Inspector Observations:
This food facility is closed due to observed live cockroach activity. Inspector observed approximately 4 live cockroaches under
hand washing sink and 1 live cockroach on wall behind cold table. Several dead cockroaches found on floor behind reach-in
refrigerator, behind deep fryer, and behind entry door to kitchen. Operator was able to provide last pest control report dated
7-27-2019 from Terminix with activity found on that date as well. Operator is to contact pest control company for immediate
service. Once all live cockroach activity has been eliminated and openings in wall behind hand sink and cooking equipment are
sealed, a reinspection to reopen facility can be conducted. Discontinue all food preparation and sales of food to public. "Facility
Closed" sign posted on front door (do not remove) and "A" grade removed. Contact our office at 951-273-9140 when ready for
reinspection.
