Atmos Energy employees arrived on site at approximately 20:55 local time and began implementing their emergency procedures. Atmos Energy determined the cause to be a service line tap at 918 Cottonwood Circle Corsicana, TX. There were no injuries or fatalities sustained as a result of this incident. Investigation (Include a probable cause statement for incident/accident investigation) Field Investigation: On March 12, 2016, Inspectors Michael Parker and Joey Bass arrived onsite to investigate an incident at 1511 W. 13th Ave in Corsicana, TX. Upon arrival it was noted that Atmos was conducting leak surveys. Atmos also had a pumping unit at the house to remove the gas venting underground. During the investigation with Atmos employees it was discovered that 1511 W. 13th Ave. has never had gas service. The natural gas was found to be coming from underground underneath the home and venting out of the bath tub, slab cut out access/chase. Atmos personnel witnessed that the first odor test at 2:00am on March 12, 2016 determined that the odor concentration was one-fifth of the lower explosive limit and was in compliance with Part 192.625. An odor concentration test was performed by Atmos which was witnessed by the Railroad Commission at 5:40pm on March 13, 2016. The test witnessed by the Railroad Commission was in compliance with Part 192.625. A special leak investigation was conducted to encompass 6 block radius and a underground leak was found at 918 Cottonwood Circle at the service tap on the main. After repair the leak at 918 Cottonwood was determined by Atmos to be the source of the venting natural gas at 1511 West 13th Avenue. Record Investigation: Inspector Michael Parker conducted an investigation with Atmos Energy Mid-Tex Division on April 4, 2016 to review records of the incident. Records for leak surveys, odor concentration tests, operator qualifications, and joining procedures were reviewed to determine adequacy of compliance.
