This is to advise you that a Performance De?ciency Warning letter is hereby issued on the above
referenced project on: Performance Category Number 5 Coordination/Cooperation with CEI Personnel,
Property Owners and Utility Companies

The Performance De?ciency Warning is issued on the following:

Protocolto inform the failed to be followed when the lagler Bridge malfunctioned on Monday May
28, 2018. The was not informed of the malfunction until Saturday June 2nd 2018.

This Performance De?ciency Warning Letter, is to inform you that your company is out of compliance on
the above referenced Performance Category. Should your company fail to notify the CEI on future
malfunctions, a Performance De?ciency Letter will be issued, and your Contractor?s Past Performance
Rating will be adversely impacted.
