This decision was made based on your coming to work on Tuesday, June 20, 2017 under the in?uence of alcohol. 011 Wednesday, June 28, 2017, you met with me and your UniServ Representative, Rina Paul, in a loudermill hearing. At that hearing, a decision regarding your termination was held in abeyance until, Thursday, July 27, 2017, at which time the regularity of your attendance at AAmeetings, regularity of meetings with the Employee Assistance Program Manager, proof of alcohol treatment assessment, and enrollment in an alcohol treatment program could be con?rmed and taken into consideration in making a decision. On Thursday, July 27, 2017, you missed the loudermill hearing when your SEA UniServ Representative changed the time of the hearing and was unable to notify you of the time change. When you didE arrive that afternoon you brought in proof that you were attending AA meetings, however, EAP Program Manager Catherine Erickson reported that although you had met with her on four occasions, you had missed a scheduled meeting on July 21 and had not called in to cancel the meeting or explain the absence. In addition, whereas you were assessed for alcohol treatment and had enrolled in an alcohol treatment program, you had done so only that morning. I took all of this information into consideration as I made a decision to terminate your employment with the District. You violated policy when you came to work under the in?uence of alcohol on June 20, 2017. However, prior to this time you had also received a three-day suspension, Written Reprimand, Documented Oral Warning, and two letters of warning documenting continued absences, tardiness and concerns that you were coming to work under the in?uence of alcohol. When you received the three-day suspension, you were told that if you came to work, were suspected to be under the in?uence of a controlled substance, and tested positive you would face termination. On June 21, 2017, after reporting to work, testing at US Healthworks revealed a concentration of .04 alcohol in your system. You failed to abide by directions a mere two weeks after you had received a 3-day suspension.
