On Sunday, November 22, 2009 at approximately 1:00a.m., a fire occurred at a residence at 503 Martindale Street in Lancaster, Texas. Bar testing showed gas concentrations in front of and along the side of the residence. Due to the amount of water around the residence, bar-testing along the service line and in the yard could not be conducted. The water and sewer lines were checked and no gas concentrations were found. According to Atmos Personnel, subsequent investigation revealed a butt fusion joint that was separated and gas blowing from that failed joint on a three-inch polyethylene main in the front of the residence. The operating pressure on this main at the time of the incident was approximately 23psig. It appears that this main was installed in 1980 No. 80-5311). There is a four-inch polyethylene main along the side of the residence on Stewart St. that was also installed in 1980 (ER-806311). It appears that the service line that supplied 503 Martindale Rd is a 3/4- inch polyethylene line. The last odorant test taken at these locations was on 4/ I 3/09. Pipeline Failure Investigation Report Odorization Two odor tests were taken on 11-22-09. #1 - 322 Stewart Ave. at approximately 9:40a. Heath Odorator-l931-3. The odorant concentration level was rated as readily detectable at 1% gas-in-air. #2 503 Martindale Rd. at approximately 10:25a.m. using Heath Odorator-l931-3. The odorant concentration level was rated as readily detectable at 1% gas-in- air.
