Attempted a routine inspection. Unable to complete due to imminent health hazard.
FACILITY CLOSED DUE: COCKROACH INFESTATION / RODENT ACTIVITY
A facility shall be kept free of vermin.
1) Observed numerous live cockroaches at various life cycles at the following locations:
- Inside an out-of-service black fan under the 3-door freezer
- On numerous out-of-service pizza screens in between the 3-door freezer and pizza prep table
- Under the 3-door freezer
- Inside the deteriorating FRP wall panel under the 3-door freezer
2) Observed rodent droppings in the small back dry storage room, under the storage racks near pizza boxes.
FACILITY CLOSED
Corrective action: Obtain approved pest control, remove all evidence of infestation, clean & sanitize all effected areas, seal all
cracks crevices and or harborage points.
Harborage:
- Repair/ Replace back door to facility to ensure door is self closing, and shuts in an approved manner. The door was not able
to close in an approved manner
- Seal the FRP (i.e. wall panel) in back of the 3-door upright cooler
- Remove the following: the out of service black fan, the pizza screens, and all out of service equipment within the facility
CLOSED SIGN POSTED
A re-inspection has been scheduled for 5/23/19. Do not reopen facility until approved by a member of this department. If all
violations have been corrected prior to re-inspection date you make contact this office at 951-461-0284 to schedule a
re-inspection. Opening facility prior to approval by a member of this department may result in a citation, office hearing, or other
legal action
