EVIDENCE OF INJURY: Note: The numbering of the gunshot wounds is for descriptive purpose only. Gunshot wound #1 to the head (fatal): Entry: Right posterior temporal, 6 inches below the vertex and 3 inches to the right of the midline, round measuring about 3/8 inch with abrasion. There is no soot or stippling. Exit: None present. Projectile: Large caliber, copper jacketed, deformed bullet recovered_from right occipital skull near midline. Deposited into safe at 1650 hours. Trajectory: Front to back and right to left. Path: It is through the scalp and skull and the projectile is lodged in the occipital skull with contusion of the cerebellum and subdural hemorrhage. Gunshot wound #2 to the head (nonfatal): Entry: Right temporal, 7 inches below the vertex, 3 inches to the right of the midline, 3/8 inch round with abrasion. There is no soot or stippling. Gunshot wound #3 to the right shoulder (nonfatal): Entry: Top of right shoulder, 10 inches below the vertex, 4 inches to the right of the midline, 3/8 inch round with abrasion. No soot or stippling. Exit: Back of left shoulder, 12 inches below the vertex, 9 inches to the right of the midline, inch and irregular. Projectile: There is none present. Trajectory: It is front to back, left to right and downward. Path: It is through the skin and underlying muscle of the shoulder. Grazing gunshot wound #4 to the right shoulder (nonfatal): Description: Grazing wound located on top back of right shoulder, 11 inches below the vertex, 7 inches to the right of the midline, diagonal measuring 2 inches 3/8 inch with the lateral margin inferior. There is abrasion limited to the skin and subcutaneous muscle. Gunshot wound #5 to the right arm (fatal): Entry: Inner aspect of right arm 7 inches below the top of the shoulder, round measuring about 3/8 inch with abrasion, no soot or stippling. Projectile: A small deformed bullet is recovered from the muscle of the arm and deposited into the safe at 1650 hours. Trajectory: Left to right. Path: It is through the skin into the arm with massive fracture of the humerus and laceration of blood vessels. Gunshot wound #7 to the left arm (fatal): Entry: Inner aspect of left arm, 10 inches below top of shoulder, 3/8 inch and atypical with abrasion, no soot or stippling. Exit: None present. Projectile: None present. Trajectory: It is right to left. Path: Skin and underlying muscle. Gunshot wounds #8 and #9 to the right thigh (nonfatal): Entry: Right thigh, outer front side, 29 inches and 30 inches above the heel, 1 inch inch each with abrasion on inferior margin, no soot or stippling. Gunshot wound #23 to the left hip (nonfatal): Entry: Back of left hip, 39 inches above the heel, inch and atypical with abrasion on medial margin, no soot or stippling. Exit: None present. Projectile: Large caliber, metal jacketed, deformed bullet recovered from left upper hip and deposited into safe at 1650 hours. Trajectory: Back to front and upward. Path: Skin, muscle and blood vessels. 76A798P Rev 2/91 COUNTY OF LOS ANGELES DEPARTMENT OF CORONEH 1 2 AUTOPSY REPORT NO 2013-04096 ZAWAHRI, JOHN SAMIR EXTERNAL EXAMINATION: The body is identified by toe tags and is that of an unembalmed, refrigerated adult male who appears to be the reported age of 23 years. The body weighs 235 pounds, measures 69 inches and is obese. There are no tattoos present. Abrasions are noted on the face and extremities. There is a contusion on the right wrist. Rigor has presumably been altered as well as livor. The head is normocephalic and is covered with black hair. Mustache is absent. Beard is absent. Examination of the eyes reveals irides that appear to be brown in color and sclerae that are white. There are no petechial hemorrhages of the conjunctivae of the lids or sclerae. The oronasal passages are unobstructed. Upper and lower teeth are present. The neck is unremarkable. There is no chest deformity. The abdomen is obese. The genitalia are those of an adult male. The penis appears circumcised. The extremities show no deformity. EVIDENCE OF THERAPEUTIC INTERVENTION: There is no evidence of any previous recent hospitalization. CLOTHING: The body was not clothed and I examined the following clothing: Black T-shirt, black pant, black underwear, all with multiple holes of varying sizes corresponding with the multiple gunshot wounds described above; black boots with holes on the right corresponding with gunshot wound to the right foot; black tactical vest. INITIAL INCISION: The body cavities are entered through the standard Y-shaped incision. 76A793P- Rev 2-?91 COUNTY OF L08 ANGELES 1 2 AUTOPSY REPORT DEPARTMENT OF CORONEH 2013-04096 ZAWAHRI, JOHN SAMIR NECK: The neck organs are removed en bloc with the tongue. There is no trauma of the gingiva, lips or oral mucosa. Both hyoid bone and larynx are intact without fracture. There is no hemorrhage in the strap muscles of the neck or around any throat organ. The tongue when sectioned shows no trauma. CAVITY: The pleural cavity contains no fluid or adhesion. The lungs are well expanded. The organs of the abdominal cavity have a normal arrangement and none are absent. There is massive blood in the abdomen. There are no adhesions. SYSTEMIC AND ORGAN REVIEW The following observations are limited to findings other than injuries, if described above. MUSCULOSKELETAL SYSTEM: There is no abnormality of the bony framework or muscles present. CARDIOVASCULAR SYSTEM: The aorta is elastic and of even caliber throughout with vessels distributed normally from it. Within the pericardial sac there is a minimal amount of serous fluid. The heart weighs 400 grams. It has a normal configuration. The valves are thin and competent. There is no lesion of the myocardium. The mucosa is intact and pale. The left lung weighs 500 grams. The right lung weighs 400 grams. The is unremarkable. GASTROINTESTINAL SYSTEM: The esophagus is intact throughout. The stomach contains a small amount of tan cloudy fluid. The mucosa is unremarkable. The small intestine and colon are unremarkable. The appendix is present. The pancreas occupies a normal position and is unremarkable. HEPATOBILIARY SYSTEM: The liver weighs 1800 grams. It is tan-brown and soft. The capsule is intact. The gallbladder is present. It contains bile and no calculi. URINARY SYSTEM: The left kidney weighs 130 grams. The right kidney weighs 120 grams. The kidneys are normally situated and are unremarkable. The urinary bladder is unremarkable. It contains 60 cc of yellow urine. GENITAL SYSTEM: The prostate is without enlargement or modularity. Both testes are in the scrotum are unremarkable and without trauma. SYSTEM: The spleen weighs 200 grams. The capsule is intact. HEAD AND CENTRAL NERVOUS SYSTEM: The external periosteum and dura mater are stripped showing hemorrhage in the right temporal occipital area of gunshot wound. There is subdural hemorrhage in the occipital area. The brain weighs 1500 grams. The leptomeninges are thin and transparent. Coronal sectioning demonstrates a uniformity of cortical gray thickness. The cerebral hemispheres are symmetrical. Anatomic landmarks are preserved. The ventricular system is unremarkable. Pons and medulla are unremarkable. The cerebellum shows contusion on the right. Vessels at the base of the brain have a normal pattern of distribution. The cerebral arteries are without arteriosclerosis. SPINAL CORD: The entire spinal cord is not dissected. HISTOLOGIC SECTIONS: Representative sections from various organs are preserved in one storage jar in 10% formalin. TOXICOLOGY: Blood, urine, bile and vitreous have been submitted to the laboratory. Investigating officer: Detective H. Ahn Agency report 13-66448 On lune 7, 2013 at 1725 hours, Senior Criminalist O. Pleitez requested additional criminalist assistance with evidence processing of a multi?decedent Coroner Special Operations Response Team (SORT) incident at Santa Monica. The team met at the Emergency Operations Center at the Forensic Science Center at 1830 hours for brie?ng. SORT arrived on scene approximately 1935 hours at the Santa Monica Command Post near 1Th Street and Pearl Street. There were a total of six decedents (2013-04094, 4095, 4096, 4098, 4099, 4100), four of which a criminalist processed at (4096, 4098, 4099, 4100) three different scenes. After processing cases 2013-04099 and 2013-04100 at a different scene, I arrived at 2300 hours to process 2013-04096. The decedent was located on the south side of Santa Monica College opposite an apparent single story family residence with an address of 1724 Pearl Street, Santa Monica. 1 observed a clothed male supine on a concrete sidewalk, oriented east to west, head to feet, respectively. His handcuffed arms were raised superior to his head. He wore black pants, black boxers, black shirt, black jacket, black tactical vest, and a black watch. There were black socks and black shoes west of the decedent?s feet. His pants and boxers were observed down at his knees, while his black shirt was observed up at his chest. While removing the vest, I observed a projectile fragment near the right side of the decedent?s head. While viewing the vest at scene, I found and collected a projectile fragment, a box of ammunition, and a single unspent round. A projectile protruded from the right collar of the jacket. I collected a separate projectile while removing the jacket and shirt from the decedent. There were black boots with defects north of the decedent and roughly halfway between the decedent and the library. Scene evidence collection was completed at 0028 hours on June 8, 2013. At 1110 hours on June 10, 2013, Santa Monica P.D. requested buccal swabs be collected at autopsy and released to them. Senior Criminalist O. Pleitez swabbed the decedent?s mouth at autopsy. At approximately 1440 hours on june 10, 2013, while viewing the clothes with the Deputy Medical Examiner (DME) O. Chinwah at the FSC, 1 saw and collected a projectile fragment from the vest and a projectile fragment from the pants. Beverly Hills Police Department Investigation: On 06/07/2013 at 1601 hrs, the Santa Monica Police reported this multiple fatality shooting incident to Lt. David Smith at the Coroner's Forensic Science Center (FSC). Assistant Chief Ed Winter assigned this field-call to me as the Leader of the Coroner's Special Operations Response Team (SORT). The following SORT members responded to the FSC at 1830 hours along with myself: Karina Peck, Fidel Fernandez, Raul Estrada, Dana Bee, County Morrow, Eucen Fu, Ruben Pena, Carlos Garcia, Oscar Pleitez, and Liz Napoles, I briefed the SORT personnel in the Coroner?s Emergency Operations Center (E00) and we departed for the scene at 1909 hours. We arrived at the Santa Monica Command Post on the coroner of 18th Street and Cedar Avenue and met with Assistant Chief Winter and Captain Brian Elias. Our multiple site scene investigation began at the residence (Yorkshire Avenue) location at 2000 hours. Investigation at the Santa Monica College and nearby vehicle death sites began at 2140 hours. The four decedents were examined then transported to the FSC by Pena and Garcia. The SORT members departed from the scene at 0045 hours on 618/201 3. Location: Santa Monica College. 1900 W. Pico Blvd, Santa Monica, CA 90405 InformantIWitness Statements: I spoke with Detective Spencer at the scene and he gave me the following information. Just prior to 1155 hrs, a shooting and subsequent residential ?re occurred at 2036 Yorkshire Avenue in Santa Monica. The decedent?s father and older brother (CC 2013-04099 2013-04100) were found dead with gunshot wounds in this house. Their bodies had not been burned. The decedent was the suspect in that incident and later was seen by neighbors ?dressed in black and heavily armed with guns and ammunition.? He then went on a shooting rampage through the streets of Santa Monica resulting in multiple victims at multiple locations. He shot at a city bus injuring a female who was transported to a nearby hospital and survived her injuries; he fired at various vehicles before nearing the Santa Monica College campus, where he continued to indiscriminately shoot at persons and vehicles. At approximately 1200 noon, the decedent was seen firing shots from an AR-15 ri?e at a man and woman driving a red 1999 Ford Explorer SUV The male was struck in the head and pronounced dead at the scene (CC 2013-04098). The female was transported to UCLA Medical Center, also with a gunshot wound to the head, and is currently on a ventilator in serious condition. The decedent then continued on foot towards the campus library, where he shot a Hispanic female (thought to be a homeless woman) in the abdomen; she was transported to UCLA Medical Center and died of her injuries (CC 2013-04094). Ultimately. the decedent was engaged by police officers and killed in an officer involved shooting. He sustained multiple gunshot wounds to his head, torso and legs. Once the decedent was shot, he collapsed onto the ?oor and Santa Monica Campus Police Officers placed handcuffs around his wrists. The decedent?s body was then dragged out of the library and placed on the sidewalk. He was pronounced dead at the scene at approximately 1230 hours. At this time, no other suspects are believed to be involved in this incident. Page 1 of 3 County of Los Angeles, Department of Coroner Investigator?s Narrative Case Number: 2013-04096 Decedent: ZAWAHRI, JOHN SAMIR Scene Description: I observed the decedent?s body lying supine on the ground in front of the Santa Monica College school sign on the south side of the campus. The decedent was dressed in black clothes and his hands were handcuffed above his head. A black nylon vest had been pulled up over his head and covered his wrists and hands. There was dried blood on the sidewalk beneath the decedent?s body. His head was turned to the right and his eyes were closed. Dried blood was seen on his face and arms. His body lay approximately 100 yards from the entrance to the school?s library. Approximately 50 yards east of the decedent's body was the red Ford Explorer that had come to rest against a brick wall with that male victim (case #201 13-04098) slumped fonNard in the driver's seat. The decedent's body was found several blocks from the Yorkshire Avenue residence which contained the decedent's father's and brother?s bodies (see case #2013?04099 for that scene description). Evidence: Criminalist Eucen Fu collected hair exemplars, a GSR Kit, the decedent?s clothing, handcuffs and multiple bullets and fragments at the scene. The Santa Monica Police Department collected numerous weapons from the scene (serial numbers not obtained). Please see evidence log for details. Body Examination: The decedent is an adult Middle Eastern male with brown hair, brown eyes and natural teeth. He was wearing a black t?shirt with written in white across the chest area, black tactical pants, black underwear and black socks. A pair of black tennis shoes was nearby the decedent but appeared too small for his feet and had no holes in the shoes. Larger black boots were found near the entrance to the library and appear to have gunshot holes in them. A black nylon tactical vest was pulled up over the decedent?s head and entangled with his wrists and hands. Handcuffs were looked around both wrists. Rigor was overall and did not easily resolve. Lividity was a light pink in the dependent areas, consistent with a supine position and blanched with firm pressure. The ambient temperature at 2350 hours was 64.4 degrees and his liver temperature was 94.7 degrees at 2353 hours. The decedent had many apparent gunshot wounds. There were two to the right side of his head behind his ear and the right side of his neck. His right thumb had been amputated. A wound was seen in his right shoulder along with abrasions in his armpit. There was an abrasion to his right abdomen and left lower abdomen. There were wounds on his right hip and approximately 9 various wounds to his right lower leg. This right lower leg was fractured. There were also wounds to his right ankle and right foot. On the posterior side of the body, there were two wounds to his left lower lateral backside and left buttocks. There were additional wounds to his left posterior upper leg. A wound was seen on his right lower back and upper buttocks as well as his right lower leg along with and abrasion to back side of this knee. Identi?cation: The decedent was identified as John Samir Zawahri by visual comparison to CDL #_at the scene. The decedent?s CDL was found at the residence on Yorkshire Avenue.
