Inspector Observations:
A reinspection following the suspension/revocation hearing held on 12/5/18 was attempted by K. Burnham and D. Garcia.
During the reinspection several live adult cockroaches were observed on and around the ice maker. A dead juvenile cockroach
was observed inside the ice maker. Multiple dead adult cockroaches were observed in the floorsink drain located beneath the
ice maker.
The bread making equipment including the mixer, dough press and dough roller were observed to still be in place. The owner
stated he has decided to keep it and resume using it. The interior motor housing of the dough press was found to have heavy
accumulation of rodent feces and urine inside stemming from a prior rodent infestation. The interior of this unit was sprayed
with bleach and cleaned during the attempted reinspection. Continue detail cleaning this equipment. If the items are found to
be unclean or unused at future inspections their removal will be required.
The facility permit is hereby revoked due to violation of the probation terms and conditions, namely the persistence of the
cockroach infestation. A closure sign was posted. The owner was directed to discard all ice in the ice maker and sanitize the
interior.
Contact inspector when the cockroach infestation has been fully eradicated. A new permit may be purchased at that time.
