Bar testing showed gas concentrations in front of the residence. Due to the amount of water around the residence, bar-testing along the service line and in the yard could not be conducted. The water and sewer lines were checked and no gas concentrations were found. According to Atmos Energy personnel, subsequent investigation revealed a butt fusion joint that was separated and gas blowing from that failed joint on a three-inch polyethylene main in the front of the residence. One person received burns as a result of the incident. The gas distribution system is owned and operated by Atmos Energy, Inc. Investigation (Include a probable cause statement for incident/accident investigation) On Sunday, November 22, 2009, Commission investigator, J. M. Kerl, conducted an onsite investigation. After the initial investigation, selected maps, records and procedures were reviewed. The results and findings of the investigation are included on the PHMSA Form 11- Pipeline Failure Investigation Report and are attached to this inspection packet. Emergency Response (Complete for incident/accident investigations) The incident occurred on Sunday, November 22, 2009, at approximately 1:00 a.m. Atmos dispatch was notified at approximately 1:16 a.m. An Atmos field service technician was notified and arrived onsite at 2:00 a.m. Upon arrival, the Atmos technician began performing a leak investigation. He did not detect gas concentrations in the sewer cleanouts and vents of the surrounding structures. He began bar testing the area and gas concentrations were found in the street with the highest gas concentration of 83% found in front of the residence at the corner of Martindale Rd. and Stewart St. The technician notified Atmos personnel, supervisors and Lancaster Fire officials of his findings. As a precaution, the Lancaster City Fire Department evacuated nearby residences. Atmos Supervisors and Managers arrived to assess the situation and called in additional staff to assist with the situation. An Atmos construction and maintenance crew began excavating the main and the leak on the main in front of the residence was eliminated at approximately 6:00a.m. Atmos begins on-site testing of the area. The testing consists of bar testing in, under and around structures, and testing of all accessible sewer openings. Atmos personnel performed leak survey of the area using flame ionization unit, Trak II and a Heath RMLD unit. Atmos technicians began bar testing every five feet to detect natural gas migration patterns. Atmos began the aeration process to remove residual gas from the ground in the affected area. The incident was telephonically reported to the Railroad Commission Pipeline Safety Emergency phone number at approximately 4:21 a.m. on November 22, 2009. Atmos reported the incident as a structure fire with gas concentrations found in the area. It appears that Atmos followed their procedures during all activities after the incident occurred.
