immediately notified staff at the first outbreak would she have been sick for so long? We?ve changed facility management since that time, the current superintendent and director of nursing are more forthcoming with information. We will soon have union representation on a safety committee so we can stay better informed. Our facility is spread out and it makes it very hard to communicate with staff, we can do better. lWith the current CDC recommendations for temperature checks on every resident every four hours and so on, we need to be fully staffed. Currently we have 26 VNAC a_nd 16 LPN vaca_ncies. The CMS grading process alone is taking 4-6 weeks. We need to do better to provide the best possible care for our veterans, we can do betterl Our local union is ready to do more to make the Quincy Veteran?s Home a better place to live and work. Anne: I do want to make the point that I am sure the committee is aware of, that several of those stricken by legionnaires disease were employees. And despite the danger during all three outbreaks, the employees stayed on the job, continued to do their jobs that the veterans rely on, and it?s obviously as Nettie mentioned, more than just a job to those who work there. But I hope this committee can help us improve the situation going forward in terms of this issue of transparency and the issue of appropriate staffing. be first on the communication issue, I want to highlight that in all 3 legionnaire outbreak cases our union found out about all 3 through media reports. In 2015 despite the obvious impact the disease was having on our employees, our union had to seek information from the CDC and the Adams Coung Health DepartmentI because both IDPH and IDVA were largely unresponsive to our phone calls and written re uests for information articularl durin the worst 3 weeks of the outbreak. So, that is why we are so focused on this issue of transparency and communication. 50 what our union has proposed is that there be a legionella committee at the Quincy Veterans Home to allow for participating and input from staff, residents, families of residents, local veterans organizations. So it could address the legionella surveillance and remediation, and to provide a greater avenue for information sharing. When we proposed this in 2016 after the ?rst recurrence, the Administration was unwilling to implement such a committee, but we recently renewed that request at the local level and we understand that the facility management is considering it.l The second issue related to communication is this question of how employees are communicated with. And as Nettie mentioned, it is certainly, in any healthcare organization it makes sense to have Commented [113]: Currently, Nl-IQhas 22VNAC and 14 LPN vacancies. Commented [115]: IDVA never received a request from AFSCK in 2016 regarding the formation of a legiornella committeenorhasAFSCMEprovided anyevidenceofsaid request to form a legionella in 2016. On Apli 20, 2016, IDVA and AFSCME met for the'I arnrnual state-wide labor manqement meeting. At no tine during ourniscussion ofthe legionella issuesat staff representatives mention the formation of a conlnittee or the unanswered attempts rmesting a committee. On June 21, 2017, IDVA and AFSCME gain met forthe'r arnrnual state-wide labor management meeting. The issues olewere noteven atopicofdiscussion attlnismeetig. (MM '0 a supervisor communicating directly with staff, to shift change, and talk about what's happening, that's important. But in terms of this issue we feel there needs to be another system in place, instead of this word of mouth communication. 50 one idea is that, Nettie she's an LPN, she?s on the email system, she mentioned VNAC and dietary workers, they are not, so to have everyone on an email system to quickly communicate with staff. Another idea as Nettie mentioned is old fashioned written communication, the idea being that there has to be someway to directly communicate with employees. So that's on the issue of communication. Secondly this question of adequate staff. Now clearly as we heard in the January hearing, there is a lot more work being done at the Quincy Veterans Home. There's more frequent vitals being taken, more ?ushing of the lines, we want to make sure that the necessary staff are in place to perform that work to supply the veterans the kind of care they deserve. And as the Director mentioned at the January hearing, there were many staff vacancies actually at a number of state veterans homes. This is still an issue at Quincy as Nettie notes, and vacancies are a problem in that they increase overtime and increase the ?oating of staff. And that means that it is more likely that a veteran is going to be cared for by someone who does not normally care for them, day in and day out. If im scheduled for overtime it isn?t necessarily going to be the unit I normally work, I?m working with folks I don't know as well and they don?t know me as well. Sot hats a problem in terms of this issue of surveillance, in trying to catch any change of status in health early. So the more you have staff working with veteran's they know, the more they will see a change in status. So again, there needs to be a focus on filling those vacancies and making sure the staffing levels are appropriate to provide the care that the veterans need. kullerton: All 3 years that the outbreaks occurred, the way employees found out was through the media? Answer: In 2015, in terms of the union, definitely it was the media. Nettie already spoke to her experience in 2015 level even at the employee level it was the media. Nettie: Yes through the media, and then supervisors came and told us and then I believe there was an all staff meeting on the 26-.l tullerton: To fill all these staff vacanciesI you stated CMS has a backlog of 4-6 weeks to get you a nurse? Answer: That's just the grading process, so the hiring is longer than that. Cullerton: Your saying you have board with healthcare information and union reps and why couldn't information just be posted there? Answer: Well I would go further than that, we did information bulletins that can be posted on a local union info board. But what we're saying is management has a greater duty not just on one unit but every pace where employees are going to see it. But we just need to be sure direct communication is going on. And not just in the workplace, as someone like Nettie said who is off sick, there not going to see that. So we need to have other means of communication to people directly at their home. kifowit: If you look on IDPH's website they talk about an infection control committee to review results of investigation results activities of infectious disease and you just testified that you have been asking for ones. What I?m hearing from you is that it is not true that there is an established infection control committee as per protocol? Answer: We have an infection control nurse Kifowit: But not a committee, one person. Answer- Anne: I think what you are getting at is that there needs to be a broader group and there isn't we don't have a part in itl Cemented Again, please refer to A, B, C, 08.1 as staffpriortomeda reports. Additionally, the Infection Control Coortinator and the Medcal Diector meet or more often as deemed necessary. Kifowit: IDPH states that there needs to be a committee though and that all stakeholders need to have a place at the tablecommittee. Jeffries testified that they immediately began educating the staff on August 21-. But you are saying is that you heard from Facebook. So it calls into question that. Answer Smith: As I remember the testimony. I feel like the Director was under the back and forth and was talking about resipatory infection. Kifowit: How would on interpret the term infectious disease on the campus as a nurse. Does it really tell anyone what steps they can take what precautions can be taken with just the term infectious disease. Answer Smith: I can?t think of anything we would just use our precautions Kifowit: So they just told residents and let them talk amongst themselves. Rather than informing them about what the increased risk was from? For your information in did submit a bill for mandated notification. There is a bill now it is just sad that we will legislate to make sure you get properly notified. bchimpf: What do you think, is there anything that the Union could have done better in dealing with this outbreak? Answer: When we were made aware of what was happening we reached out to IDVA and IDPH, and we wanted to ensure that everything that could be done was being done and that all our members had all the information possible. I have to reiterate that in a normal situation there is communication between the local union and facility management and the staff of the union and department management and that was not happening. So we contacted our national office who contacted the Then we also contacted the local health department, they were informative and /r we got information to our members through those sources. But why was that necessary? Could we have done more, with more direct communication? I don't know but we could have done it faster if there was more cooperation with IDPH and IDVA. I feel like the 2017 outbreak there was a difference in that. It improved and let us communicate with our staff faster. Hastings: After the first person died in the Veterans Home how did the staff feel? Answer: We were absolutely concerned, we did not want to see a veteran pass away from those circumstances. We were concerned for the safety of everyone. Hastings: Do you feel as if there was an adequate response right away? Answer: No Hastings: After the second death, how did the staff feel then? Answer: Probably a little more concerned and still wanting more information. Hastings: Do you feel as if there was swift action taken after the second death? Answer: I do not. Hastings: I could gone on and ask you about the the thirteenth. After the 13- death how did the staff feel? Answer: Very saddened by the deaths and still concerned and wanting more information. Still wanting more information. Hastings: Being that it is three years later from the first death, do you feel as if what a leader in our state had said "were taking swift action? do you consider three years swift action? Costello: Of 350 members, how many of those are skilled care? Answer: I couldn't answer that. If I was guessing, I would say probably in the upper two hundreds. Costello: The Department categorized the 2015 town halls as informing the staff. So how many shifts do you all work? Answer: 3 Costello: 3 eight hour shifts. That you are aware of how were things town halls conducted? Answer: As usual they are done maybe twice a day, maybe. Maybe once a day. Either staff come in when they are off or it is the staff that are available to leave the floor that go to those meetings. Costello: Do you remember how many of these meetings there were? Answer: I do not remember the meeting at all. Costello: As far as a policy to inform both the residents and their families and workers are you aware of any such policy? Answer: No Costello: So you are not aware of any policy for any type of an outbreak? Answer: Not that I am aware of no Answer Anne: If I may say it seems like in most cases communication is done from upper management to supervisor and so forth. Costello: What I am trying to get at is if there is any formal protocol in quincy or in any of those facilities. You are not aware of any protocol in place for notification? Answer: No Sir Costello: In previous testimony it was stated that there was increased vital check every 4 hours is this accurate? Answer: yes it was Costello: The communications to the family and next of kin from your eyes how was this done and if you could explain this brie?y. Answer: It was my understanding that letters were sent to the families to notify them kostello: In 2015 when were you alerted of the situation Answer: I don't remember the date Costello: Would you say it was a day after a couple days after Answer: The ?rst to my knowledge is when the supervisor came to the unit and informed the head nurse Costello: And you are the head nurse of your unit Answer: I was not at the time Costello: But you were working at the facility and to your recollection you did not know about this com [1111]; Smith's mm tam until the media knew about it] her MW SW SW "lat McAuliffe: Are all the trades people ASFME people too or are they a part of a different union or wm?x?u how does that work. and hm thehead "user? Answer: Not to my knowledge. Cullerton: Would you and your unit be responsible to go into the independent living homes? Answer: No it would not be an AFSCME member. Cullerton: What type of system do you use to check on patients? Electronic? Hand-written? Answer: Primarily electronic Cullerton: We heard in 2015 electronic was used for checking on residents in the facility, is that correct? Answer: Yes. Kifowit: What is the normal number of veterans that pass away? Do you have some idea of those going onto hospice? Are you aware of when individuals are going into end of life? Answer: There are several different buildings. I may not be aware of those at end of life in other buildings. Kifowit: On average, I mean 13 sudden deaths is abnormal, one sudden death is abnormal is that correct? Answer: That?s correct. Kifowit: Has there been any emotional support provided to the nurses to help them through this time? Answer: Not at this time. But it is not unusual for us to have a large number of deaths, and they are in pretty compromised health when they come to the facility. Kifowit: Can you expound on how important personal attention and personal knowledge is for caring for veterans? And how floating nurses could effect the care? Answer: It is very beneficial to know the patients. Kifowit: If you don?t know the patient are you are taking vital signs you still might miss some or may not be aware of some things that don?t show up? Answer: Exactly. kifowit: Director Jeffries stated that the CDC report said the CDC guidelines and healthcare protocol should be written out. But Jeffries said they change frequently and they didn't have them written out. Answer: Yes Friese: Who would send that letter? Answer: I would have thought from the Administrator. Friese: What are the duties and responsibilities of the infection control nurse? Answer: I’m not really sure. Keeps us well informed as a rule. Monitors chest X-rays and urine analysis and is contact with staff nurses. Friese: Your friend, did that misdiagnosis come from a private doctor? Answer: Yes. Phelps Finnie: The nurse to patient ratio for those requiring more care? Answer: I can’t accurately comment. In my unit there is two staff per shift. The skill areas are better than that. Phelps Finnie: Do all of these folks get medication? Answer: Yes they are. Phelps Finne: That’s a lot of medication for that staff level. Some of the residents have their own physicians off grounds as I understand it. What is the procedure for notifying those physicians? What is the trigger, is the fever is a certain amount, if there’s a cough? When is the urine test done? Answer: We would contact our physician and then take those next steps. Righter: (Reviewed names and titles of witnesses) Righter: Would a 12 hour shift make a difference? Anne: I’m going to step in here because that is a state contract. Righter: Well the impact could make it better for the staff, but I understand your point. Anne: I think that’s debatable but I think Nettie will answer whether the local staff has asked to go to a 12 hour shift. Righter: I’m talking about 12 hour shift for the nurses. Would a 12 hour shift be something that could be a useful tool in the context of this issue with regards to staffing? Anne: Well.. Righter: Ma’am I am going to please ask you to not talk and let her answer. Nettie: It is possible. Righter: That’s all I wanted to know because it came up yesterday. That is what we have been attempting to do for the last 2.5 years. We have bee supporting the state in this process. It has been a challenging process within our department. I am excited with the procpect of a longer term solution to this problem. I hope I can answer the committees questions on this mater. Costello: So the pipes are color coded, so one of the water checks that was done, I believe on Dec. 18, 2017 it showed positive for legionella before the water got to the facility, so in the public line. If you look at other nursing homes, people that are elderly maybe in the hospital system, are you aware of anyone else that may have died from legionella? Answer: I'd like to walk through our community cases. In 2105 the ?rst case we were aware of was a veteran, at that time we were averaging one case of legionella a year. So we asked the veterans home to begin surveillance to pick up additional cases. At that time we were alerted to a case in the community, that individual had a travel history and many time legionella you can't find a source for, and those cases were not associated. When we were alerted to the second case at the veterans home, actions began and we casted a big net. When we stated this to our providers we did not name the veterans home but just a long term care facility has multiple cases of legionella and we would like providers to look for test for it and report results Within that context we found additional cases in the community that were not able to be traced to the veterans home. We had four cases in the community that we did not have attributable to any specific cause. We were concerned in the broader sense and reached out to the city on Quincy and tried to get a handle on, was this central to a single entity? Costello: I?d be interested in pneumonia deaths in Quincy during that time Answer: We will bring those to you. We do have some of that data, and we will reach out to providers to be able to get those through diagnostic codes. Capitol Development Board: Amy Ramono, Thomas Bukite, Director Jeffries and Director Shaw Schimpf: Tom you were the one who produced this report Answer: Yes Shimpf: This has been a problem since 2015. If we didn’t have it under control in 2015 why is it just now that we are receiving this report why not in 2016? Answer Amy: So in 2015 we started communication with IDVA after they reached out to us for additional help. It was at this time that additional funding was identified and an emergency project was started. At that time we looked into providing design, production, and engineering support to address abate and mitigate the legionella found. Yes in 2015 CDB initiated an emergency project that started in October of 2015. The two main items accomplished were the design and construction of a new water treatment facility which would chemically treat the water. And The water being treated by heat at each individual building closer to the time of use. There were 700 mixing valves need to cool the water closer to use. Shimpf: My question still is why are we just seeing this report now rather than in 2016? Why didn’t we have a comprehensive report and look at the options then rather than 2018? Answer Amy: The thing that we were addressing were more immediate needs. Looking at a focused report to abate legionella. The report in 2018 look more broad at what the solutions are. The first was addressing more immediate concerns but we have exhausted almost all of those, we need to look more at long term solutions and things that may cause disruption to the site and residents. Cullerton: I have to agree with Shimpf Director Jeffries it was in the last hearing we asked you to come up with this report and it looks like it was commissioned after that. I have not received any emails so I have to take Director Jeffries at her word that she asked for CBD to do a report before this and yet yesterday at 4:30 is when we got this. I would like to see the communication before the FOIA but if not it seems like a long time to wait for a report. Answer Amy: Yes and I am not cetin on the timeline but a report was commissioned in 2016 but it was not a full report and was more focused. Cullerton: So you did a report in 2016 and you never reported anything to the GA until now and we had two more outbreaks. Answer Amy: The report was part of CBDB emergency project so we were not in regular contact with the GA at that time Cullerton: Director did you see the report in 16 Answer Jeffries: Yes I did Cullerton: And no recommendations for appropriations for the general assembly then? Answer Jeffries: Just to provide a little context. In 2015 we did ask CDB to come in and they were essential you us and in 16 we continued to work with them. Bric did come back and did make suggestions on what could be done. Those were not in line with CDB and we followed CBD. BRIC guild lines we felt were superseded by the CDB so appropriations were not needed at that time. What you see today in the report is a much larger scope than we had seen before. Welch: When did Governor know and what did he know Answer Jeffries: We addressed that at the last hearing. We have those timeline and you should also Answer Shah: Gov was made aware on Aug 24 th Welch: At the last meeting you said you would provide those emails Answer Shah: We received the FOIA and we are in the process of getting those documents Cullerton: That is not the case let me clarify at the end of the last hearing I asked if they could provide all the emails. The key is that my request is not a FOIA request. The response back was that it was to broad of a request. I asked you a direct question and you said yes. That should have been your answer at the end of the last hearing Welch: it seems like the taxpayers deserve an answer to what happened. So when will you provide that information and share those emails Answer Shah: We will when we sit down and narrow the scope with Cullerton’s office, Gov’s Staff and our departments. We have to comply with FOIA Welch: We need those email now will you provide any and all emails Answer Shah: Subject to the FOIA statue yes Welch: What did the governor do and was he involved in not informing the public for 6 days Answer: Shah: Let me clarify there was not a decision not to inform the public there was a discussion as with any outbreak you need all the information before going so you can answer all questions the public may need. We have to know what is going on so we could provide information. Welch: when did you notify the Gov Answer: Aug 24th Welch: At what point did you tell the Gov you were ready to go public Answer Shah: We notified the Gov on the 24 and had already been in contact with IDVA and our primary focus was working with them to aid in this outbreak. th Welch: Did Darlene Singer write an email blaming Duckworth for the entire situation in Quincy? Answer Shah/Jeffries: not that we are aware Welch: Would your emails help shed light on that Answer: I am not aware of what you are speaking about Welch: Would she have emails on the QVH and can we get those as part of the FOIA Answer: Subject to FOIA Statue Welch: Do you blame her Answer Shah/Jeffries: no Welch: Lets go back to that Aug 24th date that is when you told the Gov. I would like to say that everyone going forward need to not make political assumptions here Welch: Chairman it just feels like there is information here that is intentionally being with held. Cullerton: We are continuing on the process of obtaining that information but We also have an auditor general resolution coming forward and an auditor general that could go through all the QVH going forward. It is not what I wanted but we do have that option. Castro: Can we see the 2015 report mentioned by you and this report we got yesterday Answer Jeffries: Yes Castro: When Answer Jeffries: we are happy to get that for you Castro: They mention the 2015 the report 9 times so why if they mention it so much have we not seen it Answer Jeffries: We can look into producing it after the hearing Castro: Shah ahead of the hearing you said that you were frustrated with Blessing. You said that state facilities were going to do it. Yet the Gov. convinced them Answer Shah: We worked with them and they were able to finally get the testing online and that is all there is to that. It is now available and we can get results sooner and that is all that matters Castro: Did you ask for the GOV to weigh in before 2018. Why not ask sooner Answer Shah: We personally sat down Jeffries and I did and talked with blessing hospital Castro: But I am asking about the Gov why not ask him to weigh in when it was failing Answer Shah: I disagree it wasn’t failing testing like this takes time even more so for a large hospital system. Through our conversations they were progressing on getting the testing set up Castro: You have the Gov of Illinois that went and asked and it was done. So why not use him. Do you still not have his phone number. Answer Shah: I do have his number. I am at the Gov disposal he is not at mine. Bringing him in would have been heavy handed. We had the plan in motion. Castro: But it took 3 years Answer Shah: I wish it had been faster but testing like this takes time Castro: You announced before the hearing that state labs were going to do the testing and the Gov goes and talks to Blessing and they all the sudden can begin testing. The fact though is you did not directly inform the Gov of the issue and now that you claim you have it you could have picked up that phone and asked for his help. If it was heavy handed so be it. Answer Jeffries: We have established 2 one was announced and one will be. The first is a water management task force that will provide oversight to the water management working group. The timeline is to produce a continuous improvement with in 60 day and the final plan to be to the GA in 90 day. We are trying to do this in a timely fashion Chairman: Would it be wrong to be hopefull that part of this may come out and be in favor of building a new facility Answer Jeffries: No the second is an infrastructure taskforce. They are focused on current and future infrastructure projects for the home which may very well be a new facility Chairman: You had said that you would get me the number of patients that could not make there own medical decisions legally did you bring those/ Answer Jeffries: We have 47 residents in the dementia wing Chairman: If you can outside those are there more that can not legally make decisions for themselves. Just a last comment I would ask that we get past the partisan issues. I would hope that we could get this done cooperatively and willingly. From the bottom of my heart I ask that we do this voluntarily and expeditiously Kiffwit: Are you open to creating an infectious disease committee are you open to working with ACSFME to create one Answer Jeffries: I was wrong previously when I said we didn’t have one. One has been identifies that would generate federal revenue to complete. There are some projects that allow for federal reimbursement.
