I am writing to malte certain you know that the Honorabze Anita E. Brody, the judge presiding over
the NFL Head Concussion Case, has reappointed me sole Lead Counsel to oversee the administration of
claims- it my honor and privilege to continue to serve every one of you. My lawI firm. Seeger Weiss,
has been involved in every phase of the case from the beginning, litigating the ca se. negotiating the
terms of the Settlement, and now overseeing the implementation of the terms of the Settlement. We are
now busy worlting out issu es th at arise to be sure that you receive the benefzts to which you are entitled
under the terms of the Settlement Agreement.

For an update, I am pleased to report that in the first two and half years of the oEryear term of the
Settlement, more than militon in Monetary Awards have been approved. Furthermore, over 1100i]
BAP appointments have been scheduled and over ssoo have been conducted to date. ft is my duty and
that of my law ?rm to see that this proceeds at this vigorous pace.

I do, however, lwant to talte this opportunity to alert you about some essentia': aspects of the
application subm?ssion process that wit: have an impact on your receiving an award if you are eligibte.
AH the information you provide in your claim package must be absolutely comptete and accurate. This
includes the Claim Form, medical records, third-party affidavits, and ?nformation you provide to medical
professionals- Remember that many ofthe awards are very, very substantial and you must assume that
the NFL is doing its own research to checlt the validity of the information you submit. including any
statements you malte to your Diagnosing Physician about your claity activrties. such as driving or
employment. To avoid delay in the processing of your claim as well as possible denta: of your claim.
