Met with the manager this date and reviewed the complaint. Documentation was provided showing a pest control company serviced this facility on 4/18/19 and according to the manager the next service is due. Observed numerous live cockroaches in various life stages (juvenile to adult) under the dishwash counter running along the wall and coming from the inactive Powerwash equipment box/gears. Observed some dead cockroaches on the chemical sanitizer bucket and on the wall below the dishmachine. A dead cockroach was observed on the wall behind cooking equipment. These areas were pointed out to the manager during this complaint investigation. A heavy accumulation of grime/residue/encrustations was observed under the dish area counters, walls, floors, and floor drains. Observed a heavy accumulation of grime/grease/encrustations on the walls behind the cooking equipment, on cooking equipment, floors in the dry storage room, and inside the cabinets of the customer area buffet counters. Take immediate action to clean and sanitize all affected areas from the cockroach activity. Properly clean/sanitize the noted areas of heavy grime/grease/residue/encrustations to prevent cockroach harborage. Obtain an immediate pest control service to eliminate all cockroach activity. The manager voluntarily closed the business at this time and customers were no longer permitted to enter. The "A" placard was removed and a closure sign was posted. This facility must remain closed and may not re-open without written approval from this Agency. The facility must provide documentation showing a pest control company has serviced this facility from this date. All noted areas must be properly cleaned and sanitized to prevent cockroach activity.
