McComas. Lieutenant McComas was reporting the death of Mark- Anthony Laventure Jr, a 33 year old male. Laventure Jr. was in custody at Santa Rita. ail in the City of Dublin. - ?He was classi?ed as an Administrative Segregation Inmate and assigned to Housing Unit 8, A Pod, Cell 13. AdministrativeSegregation Inmates are housed in a cell by themselves. It was reported Laventure Jr. was last seen alive, about thirty minutes, prior to him being found unresponsive. Per Paramedics Plus Patient Care Report, ?Continuous manual CPR was initiated and performed by staff and AED was used but no shock was performed.? Paramedics documented was pale and extremely cold to the touch. PT was placed on the EKG monitor and asystole was con?rmed, no pulses were noted. Pt?s pupils were ?xed and dilated, no rigor mortis was noted. Resuscitation efforts Were considered futile.? I (Henshaw) received the report, - reviewed it and placed a copy in the case ?le. The information from the report was consistent with the information already provided to me, I 7 (JDH1886) Property and Evidence: . . 1(Henshaw) issued Coroner?s receipt #38558 to Sergeant Jones for Laventure Jr. body and property. 8 86) - Coroners Fees: .. There were body removal and body preparation fees totaling $400 associated with this case. (JDH1886) - I On February 18, 2018, I (Henshaw) reviewed the Coroner fees and saw there was $400 still outstanding for this case. (JDH1886) Investigative Details: On October 30, 2017, about 1705 hours, Deputy Ea'sling and I (Henshaw) arrived at Santa Rita Jail in the City of Dublin. We went to Housing Unit 8, A pod. I saw Laventure Jr. lying on a mattress, in the. hallway, on the second tier, outside Cell 13. Deputy Sallam reported he was passing out lunches and he opened the cuffmg port on Cell 13. He saw Laventure Jr. lying in his bed with no shirt on and a sheet was - covering his feet to his waiSt. Deputy Sallam stated it appeared Laventure Jr. was not breathing because he saw no chest rise. Deputy Sallam called into the cell and Laventure Jr. did not respond. Deputies 7 entered the cell and were unable to locate a pulse on Laventure Jr. Medical staff was requested and CPR was initiated. Paramedics arrived and moved Laventure Jr. to the hallway outside the cell. Laventure Jr.?s death was determined, at 1437 hours. During a limited externalexamination I did not see any visible trauma. Livor mortis was present in his back which is consistent with his positioning. Rigor mortis was present in his jaw and not discernible in his limbs. Deputy Easling took photographs of the scene. Paper bags were placed over Laventure Jr.?s hands to preserve any possible evidence. We prepared Laventure Jr. for transport to the Coroner?s Bureau. On October 30, 2017 Alameda County Sheriff Sergeant K. Gemmell requested a press hold be placed on this case. (JDH1886) On October 30, 2017, about 1820 hours, Deputy Easling and I (Henshaw) returned to the Coroner?s Bureau. Deputy Easling tooklintake photographs of Laventure Jr. and we processed him into the morgue. He deferred the cause of death pending the results of the toxicology report. (JDH1886) On November 7, 2017, Central Valley Toxicology completed the medical screen of Laventure Jr.?s femoral blood sample. The screen detected 0.44mg/L Methadone, 0.02 mg/L EDDP, 0.04 d? Methamphetamine and <0.01 mg/L d-Amphetamine. No common acidic, neutral or basic drugs were detected. No ethylalcohol detected. (JDH1886) On February 13, 2018, Coroner?s Pathologist Doctor T. Rogers reviewed the autopsy protocol and toxicology report and determined the cause of death to be ?Multiple drug intoxication.? (JDH1886) On February 13, 2018, Alameda County Sheriff Sergeant K. Gemmell stated the press hold for this case could be removed. A copy of the email was placed-in the case ?le. (JDH1886) - Findings: On February 18, 2018, I (Henshaw) reviewed this case to determine a manner of death. Based on my investigatiOn, the autopsy protocol, toxicology results, and the police report, I ?nd the manner of this death to be an accident. The body is 68 inches long and weighs 160./ pounds..- The body is presented in a supine position. The body is nude. Rigor mortis isfully developed. Lividity is present over the backside of the body. The only evidence of medical therapy are cardiac monitoring ?pads on both upper arms and both lower legs. There is no obvious blood on the body. There are the following BLUNT INJURIES to the body: RIGHT LEG: - 1) A 1/4 inch eschar of the patella. 2) A 1/4 inch eschar on the lateral knee. LEFT 1) Two abrasions over the patella red in color. The- largest is 3/8 inch. There are no other blunt injuries noted to the body. The hair is brown and about 1/4 inch long. Pupils are round. Irides-are brown. Sclerae are nonremarkable. Present in the right antecubital fossa is a 3/4 inch area 'of red discoloration of the skin. It is indurated. In addition, there is a 3/8 inch area of breakage in the surface of 'the skin. No needle puncture marks are identified within this area. The possibility of this representing an induration or a burn cannot be excluded. The fingernails have been clipped prior to autopSy exam. No binding indentation sites are present about the arms or hands. No smoke, powder or stippling is identified on either arm or hand. No needle puncture marks are present. No hyperpigmentEd scars are present over accessible veins. No blunt injuries are noted to arms or hands.> The back is nonremarkable. TATTOOS The body is extensively tattooed on torso, both upper arms, both lower legs and the feet. These are photographed at the time of autopsy exam. BODY CAVITIES: -The pleural cavities, pericardial cavity and peritoneal cavity are lined by smooth,-glistening surfaces and contain no fluid. Ribs and diaphragms are intact. HEART: In situ, the heart is normal, with the apex at the left midclavicular line. The heart weighs 310 grams. The epicardial surface is smooth and glistening. Ventricular myocardium is brown. Chambers are normal. Valves are normal. No left ventricular hypertrophy is present. No aSymmetric hypertrophy is present. No myxoid degeneration of the mitral valve is present. No vegetations are present on valve leaflets. QCoronary ostia are patent. The coronary system has its usual .anatomical distribution and is nonremarkable. No thrombi are present. VESSELS: The aorta and tributaries are minimally involved 7with atheromatous change. The pulmonary artery and venous system contain no thromboemboli. TRACHEA AND BRONCHI: Lined by pale yellow mucosal surfaces. LUNGS: The left lung weighs 1000 grams. The right lung weighs 1100 grams. Pleural surfaces are smooth. INTESTINES: Covered by smooth serosal surfaces. No abnormalities are noted. The intestines are opened and no foreign material is identified. There is a moderate amount of insufflated fecal material present in the large intestine. The rectum is palpated and no foreign material is identified. - LIVER: Weighs 2600 grams.? The serosal surface is smooth. The anterior margin is sharp. The is brown and moderately congested. No tumors are present. No fatty metamorphosis or cirrhosis is present.
