Lacer regarding the suicide death of Inmate Christopher Angulo, at Santa Rita Jail in Dublin. Lieutenant Lacer reported, about 0425 hours, deputies found Angulo unresponsive in his jail cell with a laceration to his right forearm and called 9-1-1. The Alameda County Fire Department responded and performed life saving measures, but were unsuccessful and death was pronounced, at 0440 hours. Angulo?s medical history and next of kin were unknown at the time of the ?rst call. (JDH1886) Medical Summary: Angulo had an apparent self in?icted laceration to the antecubital artery and vein in his right arm. This injury resulted in a severe hemorrhage. Corizon Health Services rovided Angulo?s medical records which showed a history of cm as Description of the Death/ Injury Scene: Angulo died at Santa Rita Jail, located at 5325 Broder in Dublin. He was held in Housing Unit Cell F16, which is a single occupant cell for protective custody inmates. (JDH1886) Body Identi?cation: On June l9, 2015, I (Henshaw) located Angulo?s Alameda County Sheriff?s Of?ce Custody Card on scene. I compared the photograph and general descriptors to the decedent and they appeared to be a match. I searched the Consolidated Records Information Management System (CRIMS) and located a record for Angulo. I compared the photograph and general descriptors from CRIMS to the decedent and they appeared to be a match. (IDH1886) On une 23, 2015, a fax was received from the Alameda County Central Identi?cation Bureau (CIB) con?rming that the ?n er rints taken from the decedent were a match to the ?ngerprints associated with Person File Number?with the name Christopher Angulo. 7) Next of Kin Investigation: On Saturday, June 20, 2015, I (Henshaw) spoke with Contra Costa County Sheriff?s Of?ce De uty Boswell on the telephone and he told me he personally noti?ed Angulo?s When?hf his son?s death. Based on that information, I informed-that he was the legal next of kin and would be in char of authorizin funeral arrangements. -told me the following; Angulo had two minor childrenhan en years old, but was put up for adoption contact with the family. ?who is four years old, lives with his mother?who was noti?ed of Angulo 5 eat y- (JDH1886) Other Agency Reports: Alameda County Sheriff?s Of?ce Deputy J. Gebelein wrote report #15-010423. I received the report, reviewed it, and placed a copy into the case ?le. The information from the report was accurate with the information already provided to me. (IDH1886) Property and Evidence: I (Henshaw) issued Coroner?s receipt #3 6201 to Deputy R. Pelc for Angulo?s body and personal property. (JDH1886) Coroners Fees: There are body removal and body preparation fees totaling $321 associated with this case. (JDH1886) As of Saturday, July 18, 2015, I (Henshaw) saw there was still an outstanding balance of $321 for Coroner?s fees. Iplaced a hold on this case until the fees are paid. (JDH1886) On Wednesday, September 02, 2015, all Coroner?s fees were paid in full. Investigative Details: On Friday, June 19,2015, about 0921 hours, Deputy Bordi and I (Henshaw) arrived at Santa Rita Jail, Housing Unit ?pod. We were met by Deputy R. Pelc who led us to Cell #16 where Angulo was located. I saw Angulo was lying supine on the lower bunk and his lower body, from the waist down, was covered by a blanket. I saw a pool of blood between the right side of Angulo?s torso and his right arm. I performed a limited external exam and saw Angulo?s right arm was covered by a green towel that was drenched with blood. I removed the towel from Angulo?s right arm and noticed a deep laceration on his inner forearm. Rigor mortis was present throughout Angulo?s body and lividity was consistent with his body positioning. I saw miscellaneous papers 3 read out on the cell?s top bunk. One of the papers a cared to be a hand written note that read,? hAngulo?s cell was kept neat and clean and it di not appear a strugg or any foul play had taken place. Deputy Bordi took digital photographs of the scene and Angulo was prepared for transportation to the Coroner?s Bureau. I spoke with Deputy Pelc and he reported that deputies found Angulo unresponsive in his cell at 0425 hours. They immediately placed the Automated Electronic De?brillation (AED) device on Angulo?s chest and CPR was started. He told me Angulo was classi?ed as a ?Protective Custody? inmate who was the only occupant of his cell at the time of the incident. Deputy Pelc stated two razor blades were found in Angulo?s cell, one located inside the toilet and the other behind the mattress. The last time Angulo was seen alive was the prior evening during recreation time. After recreation time was over, Angulo reportedly returned to his cell alone. During the remainder of the evening and early morning hours, Alameda County Sheriff's Of?ce 92%? Gregory J. In the morning when Angulo did not wake up, deputies responded to his cell to check on his status and found him in an unresponsive state with the laceration to his right arm. On Friday, June 19, 2015, about 1104 hours, Deputy Bordi and I returned to the Coroner?s Bureau and processed Angulo into the morgue, which included intake photographs. The scene photographs and intake photographs were later downloaded onto a disc and placed in the case file. On Monday, June 22, 2015, about 1030 hours, Coroner?s Pathologist Dr. W. Herrmann performed an autopsy and determined Angulo?s cause of death to be shock and hemorrhage due to incised right antecubital artery and vein. On Wednesday, June 24, 2015, Central Valley Toxicology (CVT) received Angulo?s femoral blood sample. CVT conducted a complete drug screen on the sample but did not detect any common acidic, neutral or basic drugs. There was also no ethyl alcohol detected. (JDH1886) Findings: . On Friday, July 17, 2015, I (Henshaw) reviewed this case to determine a manner of death. Based on my investigation, the autopsy protocol, toxicology results, and the police report, I ?nd the manner of this death to be a suicide. Angulo was the only inmate assigned to his cell and was found with a laceration to his right arm. There were also two razor blades located inside the cell along with a letter of intent. This ?nding was also supported by the cause of death as given by Dr. Herrmann, shock and hemorrhage due to incised right antecubital artery and vein. (J 886) Supervisor Review: On Saturday, July 25, 2015, I (Sgt. Baron) reviewed this case and found it to be complete. I concur with the ?ndings and approve this case for closure. However, due to the fact the Coroner?s fees are still outstanding, the case will be closed-pending fees and will not be disseminated until all fees are paid in full. The hair is black and cut quite short. The irides are brown. The conjunctivae show no abnormalities. The teeth are in good condition eXcept that the left upper medial incisor is absent with a well?healed gumline. 'There is complete rigidity of the extremities, neck and jaw. Slight lividity is present on the back. The body is dressed in the following items of CLOTHING: A pair of red pants with an elasticized waistband. It has the inscription ALAMEDA COUNTY with a beneath it on the front of the right side of the pants. The next item is a pair of khaki?colored boxer?type shorts. The next item is a pair of gray socks. There is the following evidence of MEDICAL TREATMENT: 1) Multiple EKG pads are adherent to the torso. 2) A resuscitative electrode pad is present on the left lateral chest. Another is present on the right upper chest. It is.particularly congealed just medial to the antecubital area.p Blood has run down to the wrist and onto the proximal thumb of the right hand. A small amount of blood is seen on the fingers. - The fingernails of the right hand are short and clean. A few show the presence of a small amount of blood. No other foreign material is seen. There is a small amount of blood deposited on the fingers of the left hand. The palmar surface is essentially free of blood. Some of the blood on the right lateral chest has run onto the anterior abdomen, as well. TRAUMA TO THE BODY: There is no evidence of trauma to the face or head. None is seen on the neck. The chest and abdomen show no evidence of blunt trauma, and none is seen on the genitalia. The left upper extremity, including the left hand, shOws no trauma. The left lower extremity shows no evidence of trauma, and none is seen on the right lower extremity. .61, 62 63 64 65 Page 4 Sheriff?Coroner Alameda County Body of CHRISTOPHER ANGULO The right upper extremity shows the presence of an incised. wound. It is on the medial aspect of the right antecubital fossa. It measures 1 inch in length. It gapes 1/2 inch and exposes the subcutaneous fat, and tissue beneath the fat can also be visualized. Just aboVe the lateral aspect of this incised wound-are two very superfiCial incised wounds, one measuring 1/4 and the other 1/8 inch in length. There is a crude tattoo on the right forearm just below the antecubital fossa and just below the incised wound. I cannot make out what this tattoo represents. - Just above the incised wound, there is a long scar which extends from the lateral aspeCt of the elbow, medially aoross the antecubital area, and then upward onto the medial aspect of the right upper arm. The total length of this scar is approxi? mately 7 inches, and there are suture scars along it. On the medial aspect of the right forearm, beginning approximately 2?1/2 inches below the elbow, there is a long linear scar measuring approximately 5 inches in length. There may be some suture scars along it. On the anterior aspect of the right wrist, there are some very faint, fine depigmented scars. Four of them can be definitely visualized. There are no scars on the left upper extremity. There are some depigmented scars over the right patella. There is no evidence of trauma to the back, buttocks, or the posterior lower extremities. There are crude tattoos on the buttocks bilaterally. On the left buttock is the word on the right buttock, the word INTERNAL EXAMINATION THORACOABDOMINAL AND INTERMASTOIDAL INCISIONS are made. HEAD: _There is no evidence of trauma to the scalp. The subcutaneous tissue, galea and skull are unremarkable. The .meninges show no abnormalities.- The external surface of the brain shows slight gyral flattening, but the brain on cut section shows no abnormality. The vessels of the base of the' brain are unremarkable. The base Of the skull shows no abnormality. The brain weighs 1480 grams.
