One injury & hospitalization was reported, the street has been closed to traffic, and evacuation of the home across the street. No fire was reported and the Media is present. The fire department was also on scene and the site is safe. ATMOS Energy was contacted at 8:00 AM on April 22, 2018 by Johnny Price, the son-in-law of the homeowner. Mr. Marlo Sutton of ATMOS Energy notified the RRC call center at 8:30 AM. As of 9:45 AM, ATMOS Energy is bar testing the area and have discovered gas readings in the street. The evacuated home across the street has been declared safe and the residents returned. Atmos was contacted by Johnny Price, homeowners son-in-law Note: There was an 8-hr. gap between the report to 911 and the notification to Atmos. Currently, there is no explanation as to why there was a delay in the notification to Atmos. 8:30 AM Marlo Sutton (Atmos) made the call to the RRC. 10:51 AM Leo Mundine received the call from the RRCs call-center 11:13 AM Bob Reed received the call from Leo Mundine 11:48 AM Bob Reed departed home, to travel to the accident site 1:13 PM Bob Reed was on-site in Caldwell, Burleson County (76.1 mi.) 900 ONeal St. 4:21 PM Bob Reed left the accident site 5:35 PM Bob Reed returned home (2)Pipe Details: (a)Main is 2 coated steel, in the edge of the street (ONeal Street) installed in 1965 (b)Home-owners service line was not involved in this accident. (c)Leakage was found at a mechanical coupling on a 1-1/4 service line near the tap on the 2 main, approximately 3 below the street level, in front of the homeowners house. This service line, with couplings, ran under the street to a neighbors house had been installed, also, in 1965, according to Atmos. (3)Repairs, Leak Investigation Activities and Other Details: (a)Accident Leak Survey/Bar-Testing/CGI : Results of initial bar-testing and CGI readings were reported by Atmos to be localized to an area in front of 900 ONeal Street. However, additional gas readings taken during this initial survey were found in the sewer system near the house. (b)Additional Leak Survey Activities: Atmos had begun a leak survey using specialized equipment and crews during the Late afternoon of April 22nd. They will be surveying an entire map-sheet approximately 6 blocks in every direction from the accident site and the sewer system in the vicinity of the leak-site. (c)Odor-Test was performed at 12:40 PM, 04/22, at 808 N. ONeal, by Atmos crew. (d)Repairs: The 1-1/4 steel service line, on the north side of the street, is being replaced with ½ pe line, for a distance of 100 to a meter set on the south side of the street, directly across from the house involved in the accident. The ½ pe will be inserted into the existing steel service line. The 2 main, in the edge of the street, is not affected by this repair. (e)EFV: The home-owners (Gene Barbers) service line had been replaced in 2013, at which time an EFV had been installed. The EFV appeared to have worked as it should, immediately shutting-off the gas at the time of the explosion. (f)Gas had been turned-off to five customers on ONeal Street. Starting at 3:30 PM, gas was being restored to these customers. (4)Injuries and Damages: The home-owner, Mr. Gene Barber, Mr. Barbers house was severely damaged.
