Attached is a copy of the Complaint Review report for review. Reviewing the Supplemental Report from Cameron Parish Sheriff?s Of?ce Complaint Number 14-2024 the following details were noted. Deputies were dispatched to South Cameron Memorial Hospital to meet with Jennifer M. Broussard. Jennifer is an employee of the hospital and is the bookkeeper. Jennifer stated that approximately 2 years ago a company was contracted to manage the hospital. The company is called Frontier Hospitals. James Stanger is the CEO of Frontier Hospitals and the company is out of Florida. Jennifer wrote out a check (#1153) Payable to Frontier Hospitals in the amount of $130,000 on the line where the written portion should have read ?One Hundred Thirty?. Upon realizing her mistake she voided the check and made a copy of the check for her files and put the voided check back into her check book register. On the check was written in pencil. This is her common practice. She will attached the check by either tape or paper clip. The check book register also is a carbon copy book. Jennifer stated before she wrote out the replacement to the voided check she went back an reviewed the minutes of the South Cameron Hospital Board meeting to verify the amount of the check. Then she learned that the check should have been for $110,000.00 Jennifer then made out the correct check and it was then deposited into Frontier Hospital Account at lberia Bank in Grand Lake, LA. On Thursday August 21, 2013 Jennifer received a telephone call from the auditor for South Cameron Memorial Hospital, Joey Breads. Breaux stated that according to Quickbooks the hospital had paid Frontier Hospitals two (2) payments in August 2013. Jennifer stated that she went to her check book and found that the voided check it 1 153 was missing. Jennifer then called lberia bank. lberia bank is the bank the hospital uses. Jennifer asked if check all 1153 had posted and they stated yes and faxed a copy of the cancelled check to Jennifer. The far copy is of the front and back of the check. Jennifer stated that she also pulled the copy of the voided check from her frle. On the faxed copy from Iberia Bank you could see where someone had written in the word ?Thousand" behind where the voided check said ?One Hundred Thirty?. it was clearly evident from the ?le copy that the check had been altered.
