COWAN, who informed me that a in custody inmate had just died. Technician COWAN told me that Bryan STREICHER (PFN: had been housed in Housing Unit 35. About 2205 hours, Deputy SLAUGHTER informed a Man?down? and requested a nurse to respond. Shortly thereafter there was a request for a ?code-3? ambulance. Alameda County Fire and Paramedics Plus responded. STEICHER was moved to the ambulance (which was parked in the medium yard), where death was determined at 2244 hours. Technician COWAN told me that they are not treating this case as suspicious and ETS would not be responding to conduct an investigation. Due to the ambulance being parked inside the medium yard at the jail, and security concerns I told COWAN the ambulance could be moved to the employee parking lot. About 2345 hours, I received a telephone call from Lieutenant R. CARTER, who told me that the ambulance was in the employee parking lot and he was in the process of collecting copies of medical records and property. And they did not ask the name of the child but they did get-contact information. SLAUGHTER responded with the housing unit nurse. STREICHER was unresponsive. With the help of several inmates STREICHER was moved to the floor. Deputy SLAUGHTER requested a ?code 3? medical response. STREICHER was given oxygen and had a pulse at that time. About 2220 hours, fire and Paramedics Plus arrived on scene and began CPR. (MLM2794) Other Investigative Details/ Supplemental Information: On Tuesday, June 24, 2014, about 0050 hours, Deputy A. VALENTINE and I (MONAGHAN) arrived at Santa Rita Jail. The ambulance was parked to the rear ofjail in the employee parking lot. We met with Sergeant SOUZA, Sergeant GONZALGO, and Paramedics J. BARTLETT and J. LEWIS. I spoke with J. BARTLETT, who told me that when they arrived STREICHER was non-responsive and had agonal breathing. His blood pressure was - moments before he became pulse less and apneic. As a result they began CPR. STREICHER was moved to the back of the ambulance and after working on him for 20 minutes, STREICHER met the criteria for them to determine death. I examined STREICHER who was still in the back of the ambulance. He appeared to be obese; he was fully clothed lying supine on a gurney. 1 did not see any trauma to his body. Rigor was present in his jaw and lividity was present but not set. Deputy A. VALENTINE took several photos of STREICHER in the back ofthe Ambulance. Paramedics BARTLETT and LEWIS removed STREICHER from the back ofthe ambulance and assisted us in moving him to a Coroner gurney. secured paper bags over hands with zip ties before securing him in the back of the Coroner van. We met with Lieutenant R. CARTER who provided me with copies of medical records from Corizon. He also gave me all of STREICHER?sproperty. I spoke with Deputy R. SLAUGHTER, who told me had been conducting pill call. He recalled speaking to STREICHER briefly; although he did not recall the exact conversation. Deputy SLAUGHTER said it was nothing concerning health. About 20 minutes later, Deputy SLAUGHTER was notified of someone having difficulty breathing. Deputy SLAUGHTER responded and recognized the inmate to be STREICHER. STREICHER was on the top bunk, against the cage, his bed was the second bed from the A1 door. STREICHER was lying on his side with his back against the cage. The nurse and several other inmates assisted in moving to the floor. Deputy SLAUGHTER informed me that STREICHER only received medication in the mornings. Before leaving I provided Lieutenant R. CARTER a copy of the Coroner receipt. About 0200 hours, we arrived back at the Coroner?s Bureau. 1 processed the body into the morgue and took intake photos of him. Upon reviewing the autopsy protocol, medical records, and incident report associated with this death i find this death to be natural. My findings are based on the cause of death as given by Dr. P. CLOTHING: There is clothing present on the body, consisting of a green t?shirt, a pair of khaki boxer-type shorts, and a pair of blue trousers with an elasticized waistband. The trousers and shorts are pulled down to the area just above the genitalia. Received with the body are two gray socks. Paper bags are present on the hands. There is the following evidence of MEDICAL TREATMENT: l) A plastic airway is present in the mouth. 2) .Two resuscitative electrode pads are adherent to the anterior chest. 3) An identification band is present 0n the left wrist. It shows a picture of the decedent and the name 4) An infusion needle is present in the left upper leg. It is attached to a bag of 0.9% sodium chloride. The needle is embedded in the tibia. The teeth are in good condition. There is COmplete rigidity of the extremities, neck and jaw. Purple lividity is present on the back. ?Lividity is also present in the supraclavicular areas, and the entire face shows lividity. Bloody fluid has run from the nostrils bilaterally onto the face, upward over the eyes and onto the forehead. Some bloody fluid has also run from the mouth onto the right side of the face, There is the following evidence of BLUNT TRAUMA: There is an abrasion on the forehead, right at the hairline, 3/4 inch to the left of the midline. It is a transverse abrasion measuring 3/4 inch in length and 1/4 inch in vertical width. No other trauma is seen on the face or head. There is no evidence of trauma to the neck. The chest and abdomen show no evidence of recent trauma. The right upper extremity, including the hand, shows no trauma. The fingernails are of moderate length and dirty. I The left upper extremity, including the hand, shoWs no trauma. The fingernails have the same appearance as those on the right. There is no evidence of trauma to the genitalia. INTERNAL EXAMINATION THORACOABDOMINAL AND INTERMASTOIDAL INCISIONS are made. HEAD: There is no evidence of trauma to the scalp. The subcutaneous tissue, galea and skull are unremarkable except for the area of abrasion on the mid?forehead, where there is-a 1?1/2 inch contusion in the underlying subcutaneous tissue. 'The galea is not stained. The skull is intact. The meninges show no abnormalities. The external surface of_the brain shows diffuse, moderate gyral flattening, and the unci are grooved. The brain weighs 1390 grams. Cut sections of the brain show no internal abnormalities. tThe vessels of the base of the brain show no abnormalities, and the base of the skull is normal in appearance. NECK ORGANS: The soft tissues of the neck and the cervical spine are unremarkable except that there is slight edema of the aryepiglottic folds and there is what appears to be cyanosis of the tissues of the epiglottis and aryepiglottic area. LUNGS: The left lung weighs 680 grams. The right lung weighs 720 grams. The pulmonary arteries and bronchi are unremarkable. The pulmonary is diffusely markedly congested, with minimal edema. HEART: The pericardial sac contains a small amount of clear yellow fluid. The external surface of the heart shoWs marked enlargement, and it weighs 680 grams. The main coronary artery shows no abnormalities. The left anterior descending branch shows no significant atherosclerosis. The right coronary artery approximately 1/2 inch from the ostium shows approxi? mately 20% narrowing due to some atherosclerosis. The remainder of the vessel is widely patent. The left circumflex branch shows an area of approximately 50% narrowing in one small area. The remainder of the vessel is unremarkable in appearance. The left circumflex and right coronary arteries are equal in size.. The cardiac chambers show no abnormalities. The left ventricle is markedly thickened, measuring 21 to 22 mm throughout. The right ventricle is thickened, measuring 6 mm. The is acutely congested, maroon?brown in color. The gallbladder and extrahepatic ducts are unremarkable. SPLEEN: The spleen is enlarged. It weighs 420 grams. On cut section it is firm and congested. PANCREAS: The pancreas is of normal size and retains a firm, tan, lobular architecture. ADRENAL GLANDS: The adrenals are equal in size. The cortices are thin, bright yellow. The medullae are unremarkable. GASTROINTESTINAL TRACT: The mucosa of the esophagus and stomach is unremarkable. The stomach contains approximately 200 cc of pultaceous food material, within which there are particles of brown?green food material. The duodenum and small bowel contain bile?stained fluid which is quite thick.
