Charge 1: Performing outside duties during duty hours

Specification: On June 19, 2014, you reported to duty with your personal cooler
containing fruit which you stored in the Nutrition &Food Service refrigerator. You
brought thefruit as refreshments forthe Community Living Center (CLC) Senior Prom.
This event is sponsored by volunteers and the CLC staff after duty hours. This event
was not sponsored by CAVHCS leadership. You were witnessed preparing
refreshments forthe CLC Senior Prom during duty hours. You were not authorized to
perform these duties while on payroll.

Specification: On June 29, 201 4, you prepared food for a church eventduringduty
hours inthe Nutrition Food Service kitchen.

Charge 2: Solic'ning employees to participate in unauthorized duties

Specification: OnJune 29, 2014, you approached --

and asked himto assist you inpreparingfood foryour
church. declined to assist you and stated, "That is not my job. This is

your private catering business."

showing why the charge(s) is (are) unfounded and any other reasons why your suspension
should not be effected. Your written reply should be submitted to the Director, Central
Alabama Veterans HCS. The Director, CentralAlabama HCSwill receive youroral reply orwill
designate anofficial orofficialsto receive it. You may make arrangements foryouroral reply

official duty time for reVieWing the eVidence relied on to support the reason in this notice,
preparing a written reply, securing aflidavils, and lor making a personal reply. Arrangements for
the use of official time or requests lor additional lime should be made with me.

4. Your priordisciplinary record includesaS-Calender Day Suspension on February 5,2008
for Failingto Follow Proper Leave Requesting Procedures. This past record will betaken into
account in determining proper disciplinary action, if one or more of the above reasons are
sustained. You may reply orally or inwriting, or both orally and inwriting, with respect to the
previous infractions and penalties and you may submit supporting evidence, including
affidavits. In this regard, you may make a statement expressing your views as to the
consideration to be given such past record indetermining proper action.

5. You may be represented by an attorneyorother representative of yourchoice at all stages
of this matter. up to and including the issuance of the decision. Please advise me inwriting of

6. The final decision to effect the action proposed has not been made. The Director, Central
