OLDHAM, Gary Clee Case Number: 2015- 00772 Investigator: Tim Engel First Call Information: On Tuesday, Ma1ch 3 2015, about 2320 hours, I (Engel) received a phone call from Nurse P. Fernando of Valley Care Medical Center (VCMC) Critical Care Unit (CCU) Fernando called to report the death of Gary Oldham, a 37 year old male. On Saturday February 21, 2015, about 2022 hours, Deputy D. Lincoln #943 found Oldham hanging in cell 1 in the Out Patient Housing Unit of Santa Rita Jail. Oldham was suspended by a bed sheet. Deputy Lincoln and Deputy J. Rubalcava?Gomez removed Oldham from the sheet ligature and a nearby nurse began CPR. EMS was called and responding paramedics took over life saving measures. Paramedics were able to obtain a pulse. Oldham was transported ?code 3? to VCMC Emergency Room (ER) Oldham went into cardiac arrest and life saving measures were continued. After multiple rounds of attempts to revive Oldham, he was intubated and placed into the CCU on life support. On Wednesday, February 2411,01dham was released out of custody from Santa Rita Jail. On Tuesday, March 3, 2015, about 1500 hours, Oldham Was extubated and was placed on comfort care by his family due to his poor prognosis. About 2245 hours, Oldham?s family members observed him to stop breathing. Nurse P. Fernando was called to the room and pronounced Oldham?s death. Neurochecks were consistently made throughout the rest of the shift and into Saturday, Febiuary 21, 2015. About 2015 hours, Oldham was observed to be lying prone on his mattress. Oldham told the deputy he was feeling okay. About 2025 hours, a deputy and nurse went to Oldham?s cell to give him medication and found Oldham hanging by a sheet formed into a ligature. Two deputies got Oldham down, the nurse began CPR, and 911 was called. Responding paramedics took over CPR. Oldham was stabilized and was transported to VCMC ?code 3? where, on March 3, 2015 at 2245 hours, he eventually succumbed to his Injuries. When Oldham had been booked into Santa Rita Jail he? He did ?0?5 take an medication. Oldham He also? Description of the Death/ Injury Scene: Oldham died in the CCU of Valley Care Medical Center. Prior to being transported to VCMC, Oldham had been in cell #1 of the Out Patient Housing Unit at Santa Rita Jail, where he had hanged himself using a bed sheet as a ligature. Body Identi?cation: While at the hospital, I (Engel) used the ?Blue Check? mobile identi?cation device to obtain Oldham?s ?nger prints. The ?nger prints were a match for Oldham. I placed the results into the case ?le for future reference. On Friday, March 06, 2015, I (Beasley) received a fax from the California Identi?cation Bureau (CIB). A comparison was made between Oldham?s ?ngerprints and the ?ngerprints associated with PFN (Person File Number)- with name Gary Oldham and with DOB: 01/ 17/ 1978 from the records and ?les of the Alameda County Sheriff 5 Of?ce in Oakland. The ?ngerprints were identi?ed to have been made by the same subject. (SB2018) Next of Kin Investigation: Oldham was never married and had no children. His legal next of kin (NOK) is his mother- who was present in the room when Oldham was pronounced deceased. Other Agency Reports: Alameda County Sheriff?s Of?ce (ACSO) Deputy J. An invoice will be mailed to Oldham?s mother,- to request payment. I (Engel) placed a hold on this case pending payment of fees. Other Investigative Details/ Supplemental Information: On Wednesday, March 4, 2015, about 0230 hours, I (Engel) arrived at VCMC ER. I was met by Nursing Supervisor Tami Orr and was escorted to the morgue. I located Oldham?s body in a body bag on a metal tray. The body bag had a tag on the outside with Oldham?s name on it. I opened the bag and saw he was in a supine position and he was completely nude. Oldham had an intravenous line on his right shoulder, a catheter, and bag for collection of fecal waste. Oldham had a light mark on his neck and an approximate 4 inch scar on his abdomen but no other trauma was noted. His skin was warm to the touch. Rigor mortis was minimal. Livor mortis was not observed. I took digital photographs of Oldham. I used the ?Blue Check? mobile identification device to obtain Oldham?s ?nger prints. I placed paper bags over both hands and secured them with zip ties. I moved Oldham over to my gurney and placed him into the Corner?s van. I left for the Bureau. Upon my arrival at the Bureau, I processed Oldham?s body into the morgue and digital intake photographs were taken at that time. All digital photographs were later transferred to a compact disc (CD) and were placed with the case ?le for future reference. Findings: . On Thursday, March 5, 2015, about 0855 hours, Coroner Pathologist Doctor T. Rogers performed an autopsy on the body of Oldham. Doctor Rogers determined the cause of death was anoxic ence halo ath due to due to han inO. Oldham did make the overt action to make a ligature out of a bed sheet and he secured 1gature 1n such a way as to make it possible to hang . himself. Based upon these facts, I (Engel) determined the appropriate manner of death to be suicide. (TPE1611) Supervisor Review: On Tuesday, July 7, 2015, I (Lt. Bowers) reviewed this report for closure. There is a Coroner's identification-tag 'present on the right big-toe. ?The body is 70 inChes long and weighs 155 pounds. The body is presented in a supine position with the head rotated towards the left shoulder. Both arms lie alongside they body. The body is nude. Paper bags cover the distal aspects of both arms. There is no obvious blood on the body. Rigor mortis is fully developed. Lividity is present over the backside of the body. There is the following evidence of MEDICAL THERAPY: l) A vascular catheter in the right subclavian area. 2) A Foley catheter in the external genitalia, secured to the medial aspect of the right thigh. No blunt injuries are noted to the body. The hair is black and about 3/4 inch long. Pupils are round. Irides are brown. Sclerae are white. No petechial hemorrhages are noted. Ears, nose and lips are normal._ A short mustache and beard are present. The neck is nonremarkable. A furrow is not grossly visible about the neck. The chest is flat. Breasts are consistent with an adult male. The abdomen is flat. Just to the right of the midline, at the level of the umbilicus, is a 4?inch, well-healed, linear scar with punctate suture_marks to either side of it. The external genitalia are normal. Over the anterior medial right lower leg is 2?1/2 inch, vertically oriented, well?healed scar. Legs and feet are otherwise nonremarkable. Arms and hands are normal. INTERNAL EXAMINATION The body is opened through the usual BODY CAVITIES: Focal adhesions are present in the left pleural cavity. The right pleural cavity, pericardial cavity' and peritoneal cavity are lined by smooth, glistening surfaces and contain no fluid. Ribs and diaphragms are intact. HEART: In situ, the heart is enlarged, with the apex just beyond the left midclavicular line. The heart weighs 450 grams. The epicardial surface is smooth and glistening. ?Ventricular myocardium is brown. There is a mild, concentric left ventricular hypertrophy present. Chambers are normal. Valves are normal. No myxoid degeneration of the mitral valve is present. No vegetations are present on valve leaflets. Coronary ostia are patent. The surface of the right lung is smooth. Lung is prominently congested and edematous. -In addition, there is a rubbery consistency to the lungs, most obvious in the posterior aspects, consistent with an ARDS. A prominent bronchopneumonia is present in the right lower lobe. Airways contain no sputum. Arterial system is nonremarkable. ORAL CAVITY: The teeth are natural. None are acutely- missing or loose.
