Your ?rm is hereby issued a Performance De?ciency Letter under the Contractor?s Past Performance
Report, Category 5 Coordination/Cooperation with CEI Personnel, Property Owners and Utility
Companies. This comes as a result of the CEI staff not being noti?ed that a malfunction occurred on June
4, 2018 at the 2:15pm opening.

You have 10 (ten) days from the date of receipt of this letter to appeal its issuance in writing and request a
meeting with the District Construction Engineer. If no meeting is requested, or a meeting is requested and
you fail to attend, then this De?ciency Letter shall become ?nal.
