InSpector Observations:

 

A routine inspection was attempted this date. Upon arrival, live german cockroaches (too numerous to count) of varying life
stages were Observed throughout the facility (prep line, cooks line, warewash area, dry storage, janitorial area, etc.) as well as
extensive numbers Of dead german cockroaches on the floor as well as on and in equipment throughout the facility. Extensive
buildup and Old food debris were Observed on the flooring, shelving. and equipment throughout the facility. Gaps and crevices
were observed along equipment (Le. hood) and along the base coving throughout the facility. Ensure pest control services the
facility. All evidence Of vermin activity must be removed and the affected areas thoroughly washed, rinsed, and sanitized. Once
the infestation has been eradicated and all areas cleaned, contact this department at (951)273?9140 to schedule a
reiOSpection.

The Department ?Facility Closed" sign was posted. Ensure the sign remains posted and the facility remains closed until
a representative Of this department verifies the facility meets health standards and can be reopened.
