Note: Following the autopsy, I reviewed several of the Sheriff deputies? body cameras from the jail. Those audiovisual recordings confirmed that Mr. appeared to be in an agitated and confused state. The deputies applied ?The Wrap? restraint device. The positioning of the restrain device was notable for the following: 1). His hands were pulled behind his back by a single set of handcuffs. 2) The cinching straps of ?The Wrap? were tightened such that his torso was bent into a ?sitting? position at greater than 90?, compressing his obese abdomen. 3) The cinching straps also were pulling his thick neck downward onto his chest. 4) Finally, an opaque spit mask covered his face, including his nose and mouth. In my opinion,_each these factors increased this gentleman?s risk for Furthermore, the opaque spit mask made difficult any assessment of Mr. medical status during transit down the jail hallway to the outpatient housing unit. Except for the force used as an attempt by deputies to gain compliance of Mr. and the force used to apply the restraint device, I saw no evidence of other significant force used against him in the recordings. I did not identify unexplained blunt injuries on the body. Postmortem.toxicology studies do not provide a clear explanation of his agitated and confused state. Page Sheriff-Coroner Alameda County Body of DUJUAN OMAREA CIRCUMSTANCES OF DEATH This 23?year?old gentleman was servimg a weekend sentence at Santa Rita Jail. Sometime after admission, he showed bizarre behavior and became assaultive. This appears to have corresponded to his reporting that he had used drugs. A urine drug screen by medical staff was positive for marijuana, cocaine and methamphetamine. Soon thereafter their attempts at restraining him, he became unresponsive. Resuscitation restored a pulse; however, he expired in a local emergency department. PRELIMINARY EXAMINATION The unembalmed body of a young man is in a plastic pouch, is dressed only in blue underpants, and has a properly labeled identification tag. Paper bags are on the hands. Fingernail clippings are taken. RECENT INJURIES Minor external injuries are described below. EXTERNAL EXAMINATION The body of a well?developed, moderately to markedly obese, young, black man is reported to be 23 years, 69 inches, and 266 pounds. Rigor mortis is moderate. Occasional petechiae are (M1 the conjunctivae and sclerae. The eyes are brown and cloudy. The nose and mouth show no lesions. The teeth are in fair to good condition. An endotracheal tube is present. The ears are intact. The neck is symmetrical without significant trauma or scars. The chest is symmetrical without significant trauma or scars. There is aa 2X2 inch, red-tan, irregular?shaped area on the sternal area consistent with cardiopulmonary resuscitation. The breasts are of 21 man. The abdomen is protuberant but not distended and shows no significant trauma or scars. ECG pads are on the torso. The external genitalia are those of a male with a short foreskin. The forearms and upper arms are symmetrical without significant scars. There is a tattoo of a 5?pointed star and an undecipherable word (N1 the right forearm. The hands, fingers, and fingernails are intact. On the ulnar surface of the right wrist is a transverse l-l/2X1/8 inch red-brown abrasion. The mediastinum shows no hemorrhage. The pericardial sac is intact. The diaphragm is intact. The peritoneal cavity shows no adhesions and minimal liquid. The retroperitoneal fatty tissues are moderately to markedly increase. The major organs are normally positioned. CARDIOVASCULAR SYSTEM: The epicardium is smooth and glistening. The coronary arteries follow a balanced distribution and show no atherosclerosis. The heart is 450 g. The myocardium is red?brown, normal texture, and uniform. The right ventricular free wall shows no increased fatty infiltration or increased fibrous tissue. The left right ventricles are 1.4 to 1.8 cm and 0.2 cm, respectively. The chambers are all moderately dilated. The endocardium, chordae, pepper most is intact. The coronary sinus ostium is widely patent. The foramen ovale is closed.
