As a matter of record, this alleged violation is based on the status of the ATMOS replacement at the time of the incident and incident investigation. At the time of the incident in Irving, Texas, ATMOS did not have an accepted or dedicated program in place to recondition or phase out steel service line segments at the time of the incident. The Commission will however agree that Atmos had initiated its efforts to recondition or phase out steel service lines based on prior discussions with the former director and the deputy director of Pipeline Safety. This program was in the development stages and no formal replacement program had been established or agreed upon. While the Commission agrees that ATMOS currently has committed resources dedicated to steel service line replacement, this was not formally put in place until the fall of 2010. With the resources committed to this effort, the final step will be the formal approval of the risk model that addresses Atmos' statewide removal of steel service lines based on its risk model, Actions taken to date are acceptable and no further information is required. Leak complaints must always be fully investigated to determine the source of the complaint. Frequently sources of odor may be identified, however the technician will ensure the source found is unquestionably the source of the original complaint. Often environmental conditions hinder an investigation and render some equipment unusable. Some wet soils will alter gas migration patterns in ways that may seem to be contradictory to common sense and logic. Surface sampling equipment and visual inspections may not be sufficient without providing gas a manmade path of least resistance to the surface, such as a bar hole. Additionally, many bar holes may be necessary to ensure a path is close enough in proximity to a leak to provide gas a path to the surface. Technicians must be diligent, thorough, persistent, and resourceful in determining the source of complaints. When wet conditions exist, Mid-Tex employees performing inside and outside leak investigations and/or leak monitoring will include the following steps in addition to those currently required by procedures. • Remember weather conditions (rain, wind, snow, ice,- and water saturated soil) may prevent use of survey equipment. • Remember water-saturated soil may prevent the use of a CGI. • If weather conditions or water saturated soil prevent the use of survey equipment or the CGI, then visually inspect for the presence of gas: i.e., water bubbles, vapors, etc. • To allow visual inspection, multiple bar holes must be made in locations over the service line at the meter, at the street, at the tap location, on each side of the driveway or sidewalk to the house. Holes will also be made at the water meter box and foundation of the house. • Gas detection equipment should be used at the surface over holes or if water flls the holes observation for bubbles is required. •. If gas is detected at any point during the outside leak investigation, bar test at approximately 5-foot intervals until 0% to pin point the leak and determine the migration pattern. If at anytime during the investigation, natural gas is detected but the source cannot be found, contact the appropriate supervisor. • Survey or bar test customers dwelling and adjacent structures. The natural gas distribution system is owned and operated by Atmos Energy Corporation. The Irving distribution system has a maximum allowable operating pressure (MAOP) of 50 p.s.i.g, At the time of the incident on Sunday, January 31, 2010 the actual operating pressure was 48 p.s.i.g. at the Measuring Station designated as MS 4611 Irving North - Finley Road. The main supplying 3521 Finley Road is a four-inch coated "steel line and is located in front of the residence, in the parkway between the sidewalk and the street curb• The main was originally installed under Project Number ER 7123-66 on July 18, 1966. The service line for 3521 Finley Road was installed under Service Line Report Number 21907 with one-inch coated steel pipe, the total footage is unknown. The service for•3521 Finley was replaced with fifty-two feet of one-half inch polyethylene pipe on November 11, 1993. Atmos records indicate that under Service Line Report Number 20356 dated July 18, 1966 that a five-foot length of one-inch service line stub was installed for 3524 Finley Road. The odorant test was preformed at 747 N. Beltline Road at the Shopping Center (Reference OC-13794), The distance from 3521 Finley Road to the odorant test point is 1.25 miles. On Sunday, January 31, 2010 at 10:51 a.m., Atmos Technician, Denzil Dean performed an odorant test at 3309 Cardinal Lane and witnessed by RRC Investigator T. Sullivan. The threshold reading of 0.17 percent gas in air. The 1% Gas in Air rating was Strong. The service tap for 3524 Finley Road is located between the sidewalk and curb in front of the 3521 Finley Road address. Once the service tap for 3524 Finley Road was excavated, a leak was found, blowing at the tap. }[ On Tuesday, February 2, 2010 RRC inspectors witnessed the bar testing around the structure at 3521 Finley Road. On the west side of the structure, two high gas concentrations were detected at the foundation at bar hole number 16 and bar hole number 17 noted on the Atmos bar test data record. Atmos, at the request of RRC inspectors took gas samples of the two high readings. In Armstrong Forensic Laboratory, Inc. Report Number BOGA0646-1 dated Friday, February 5, 2010, the samples taken by Atmos from the west side of the structure at 3521 Finley Road showed to have contained "Pipeline Natural Gas" in both samples. On Friday, February 19, 2010 at 10:31 a.m. Atmos Energy obtained soil samples at the site. In Armstrong Forensic Laboratory, Inc. Report Number BOGA0646-2 dated Wednesday, February 24, 2010, the samples obtained by Atmos from the structure at 3521 Finley Road showed to have contained "Pipeline Natural Gas" in both samples. In addition, Atmos technicians, witnessed by RRC inspectors, bar tested the main along Finley Road in front of the residence, there were some gas concentrations found, but appeared to be residual concentrations since the back-fill was made with non-re-mediated soil. The main on the west side of the residence on Cardinal Lane between the sidewalk and street curb was bar tested and there were no gas concentrations detected. Sullivan was notified of an explosion. 6:47 a.m. RRC investigator T.E. Sullivan received telephonic up-date from Atmos Energy on-site coordinator. 7:05 a.m. RRC investigator T.E. Sullivan en-route. 7:45 a.m. Atmos Tech surveyed the sewer stacks of four houses north on Cardinal, four houses south on Cardinal, four houses east on Finley, four houses west on Finley and on both sides of the Finley location. RRC investigator T.E. Sullivan arrived on-site received up-dates from Atmos Energy Emergency Response Coordinator and began investigation. 8:05 a.m. 10:51 a.m. Atmos and the RRC investigators performed an odorant test. 12:00 p.m. Atmos made telephonic notification to DOT, Petty Officer Procko # 930142 12:35 p.m. Atmos began excavating the tap for 3521 Finley. 1:02 p.m. The tap for 3521 Finley was uncovered. 1:16 p.m. Atmos performed a soap test at the tap to the service to 3521 Finley. 1:35 p.m. Atmos performed a pressure test of the service line to 3521 Finley. 3:30 p.m. Atmos removed the tap for 3521 Finley. 5:07 p.m. Atmos in the process of hand digging the service for 3524 Finley. Audible hissing and strong gas odor observed while digging. 5:47 p.m. Atmos discovered a leak on the service line for 3524 Finley and subsequently graded it as a Grade 1. Atmos removed the regulator and meter from 3521 Finley and Unified Investigations took it into custody. 8:32 p.m. Monday, February 1, 2010 1:20 a.m. Atmos eliminated the Grade I leak on the tap at 3524 Finley., 2:43 a.m. A section of the main, the service line tap and a portion of the service line were removed and taken into custody by Unified Investigations. A pressure test was performed on the service line to 3524 Finley, the service line held the pressure during the test. The service line for 3524 Finley was Replaced and Tested by Atmos. Subsequent investigation revealed that there was a leak on a WeldOn Tee Tap with a Compression Coupling Nut connecting the coated steel service line for 3524 Finley Road. The main supplying 3521 and 3524 Finley Road is a four-inch coated steel line and is located in front of the 3521 Finley Road residence, in the parkway between the sidewalk and the street curb. The natural gas distribution system is owned and operated by Atmos Energy.
