PUBLIC NOTICE FOR INVOLUNTARY TERMINATION OF
MEDICARE /MEDICAID PROVIDER AGREEMENT

Notice is hereby given that effective July 3, 2019, the agreement between Mount Sinai
Medical Center, 4300 Alton Road, Miami Beach, Florida 33140, and the Secretary of Health
and Human Services, as provider of Hospital Services in the Health Insurance for the Aged
and Disabled Program (Medicare) is to be terminated. Mount Sinai Medical Center does not
meet the following conditions of participation:
42 CFR 482.12 Governing Body
42 CFR 482.13 Patient Rights
42 CFR 482.21 QAPI
The Medicare program will not make payment for hospital services to patients who are
admitted on or after July 3, 2019. For patients admitted prior to July 3, 2019, payment
may continue to be made for up to 30 days of services furnished on or after July 3, 2019.
