A complaint was recently filed with the lowa Department of Human Services indicating that
you care for more then 5 chiidren atone time. As an un~regletered provider you may not-
care for more than 5 children at one time. i have made several attempts to visit your home to
discuss this with you and have had, no success. i stopped once in the morning, afternoon and
late afternoon on 3/8, 3/9 and 3/11. i am sending this letter is a reminder.

lowa Code Section 237A.19 states, ?a person who ee?teblishee, conducts, manages or
operates a child development home without commits a simple mi?edemeenori
Each day of continuing violation, is a separate offense. A single charge alleging continuing
i violation may be made in lieu of filing charges for each day of vioietion.?
