March 12, 2018 Kevin Wells, General Counsel Maine Department of Health and Human Services 221 State St. Augusta, ME 04333 Dear Mr. Wells, The Bangor Daily News respectfully requests that the Maine Department of Health and Human Services make available for inspection and copying, pursuant to 1 M.R.S.A. §408-A, all public records in the possession or control of the agency that meet the following description: ● Records detailing the number of calls received by the Developmental Services crisis team and the number of face-to-face interventions in 2015, 2016 and 2017. ● Records detailing the number of stays in hospital emergency departments by clients who have had involvement with the Developmental Services crisis team in 2015, 2016 and 2017, including records detailing the length and cost of those emergency department stays. ● Records detailing the number of stays in psychiatric hospitals by clients who have had involvement with the Developmental Services crisis team in 2015, 2016 and 2017, including records detailing the length and cost of those psychiatric hospital stays. ● Records concerning whether follow-up was performed by the Developmental Services crisis team for clients who have been restrained, been in a hospital emergency department or been in a psychiatric hospital, including records detailing the nature of the follow-up. If the Maine Department of Health and Human Services objects to the provision of any of these public records on the basis that they are not subject to the FOAA or otherwise not subject to disclosure, please specify in writing the nature of the materials that the Department of Health and Human Services refuses to provide and the legal basis for that denial within five (5) days, pursuant to 1 M.R.S.A. §408-A. We are prepared to receive responsive documents via email at the address provided below. We are also prepared to inspect and copy the records during regular business hours.
