A CPD officer requested my assistance with traffic control at the corner of 28th St. & Michigan Ave. I remained on traffic control throughout the incident. No NARRATIVE *IN SUMMARY* The purpose of this report is to document my assistance with the Chicago Police Department during the active shooter critical incident at Mercy Hospital Chicago. On November 19th, 2018, at approximately 3:30pm, I, Tpr. S. Swanson #6499, was on my regular patrol duties and had just left court at the Daley Center in downtown Chicago. I heard over the radio about the active shooter incident occurring at Mercy Hospital, and checked its location. Realizing I was close-by, I informed my dispatch I would be proceeding code 2 to the hospital. While enroute, our dispatcher advised Chicago Police was stating the initial shooter was down. Tpr. M. Soto #6728 and I were the first Troopers on scene. We grabbed our AR-15 long rifles, and ran into the hospital. We entered the hospital at the first entrance we saw, which was just south of the main emergency room entrance, and the location where one of the victims had been shot inside an elevator. There were scores of Chicago Police Officers on scene standing by various doorways with their attention focused further into the hospital. Tpr. Soto informed our dispatch that according to Chicago Police, there may be multiple shooters involved. I spoke to a few Chicago Police supervisors on scene, who said the hospital was not secure, and their SWAT team was in the process of clearing various hallways and rooms. I moved towards the front of the group of Chicago Police Officers and located one of their SWAT teams, designated by their specific uniforms. Trooper Soto and I, with CPD officers, helped provide cover for the SWAT team while they cleared various rooms and hallways. At one point Tpr. Soto and I, along with a few CPD officers cleared a few rooms ourselves. We made our way further into the hospital, coming across the body of the shooter, which was lying face down, handcuffed, with a gunshot wound to the head along with a large puddle of coagulated blood around his head. There was also a semi-automatic pistol next to the body, with an empty magazine and a full magazine alongside it. Soon, we were joined inside the hospital by other responding Troopers, namely: Tpr. E. Gonzalez #6578, Tpr. D. Avila #6564, and Tpr. M. Vodicka #6235. We all continued to provide assistance to the Chicago Police SWAT team until they determined the wing of the hospital we were in was clear. Their SWAT team met up with another SWAT team, and they began to discuss a plan for how to proceed clearing the rest of the hospital. At this point Illinois State Police Sgt. V. Harris #4779 requested over the radio for all Troopers in the hospital to meet outside the emergency room entrance. Once all Troopers were clear of the hospital and accounted for, we were instructed to rendezvous at 25th and Prairie Street. Once there all Troopers who responded to the incident awaited further instruction from Lt. H. Spight Page 1 of 2 #4887, who was on scene. A determination was made for all Troopers present to pair up and post at various intersections around Mercy Hospital, assisting CPD with cordoning off the area. Eventually, I was given the instruction that no further assistance was needed from the Illinois State Police at that time, and to return to my regular patrol duties. On 11/19/2018 at approximately 03:31pm District Chicago Radio Communications advised Illinois State Police District Chicago that Chicago Police Department is responding to an active shooter situation. I placed myself en route from I-57 Northbound and Halsted Street. While responding to the scene I learned that the threat has been neutralized, an officer was down, and that there is a potential second shooter inside the hospital. Responding Troopers whom have arrived, entered the hospital in an effort to locate/isolate the potential second threat as well as account for the safety/wellbeing of others. Upon my arrival, I have learned that responding Troopers have been told to vacate the building as Chicago Police Department SWAT personnel were entering the hospital. I gathered with fellow Responding Troopers and took an assignment from ISP Supervisors ( Lt. H. Spight, M/Sgt. B. Epps, M/Sgt. S. Orta, & M/Sgt. V. Harris) to guard the outerperimeter of the hospital until the situation was deemed safe. I assisted in lane closure for the procession of the slain CPD officer and continued with the procession after my assignment has been complete. While responding to the scene I heard via radio broadcast that a police officer had been shot, there were multiple victims and that there was possibly a second shooter. A/Msgt. V. Michigan Ave Chicago, Illinois to assist Chicago Police Department (CPD). Reference Illinois State Police Field Report #F03-18-4219. On 11/19/18 I, Tpr. M. Soto #6728 was on my way to my patrol area from court on 50 W Washington when Illinois State Police Radio at approximately 3:31 pm dispatched to the district of an active shooter at Mercy Hospital. I put myself Code 2 to assist CPD at Mercy from Lake and Dearborn. I arrived on scene at approximately 3:36 pm and parked my squad car at 26th and Michigan Ave. I proceeded to the truck of my squad car to retrieve my Armalite m15 rifle and proceeded towards Mercy going on foot eastbound on 26th Street. Once I arrived at the Outpatient entrance, I saw numerous CPD officers escorting a number of people to safety. I was advised at the time by a CPD officer that there might be another shooter. I advised radio of the possibility of another shooter and at approximately 3:41 pm I made my way inside the building through the Outpatient entrance. Shortly after I made my way inside, Tpr. S. Swanson #6499 had made his way inside and we parented together to see what had to be cleared. Once inside I saw people still being escorted to safety by CPD and in the middle of the floor entrance was blood. Tpr. Swanson and myself had our ways down a hall where numerous CPD officers where. As we approached two doors to our right a CPD officer advised us the . At that time I positioned myself to the right of the door as we were waiting to get a key to open the door. As we were waiting Tpr. D. Avila #6564 and Tpr. M. Vodicka #6235 made their way to our location. I advised both of them the rooms have not been cleared and we Once we received the key to open the door we signaled to each other that we were ready to enter by . The order we entered the room was . As we were trained during an active shooter situation Once we cleared the room we made our way down another hall were we saw the offender on the ground handcuffed and neutralized. The hall where the offender was neutralized I saw a number of spent casings on the floor, a black handgun, and an driver’s license. We did not go any further as we were advised by Illinois State Police Command for all Troopers inside the building to meet at the emergency entrance for assignment. We meant with M/Sgt. B. Epps #5679 and AM/Sgt. V. Harris #4779 for assignment. They both advised all the Troopers on scene to pair up and post on every corner of the hospital. Tpr. Antczak #6660, was investigating a property-damage crash relocated to 130th Street eastbound just east of Stony Island Avenue when Illinois State Police Communications broadcasted a message for an active shooter situation at Mercy Hospital at 2525 S. Michigan Avenue, Chicago, Illinois 60616. After clearing from the crash (Crash Report #03-18-18416), I enacted my marked Illinois State Police squad car’s emergency sirens and lights and notified Communications that I would be en-route to the scene. Upon my arrival, I positioned my squad car on 25th Street eastbound at Prairie Ave. I exited my vehicle at this location. I retrieved my Armalite M15A1 rifle (serial # from my squad car’s trunk and maintained my rifle in a slung position, with rifle hanging in sling in front of my person. Illinois State Police Command assigned me to secure the outer perimeter, and to post at the intersection of 26th Street and Prairie Avenue. I was partnered with Trooper A. Armando #6611. During my assignment, I observed Chicago Police Department (CPD) units establish an outer perimeter at the intersection of 26th Street and Prairie Avenue. CPD units utilized barrier tape and CPD squad cars to control entry points. Pedestrian and civilian vehicular traffic was redirected away from the scene. At approximately 5:44 p.m., Illinois State Police Command advised me to clear from the scene. I then resumed my normal patrol duties. I was not assigned to assist with the procession. At approximately 3:31pm District Chicago Communications advised the district of an active shooter at Mercy Hospital. I informed Chicago communications that I was en route (code 2) to Mercy Hospital. Upon arrival at the hospital, I parked my squad car on the north east end (25th and Prairie). I exited my squad car and approached the nearest CPD officer at the same location and asked him how many shooters were in the hospital, if he knew the count for victims, and if he knew the last known location of the shooter. The CPD officer informed me that they believe that there were one or two shooters, that one CPD officer was shot, and that there were multiple victims in the basement. I returned to my squad car to call Desk Operations and relay the preliminary information I was able to obtain. Once I finished speaking with Desk Operations, I grabbed my issued rifle and at approximately 3:37pm entered the hospital through the east entrance. Once inside the hospital, I attached myself with a Chicago SWAT/SRT team that was about to start clearing the east side of the first floor. While clearing the east end of the hospital we helped a few people that had barricaded themselves inside, evacuate the building. While we were clearing the west end, I noticed the shooter suspect (later identified as Juan Lopez M/H 12/31/85) face down on the hospital floor, handcuffed and what appeared to be blood on the floor next to his head. I also noticed a few pistol and 5.56/.223 brass casings on the floor, and a Glock handgun that was unloaded with its slide locked to the rear next to two loaded magazines. At this time MSGT. B Epps #5679 requested for all ISP units to meet up at the emergency room entrance. From there we helped CPD cordon off the area until we were no longer needed. While enroute to my patrol assignment, I (Trooper M. Vodicka #6235), was traveling on I-57N/B near 127th street when I learned of an active shooter situation at Mercy Hospital. I advised radio I was enroute code 2 to assist CPD with the active shooter. While enroute, I learned the shooter was neutralized but it was unconfirmed if there was a second shooter. I arrived at the intersection of 26th and Michigan Avenue and positioned my squad facing N/B on Michigan at 26th street; (multiple emergency vehicles were in the vicinity). I quickly loaded my rifle, paired up with Trooper D. Avila #6564, and made entry into the hospital at approximately 3:47pm. Myself and Avila entered the public entranceway closer to Michigan Avenue and observed a pool of blood and shell casings near the elevator/lobby area. We continued down the hallway and teamed up with Troopers Soto #6728, and Swanson #6499. We coordinated with CPD officers and cleared 2 rooms. We continued down another hallway and observed the shooter in custody (deceased) on the floor. At this point, Sergeant V. Harris #4779 informed all ISP units inside the hospital to meet in person outside at the front entrance of the emergency room. Next, we were advised to meet at 25th and Prairie Avenue. The director spoke to a group of ISP Troopers regarding the situation and health status on a CPD officer that was shot. S.Orta #5228) was leaving ISP District Chicago Headquarters when R/O notified through the Illinois State Police south radio channel of a active shooter incident at Mercy Hospital. R/O immediately responded to the scene (code 2 response) from Desplaines, IL. While enroute R/O monitored Chicago Police Department Citywide radio and learned that several people had been shot including the alleged gunman. Upon arrival at Mercy Hospital R/O was met by other ISP officers who were already on scene. R/O at this time learned that the shooter had been neutralized. R/O was advised by Acting Lieutenant Spight #4887 that all ISP officer's on scene would rendezvous at the intersection of 25th and Prairie. Shortly after, R/O met with A/Lt. At approximately 3:33 p.m., District Chicago radio advised that the shooter had been neutralized at which time I returned to stationary patrol at the I-94 southbound 75th Street accident investigation site. At approximately 3:41 p.m., Trooper Soto #6728, who was on scene of the active shooter at Mercy Hospital, stated that Chicago Police advised of a possible second shooter. I grabbed my AR-15 long rifle and placed it on my passenger seat and responded code 2 to Mercy Hospital. Harris #4779 over the radio instruct units to secure the outer perimeter at which time I responded to 25th and State to divert all traffic onto northbound I55 and away from the vicinity of Mercy Hospital. I later assisted Chicago Police with the funeral procession. Based on the totality of the events that unfolded, an immediate intervention and Rapid Deployment response was required in an effort to neutralize the threat and to mitigate further harm or death. It was later learned that a domestic dispute was the impetus for the subsequent aggression which resulted in four deaths. On 11/19/18 at approximately 3:31 pm, I (A/M.Sgt. V.L. Harris #4779) responded to Mercy Hospital to assist with an Active Shooter incident. Prior to arrival, I monitored CPD Zone radio traffic in an effort to gather intelligence to disseminate to responding Troopers. After arriving on scene, I was deliberate in preparing for entry into the building after learning there was a possible second shooter. As I approached the emergency entrance, I quickly noticed that the scene appeared to be chaotic and the information was rapidly evolving. At this time, I became actively involved in developing an actionable plan, with M/Sgt. B. Epps, for the safest course of action for all ISP Troopers on scene. Further information gleaned revealed that the CPD SWAT Teams were sweeping the building from top to bottom. Understanding that our involvement was ancillary, it was at this time that we decided to extract our Officer's from the building. All ISP Troopers on scene were then given outer perimeter assignments until the scene was deemed safe. Immediately following our departure, all supervisor's (M.Sgt. Epps, M.Sgt. Orta, A/M.Sgt. V.L. Harris) on scene were tasked with developing the logistics for the procession of the slain CPD Officer. On 11/19/18 at approximately 3:22 PM, I Trooper Armando #6611, arrived on scene to a traffic crash with personal injury which was relocated to the Polk Street Accident Investigation Site (See ISP Crash Report 03-18-18419). At approximately 3:31 PM an active shooter incident at Mercy Hospital was announced by Illinois State Police Telecommunications. Within minutes ISP TC stated that the suspect had been neutralized. At approximately 3:41 PM it was reported that there may be a second shooter but this was not confirmed. At approximately 3:52 PM the party that requested an ambulance refused medical transport and the crash was changed to property damage only. At approximately 3:57 PM I cleared myself from the crash and put myself en route to the active shooter incident at Mercy Hospital. ISP personal were directed to gather at 25th Street and Prairie Avenue. I arrived at approximately 4:10 PM and relayed this to ISP TC, but I was not marked as arrived until approximately 4:14 PM, after ISP TC conducted a status check. At approximately 4:25 PM Trooper Antczak #6660 and myself were partnered and directed to 26th Street and Prairie Avenue. We posted at that location, assisted with traffic control, and provided area security with our department issued rifles. During this time I made no significate observations. At approximately 5:44 PM Trooper Antczak cleared from the incident and I relocated to 25th Street and Prairie Ave for traffic control. During this time I made no significate observations. At approximately 6:02 PM I cleared myself from the incident and resumed regular patrol. At approximately 7:50 PM I was directed to Garfield Boulevard and Wentworth Avenue to advise when the CPD escort procession began and ended. When the last CPD vehicle passed I entered the procession as the last police vehicle. From this position I was able to assisted IDOT trucks to ensure no non-emergency vehicles entered or passed the procession. Additionally, we provided space for emergency vehicles to enter the procession. At approximately 10:02 PM, when I reached Damen Avenue and Harrison Street, I cleared myself from the procession incident and proceeded home.
