If the permit holder wishes to have the permit reinstated, they must schedule an office conference with the assigned inspector and a program supervisor or lead. The permit holder must provide or revise a written food safety risk control plan at or before the scheduled office conference. All fees owed to BFHD must be paid before reinstatement of the permit can occur. Second Suspension When a food establishment permit is suspended a second time, the food establishment must close immediately. If the permit holder wishes to have the permit reinstated, they must schedule an office conference with the assigned inspector, a program supervisor or lead, and program director. The Benton-Franklin Health District Health Officer, or their representative, shall make a final decision at this meeting to either re-instate the permit, sustain suspension, or permanenetly revoke the food establishment operating permit. Appeals Any person who is aggreived by any action or decision made by the Health District may request an appeals hearing before the Health Officer.
