Kang Zhang, Robert Weinreb, Napoleone Ferrara, and Michael Goldbaum.”
Please find enclosed the responsive disclosure forms for Drs. Weinreb and Goldbaum. Faculty that are
part of the Health Sciences Compensation Plan are not required to file APM-025 forms; they are instead
subject to the requirements of APM-671. Since these forms are otherwise equivalent we are interpreting
your request as seeking the most recent APM-025 and/or APM-671 form for the specified faculty.
Our search did not identify any APM-025 or APM-671 forms on record for Dr. Zhang.
The form for Dr. Ferrara is anticipated within the next two weeks and will be produced to you as soon as
