nun/21:19 a 0539 Entry ID: 1:117 nun/21:19 a 1005 Modified ID: 1:117 nun/21:19 a 2349 Approval ID: 1:94 On January 4, 2019 at approximately 0516 the Reynoldsburg of Police (RPD) was contacted by the Franklin County Coroner's office and asked 1f a report had been made on an at LBrands/ Bath and Body Works, located at 7 Limtted Parkway There had been a medica1 rim reported, but Police were not requested nie later identified as Harvey Beavers, was lransported to Mount Carmel East (MCE) Hospital where he was pronounced deceased 1 made contact with LBrands onrduty Security Head/ Safely Specialist, Andrew Jaquter (Al) Al reported he was contacted by radio at 0149 that EMS was being requested at DC7 for an associate who'd been pinned by a conveyor He immediate1y responded to the scene Upon his amva] (Door 40 on the 1oading dock), two of his Asset Prolection Officers were CPR w'tth an AED connected to Beavers Jefferson TWP FD medics arrived on scene at 0201 and took over CPR Beavers was transported lo MCE by JTFD medics AJ states that afier Beavers was transported, he obtained written statements from associates that were present (copies of associates statements were provtded for this report) I requested to speak with the supervtsor for Beavers was taken to the loading dock and introduced to-- - states that he was notified by one ofhis Lead Assoctates that Beavers was ptnned in a Iratler 1n door 40 When _arrived, Beavers was on the floor and appeared blue and associates stated he wasn't breathing .ran back to his office and contacted the emergency line and was advised they'd already been made aware oflhe incident _as advtsed EMS was already enroute He then had an associate retract the conveyor, lo provtde more room for EMS personnel, and had door 41 opened and cleared, to allow EMS personne1 easier access to the scene Based on interviews and statements obtained, Harvey Beavers was working tn the loading facility of DC7 at LBrands, Bath Body Works, located at 7 Limtted Pkwy Beavers was teamed with -- at Dock #40 Both are Outboard 1oaders Beavers was responsible for loading/ stacking boxes in the Iratler at Dock 40 was his partner and responsible for placing packages on the conveyor into the trailer At approximately 0140 _heard 34 loud bangs His immediate thought was packages talting from the conveyor He checks the are but nothing has fallen He then noticed a red 1ight above his conveyor, signifying a malrunction/ stoppage - then looked the trailer and observed Beavers ptnned against the wall ofboxes stacked 1nside the Iratler _ye11ed for help According to- Beavers was shaking Beavers was facing the wall of boxes and the conveyor was pressing him in the lower back and against the wall of boxes The end that was pressed against Beavers back contains all the controls for movtng the conveyor 1n and out of the tratler With those controls is also the emergency shut offand the dial to retract the conveyor The conveyor was so pressed against Beavers lower back, associates were unable to gain access to move the conveyor back _and the associates that responded to alen had to tear down the wall ofboxes around Beavers to get his free When Beavers was freed, he was laid on the ground and reported had a very weak or no pulse Security personne1 responded and immediately started CPR and applied the AED, until JTFD arrived and took over CPR MECC advised that Police were not required on the scene, it was deemed a medical sttuatton Beavers was then transported to MCE Reynoldsburg Police dispatch was contacted at 0516 by the Franklin County Coroners omce They asked it RPD had taken a report orthe incident at LBrands because the victim was deceased A report was taken and OSHA was notified ofthe incident 1 received a can back from OSHA and advised that they had since received a call from LBrands reporting the incident OSHA was provided the report number and officer name associated with the report City of Reynoldsburg Division of Police PERSONNEL NARRATIVE FOR OFFICER JASON AAMODT Ref: 19REY-422-OF Entered: 01/04/2019 @ 0839 Modified: 01/04/2019 @ 1005 Approved: 01/04/2019 @ 2349 Page: 2 Entry ID: P117 Modified ID: P117 Approval ID: P94 Photographs were taken of the scene.
