I have to report the following -- This file relates to the sudden death of Aaron Leafe on 24/12/94 who died at Ivy Cottage children's care home on the 1216/1 0. At 2205:53hrs on Saturday 12'" June 2010, Police received a call from EMAS to attend a children's home called lvy Cottage, Press Lane, Old Tupton, Chesterfield. This was following the report of a 15 year old male hanging that was unconscious and not breathing. Police Officers--attended the scene, shortly followed by - . The Officers reported that a male had tried to hang himselt tram a curtain rail in the rear ground floor office area; however he had been cut down before their arrival. At 2232hrs the male was pronounced deceased by the ambulance staff. The male was identified by care staff to the Police as being Aaron Leale. A scene log was commenced. (full list recorded on the scene log). Also attended was On the Police incident _describes the scene as being split between two rooms The room where Aaron was found, as being a ground floor rear room that is known as the 'Education Room'._ reports that the deceased had tied a ligature to the curtain pole, stood on a chair and placed the ligature round his neck and then either dropped or kicked the chair away. The room where Aaron was lound was believed to have been locked from the inside and then opened by staff using a key. The second scene was where Aaron was moved to after he was found. From enquiries it transpired that Aaron had been a resident at Ivy Cottage since 7/5/10 and prior to this he had been a resident at Duncanwood Lodge in Worksop. Both the children's homes are run by 'Keys Group'. Aaron had moved care homes after he had been arrested over an incident with another resident at Duncanwood Lodge. Aaron was known to have previous mental health issues and a history of self harm that included overdose, cuts to arms and a previous attempt of trying to hang himself. The later incident was on the 1" of May 2010. Since moving to Ivy Cottage,on the 7/5/10, Aaron had further self harmed on the 11'" June when he also wrote a suicide note. After the game, the other residents played on computer games whiist Aaron changed into a suit that he had purchased for a pending court appearance. Also during this time, Aaron showed the other residents two 'Raps' that he had written about hanging himselfthat night. Due to the sexual assault Aaron got moved to another home in Nottingham. It was here that he began smoking and his attilude got worse. Aaron asked to look after his mother. This was taken as if Aaron might do something to himself, so he asked him if he was going to do something stupi Aaron mumbled a reply. On the 12"1 June 2010. Deborah received another phone call from Aaron nd 3 am he spoke Again he did not sound happy and he said "look after my Mu made light of the comment as he had said this previously. Nothing else was said. Later on all of the staff and the children watched the football on TV. with Aaron coming in and out of the room at various points' 80 minutes into the ame Aaron came into the room holding a piece of paper (exhibits and read the first line which said that he was going to hang himself tonight, At the time Aaron was wearing his suit. Aaron said that he was going to hang himself and kill himself. He asked- if he wanted to do it with him but he didn't want to do it himseli.-declined. On the 10'" June. Aaron continued to talk about killing himself and he continued to ask- If he also wanted to do it.- again declined. He then showed- his wrists and the previous scarring. Aaron said he was still going to kill himself. -later saw some ropes hanging in the tree house. He asked Aaron if he had hung himself. which he replied On the 12m June,- saw Aaron with a white shoelace tied to a black shoelace. The black shoelace was tied in a hoop. He saw Aaron take this out of his pocket and he also saw him with it in his bedroom. -was shown exhibits and which looked like the laces he saw Aaron with. They watched the football and during the 80'h minute Aaron showed-a piece of paper with a rap on it in which he mentioned about hanging himself. He then asked-to go into the education room with him. Here-saw a rope tied in a hoop on the aorta rail. Aaron asked for a septic wipe. Aaron asked what would happen if someone self harmed. The procedure was explained to Aaron re first aid etc. Later. it became apparent that Aaron had hurt himself which-examined to find that Aaron has superficial cuts to his arm. This was treated and Aaron refused anything further. Incident logs were created. Aarun retused a bandage and didn't want any further medical attention. Staff would normally carry on as normal, but keep an extra on the resident. They had no real concerns for Aaron. This comment was reported to- who informed her line manager- who replied that no further action was necessary and the comment was to remain on his file. They went to check and they could hear the bathroom taps running. They checked Aaron's bedrcom and they found his pyjama bottoms on the floor covered in blood. Aaron then poked his head out the bathroom door and he was asked if he had cut himself. Aaron told him that one at the cats had scratched him. Eventually Paul managed to get the truth out of Aaron and he disclosed that on the 1" March he had tried to hang himself with a belt, een sexually abused for the past 4 years by a care worker from his previous care home in Leicester, Aaron named the care worker as Anwar lsmail. However. they remained in contact afier they managed to exchange numbers Ismail would collect Aaron from the care home and drive him around and supply him with cannabis and alcohol. So much so, that he wanted to bulk up in case he was sent away. Aaron had conversations with many people over this issue and even made comment that he would rather die then be sent away. He was clearly confused and upset by what was happening. Aaron changed into his suit and wrote 2 'Raps'. Nothing in his behaviour arose any suspicion amongst the staff or residents, despite the residents stating to them in person that Aaron had said he was going to hang himself and he wanted them to do it with them. From checking the house and grounds after his death, further nooses were found in his bedroom and also in the tree house, This is consistent with what the resident's state in their statements. An assumption would be that Aaron had been experimenting with the nooses previously especially as one of them had snapped possibly at the point of strain. This raises the question as to whether Aaron had been experimenting, tried and failed in the past or as - in the past when he self harmed whether he was seeking attention rather than wanting to carry out the actual act.
