On Tuesday, December 6, 2011 at approximately 7:02 a house explosion and fire occurred at 4908 Orien Street, Haltom City, Tarrant County, Texas. Subsequent investigations of the incident showed gas migration found along the service line supplying the residence. A compression coupling was found on the service line between the service tap and the residence but was damaged by Atmos Energy during excavation of the area. There was one injury as a result of the incident. The natural gas distribution system is owned and operated by ATMOS Energy Mid-Tex. The natural gas distribution system is owned and operated by Atmos Energy Corporation. The Haltom City distribution system has a maximum allowable operation pressure (MAOP) of 60p.s.i.g. At the time of the incident on Tuesday, December 16, 2011, the actual operating pressure was 26p.s.i.g. This was noted at the District Regulator Station designated as DR8328A located on Glenview Drive Jackie Lee Drive. The main supplying 4908 Orien Street is two inch coated steel line and is located across from the residence, between the property line and the street curb. Records of the main supplying 4908 Orien Street were not available for review. The main from Orien Street was originally installed under Project Number ER 4044-52 on February 5, 1952. The service line for 4908 Orien Street was installed under Service Line Report Number 9991-74 with a half-inch polyethylene pipe dated September 5, 1974, the total footage was 45 feet. The original service line being used as a conduit for the inserted ply lines. An additional Service Line Report Number 892-2 dated September 9, 1974 noted a compression coupling (half inch by one inch) was installed that joined a half-inch service line with a three quarter-inch service line. On Wednesday, December 7, 2011 at 11:40am, Atmos Energy Technician, Denzil Dean performed an odorant test at 5005 Orien Street and witnessed by RRC Investigator Goodluck Onukwufor. The threshold reading of 0.17 percent gas in air. The one percent gas in air was rated strong. Atmos Energy and RRC (Railroad Commission of Texas) then began to determine gas migration patterns. Additional RRC personnel and Atmos personnel arrived and began bar testing the areas where conditions permitted. The soil around the structure was saturated with water from the rain as well as the water the Haltom City Fire Department used to extinguish the fire. The combination of water did not affect bar-hole testing around the perimeter of the structure. The service line at 4908 Orien Street was not excavated because the flow of gas was isolated at the north end of the compression coupling utilizing a squeeze-off tool. The bar hole testing revealed gas concentrations in front of 4908 Orien Street near the water meter, along the service line and along electrical conduit from the decorative pole. The service tap for 4908 Orien Street is located between the property line and street curb in front of the 4905 Orien Street address. On Wednesday, December 7, 2011, RRC inspectors witnessed the pressure testing of the service line from the compression coupling to the service riser stop. The compression coupling was removed prior to pressure testing. The compression coupling was damaged as a result of an Atmos backhoe operator during excavation. The area that the operator excavated was in the lawn of 4908 Orien Street and appeared to be approximately twelve inches in depth. The damage to the compression coupling occurred prior to the arrival of RRC inspectors. The compression coupling was secured and sealed by Unified Investigations Sciences, Inc. A test was conducted of the service line from the riser to where the compression coupling was removed. A leak was noted at the riser stop valve. The riser stop valve was removed from the service line and the service line was capped. Then, the segment was tested again from the cap service line to where the compression coupling was removed. There was no leak detected. A test was conducted from the service tap to where the compression coupling was removed. There was no leak detected. Liquid soap was applied around the service tap and no leak was detected. At the request of RRC inspectors, Atmos Energy collected three soil samples along the service line of 4908 Orien Street. On Thursday, December 8, 2011, Atmos Energy hired a third party consultant to investigate the incident at 4908 Orien Street. Railroad Commission of Texas inspectors observed the investigation by Sammy Russo. Additional bar hole testing was performed under the direction of the third party consultant hired by Atmos Energy. Although a request by RRC personnel had been made to dig up and test the bare steel casing, the request was refused and the testing was conducted only after the third party investigator arrived and made the same request. Atmos Energy pressure tested the north end from compression coupling to the service tap at 46psig for ten minutes. No leak was detected. Atmos Energy pressure tested the one inch bare steel service line casing at 46psig for ten minutes. No leak was detected. Atmos Energy excavated the decorative light pole to identify the position of the concrete and electrical conduit. The concrete was placed directly on top of the one-inch bare steel line casing. Atmos Energy removed service line from casing and taken into evidence by Unified Investigations 8: Sciences, Inc. 3'36pm RRC Inspectors requested soil samples to be collected along service line. Unified Investigations 8: Sciences, Inc. representative collected three soil samples from 4908 Orien Street. l6 RRC InSpectors requested service line bare steel casing to be pressure tested. Atmos Energy representative denied 3:45pm request. 4:20pm RRC Inspectors departed from the incident. Thursday, December 08, 2011 9:04am RRC Inspectors arrived at the incident with Atmos Energy and APR Consultants. APR Consultants was hired as a third party consultant to investigate the incident on behalf of Atmos Energy. 9:453.? Atmos Energy and APR consultants performed additional bar hole testing along the service line, property and main. 1: 45pm Atmos Energy pressure tested north end from compression coupling to the service tap at 46psig for ten minutes. No leak was detected. 2:45pm Atmos Energy pressure tested the one inch bare steel service line casing at 46psig for ten minutes. No leak was detected. Atmos Energy excavated the decorative light pole to identify the position of concrete and electrical conduit. The 3:22pm concrete was placed directly on top of the one inch bare steel line casing. No corrosion appeared to be located on one inch bare steel line. pm Atmos Energy re-applied the decorative pole to its original state and covered the excavation from the compression coupling area. 4:30pm RRC Inspectors departed from the incident.
