I am writing on behalf of my constituent, Master Sergeant Jay Ellis, a member of the 1 15th
Fighter Wing at Truax Field in Wisconsin.

Please ?nd attached Ellis? completed Privacy Act Release form. As you will see, 
raises several very serious allegations dating back decades. In the Form attached Ellis
gives several examples of troubling behavior he has witnessed and has been told about,
including, but not limited to inappropriate comments, to actual sexual assault. I am asking that
you look into this matter with all seriousness and thoroughness and provide my of?ce with a
written response detailing what investigation you have undertaken and what actions you are
taking/will take. Ellis is available to give more details if needed, including putting the Air
Force into contact with some of the victims of this behavior. Please direct a response to my
of?ce.
