The Department's decision to revoke your licenses is based upon failure to comply withthe Human Services Code and Department regulations (62 PS. 1026(b)(l); 55 Pa. Code 55 Pa. Code Chapter 3800); gross incompetence, negligence and misconduct inoperatingthe facilities (62 PS. 1026(b)(4); 55 Pa. Code and mistreatment and abuse of children in care at the facilities (62 PS. 1026(b)(5); 55 Pa. Code The Department?s actions are based on the following violations: 3800.], Purpose of Chapter 3800; 3800.15, Child Abuse; Speci?c Rights; 3800.33, Deprivation of Rights; 3800.53(b) Director; 3800.55(a) Child Care Worker; 3800.148(b) Health and Behavioral Health Services; Use of Restrictive Procedures; and 3800.211Cb) Manual Restraints. The violations speci?ed above and attached to this revocation notice demonstrate a failure to comply with the Human Services Code and Department regulations, gross incompetence, neglect and misconduct in operating a facility, and mistreatment and abuse of children in care.
