PAYMENT OF CIVIL PENALTY

Mr. Kuchar:

Based on an inspection conducted on May 7, 2018, it has been determined that you have complied with
some Of the requirements of Citation No. 654. However, in order for this Office to consider this Citation
fully compliant, the Civil Penalty assessed by letter (cOpy enclosed) dated December 22, 2017 must be
received. Payment of Civil Penalty should be by check or money order made payable to the Of?ce of
Conservation. Numbered invoices are included with this notice and is to be returned with the remittance.

Failure to immediately pay the Civil Penalties will result in the imposition of additional regulatory
sanctions as deemed necessary by this Of?ce.

Regarding the MAOP violations discovered at the inSpection, you are hereby ordered to update this of?ce
by letter every 6 months with the status of the MAOP resolution. This citation will remain open until an
MAOP is accepted.
