Lacer #1669 who called to report the death of an inmate at Santa Rita Jail in Housing Unit 6. The inmate who was identi?ed as Mario Martinez was having difficulty breathing so he was escorted to the Quasi yard, with his respiratory inhaler, to get some fresh air._ Corizon Health Services (CHS) responded to Housing Unit 6 to give Martinez a breathing treatment, but as Martinez was escorted to the sick call room he became unrespOnsive. CPR was started by Corizon medical staff and 9-1?1 was called. Alameda County Fire Department, Truck 17 and Engine 17, arrived on scene and continued with CPR, but life saving measures were unsuccessful and death was pronounced at 1144 hours. Medical Summary: De Christiansen reported to me that Martinez had an extensive history of I(Beezley) ordered medical records from Corizon Medical Services at Santa Rita Jail. 1? received and reviewed the records and then placed a copy into the Coroner?s Pathologist box for review. The medical records showed Martinez had a history of but did not have any known allergies to medication. (SB2018) Description of the Death/ Injury Scene: I Martinez died at Santa Rita Jail, Housing Unit 6, in Dublin. (SB2018) Body Identi?cation: Deputy Christiansen identi?ed Martinez to me with the use of his jail issued wristband that included his name, photograph and Personal File Number. The photograph on the wristband appeared to match to the decedent. (SB2018) On Friday July 17, 2015, I (Henshaw) received a fax from the Alameda County Sheriff?s Of?ce Central Identi?cation Bureau (CIB) regarding the ?nger print comparison of Mario Martinez. The comparison of ?n er rints from the decedent were a positive match to ?nger prints associated with person ?le number Mario Martinez on record with the Alameda County Sheriffs Of?ce. On Wednesday, July 15, 2015, about 1545 hours, Deputy Meldrum contacted Martinez?s mother to notify her of her son?s death-informed Deputy Meldrum? that Martinez was not marrled an a no children, which made her his legal next of kin. (SB2018) On Wednesday, July 15, 2015, about 1600 hours, I (Bee'zley) called? phone number but someone who identi?ed themselves as the Human Resources Manager at her jO answered the phone. She stated Alameda County Sheriff's Of?ce Gregory J. Ahern, Sheriff Coroner Coroner's Bureau, 2901 Peralta Oaks Ct., Oakland, CA 94605 (510) 382-3000 (510) 382-3033 (fax) was extremely upset at not speak on the phone. I could hear someone crying in the background, so I asked her i?ould call me back. I gave my telephone number, but I have not received a call back. (SB201 Other Agency Reports: Alameda County Sheriff?s Of?ce report number 15-012121. Deputy Christiansen is the primary deputy on this case. I received a copy of this report, reviewed it, and placed the copy in the case ?le. The information provided to me while on scene was accurate, however additional information I reviewed in the report was added to the investigation section of this report. (SB2018) Property and Evidence: I provided ACSO Sheriff? 5 Technician Anderson a copy of Coroner?s Receipt number 36166 for Martinez?s body and personal property. (SB2018) On Monday, July 27, 2015, about 1145 hours, I (Frazier) arrived at Santa Rita Jail in Dublin as requested by Watch Commande pickup money belonging to inmate Martinez and a death summary.W I received the money and I documented it on Coroner?s receipt #36150. At the Coroner?s Bureau, I entered the amount into CME and I placed it into the temporary drop box in the morgue. I placed the copy of the death summary in the case ?le and gave a copy to Coroner?s Pathologist Dr. J. Iocco. (CEF1691) Coroners Fees: There are body removal and body preparation fees totaling $321 associated with this case. (SB2018) On Saturday, August 15, 2015, while preparing this case for closure, I(Beezley) saw the Coroner?s fees have been paid 1n full. (SB2018) Investigative Details: On Wednesday, July 15, 2015, about 1310 hours, Deputy Henshaw and I (Beezley) arrived at Santa Rita Jail. We entered the East side of Housing Unit 6 and saw Martinez lying on his back, on the ?oor outside of the nurses? station, with his hands at his side. Martinez was wearing a pair of boxer shorts and socks. Medical therapies were still in place. There appeared to be bruising on his chest in the area consistent with the use of a CPR device. I was informed that ACFD personnel had Used a mechanical device for CPR. There was no other noticeable trauma to the body. I placed both of Martinez?s hands into paper bags to preserve any trace evidence. Deputy Henshaw took photographs of Martinez and the scene. Deputy Henshaw and I Beezle searched Martinez?s cell, C-8, which he shared with another inmate identi?ed as (PFN: ?had been placed into an isolation cell in the back hall of Housing Unit 6 during the 1nvest1gation. -8 was appeared organized and it did not appear any struggle or altercation had occurred. Martinez?s bunk was the top bunk which contained a rolled up mattress and a plastic bin with personal belongings. I collected photographs and other paperwork from the cell and later placed it with his personal property that 'was provided to me by jail staff. I later irocessed all collected property at the Coroner?s Bureau. A pill was located in cell C-8, later identi?ed as which was located in a plastic cup on the desk. ACSO Detectives Gemmell and Deputy Hawks'collected the pill. Per CHS, neither Martinez 01- had a prescription for the- pill found in the cell. Alameda County Sheriff's Office Gregory J. At 1058 hours, Martinez was out for recreation time in C-Pod whe oticed Martinez appeared to be having dif?culty breathing-told Martinez to tell the Housing Unit Depu staff so they could call for medical assistance. Martinez refused an medical assistance at ?rst. ?convinced Martinez that he needed medical help, so ?Went to the 0d door and contacted the Housing Unit Technician, Padilla, via the pod door intercom button. iinformed Sheriff? 5 Technician Padilla that Martinez was having dif?culty breathing. Technician Padilla then relayed the information to the Housing Unit Deputy, Carriedo, about Martinez?s situation; Deputy Carriedo responded to C?pod, where he saw Martinez having dif?culty with his breathing. Martinez was allowed to walk from the pod into the day room, unassisted, but did so very slowly. Deputy Carriedo requested Sheriff?s Technician Padilla contact CHS to have a nurse respond to Housin Unit 6 for medical attention. Martinez sat at a table in the day room, where he used his inhaler it Deputy Carriedo asked Martinez if he wanted to go outside into the quasi-yard for fresh air. Martinez agreed and walked to the yard with his inhaler and then began to walk around. Deputy Carriedo stated while Martinez was in the yard he could hear him wheezing as he walked by. Deputy Christiansen said, about 1101 hours, CHS Registered Nurse (RN) Inocencio and LicenSed Vocational Nurse (LVN) Padmore arrived at Housing Unit 6. RN Inocencio began preparing a breathing treatment for Martinez who was now having even more dif?culty breathing and was now on his hands and knees in the yard. Martinez was instructed to walk to the nurses? station but remained -.on his hands and knees. Martinez was then assisted to the nurses? station by Deputy Christiansen and LVN Padmore and seated on the ?oor just outside the nurses? station located on the East side of Housing Unit 6. While seated on the floor the breathing treatment was still being set up by Nurse Inocencio. At 1110 hours, Martinez became unresponsive and CPR was initiated by Nurse Inocencio. Deputy Christiansen. contacted Control Point 1 (CPI) via his handheld radio and requested a ?Code 3? medical response. At 1120 hours, Alameda County Fire Department responded and continued to administer CPR to Martinez. At 1126 hours, Paramedics Plus arrived On scene and assisted with Martinez medical aide as well. At . 1144 hours, it was determined that life saving measures had been unsuccessful and death was pronOunced by Paramedic Pita. On Wednesday, July 15, about 1455 hours, Deputy Henshaw and I (Beezley) returned to the Coroner?s Bureau. Deputy Henshaw took intake photographs and we processed Martinez into the Coroner?s Bureau Morgue. The photographs were later downloaded to a disk and placed into the case ?le. (SB2018) On Thursday, July 16, 2015, at 1013 hours, Coroner?s Pathologist Dr. J. Iocco performed an autopsy on Martinez. The cause of death was deferred pending toxicology results. (SB2018) On Friday, July 17, 2015, Central Valley Toxicology (CVT) received Martinez?s femoral blood sample. CVT conducted a complete drug screen on the sample and there was detected. There were no 'other common acidic, neutral or basic drugs detected. There also was no ethyl alcohol detected. See a copy of the included Toxicology report for the levels detected. As I reviewed the report, I saw there was additional information regarding Martinez that I was unaware of 'at the time of his removal. I saw that while Martinez was at his dental a pointrnentI the dentist i Fr Han) had iven Martinez the prescribed medications of and two These medications were given to Martinez approximately 30 minutes prior to his breathing dif?culties. It was also reported that while the asthma breathing treatment was being set up, LVN Padmore provided Martinez with oxygen, via an oxygen mask, from an oxygen tank. I Beezley) spoke with Dr. Iocco regarding Martinez having been prescribed, and administered, prior to his return to the Housing Unit. Dr. Iocco told me the _Was not a contributing factor in the death of Martinez.
