Call meeting to order
Rudy Azevedo called meeting to order 13:35.

III.(a) John Erquiaga moved to accept the agenda.
Ray Gorzell 2nd motion.
III.(b) Public Comments
Numerous public comments, regarding worries of budget and debt of the SVHCD. Concerns of
when and how long it will take to find out outcome of financials and possible Audit findings.
Concerns of how much the board will be involved in the future regarding financial decisions,
and their role in upcoming financials. Concerns over SVHCD website being up to date with Board
Meeting minutes as well as Audio Files of previous meetings.

Items for Discussion and/or Action:
1. Election of Board Officers-Discussion/Action
Rudy Azevedo tendered his resignation as President, due to medical and health issues.
John Erquiaga moved to accept tendered resignation.
Bruce Tozier 2nd motion.
John Erquiaga moves to table the election of new President until next board meeting.
Ray Gorzell 2nd motion to table new President election.
2. Administration and Governance of the District-Discussion
Rich Cornwell (previous CEO) was terminated. Board member Bruce Tozier is interim CEO for the time being.
This matter was approved by other board members as well as approved by the State. SVHCD will advertise and
take measures to gain a replacement for the CEO position. Willetta Callaghan resigned from the Administrative
Assistant position, Bill Bostic is currently filling in for this position, and will be with us for at least 90 days.
Suzie Wright resigned from her position as D.O.N.,,RN Leon Phillips has taken the spot of interim D.O.N. for the
time being, until the position can be filled.
Traveling Radiology Tech Ruth Ann Baxter chose not to extend her contract, we have coverage for the next 2
weeks, with Bill Boland, and a Xray tech from Lakeview to cover weekend.
SVHCD is currently contacting local accounting firms to discuss help with financials and to look into doing an
Audit to see where the facility stands with financials.
Received new patient today in LTC/Swing.
Physical Therapy room at clinic next door is moving along quickly, and almost done. It is taped and trying to
finish soon.
Dr. Roberts fund is going to try to do some fundraising to earn some money to buy some Physical Therapy
exercise equipment for the new PT room.
