On 0912112018, at approximately 1837 hours, I made contact with Rosa Flores (DOBI)

Youngtown, AZ 85363.
Rosa advised me she needed an informational report for an incident involving a child at the Southwest Key Program. Rosa
requested I watch video surveillance of the incident and document my observations in a report. Rosa also stated she
would not be able to provide me with a copy of the surveillance footage to impound as evidence.

At approximately 1953 hours, I arrived at the Southwest Key Program and made contact with security personnel. The

security personnel advised me the incident occurred earlier in the day at approximately 1612 hours in Classroorn D. I was
advised the subject, Mauro Lopez, was escorted off of the property after the incident and is no longer working for the
Southwest Key Program. The security personnel added the children in the classroom were watching a movie and Mauro
was trying to open the door for children returning from a bathroom break. I then proceeded to watch the video and
observed the following:
