SUMMARY OF INVESTIGATION This investigation was initiated on 2 Aug 18 based upon information from the 88th Air Base Wing Base Defense Operations Center (BDOC), 88th Security Forces Squadron {88 SFS), Wright-Patterson AFB (WPAFB), OH, who relayed a Security Forces member discharged his firearm to make entry into the Wright-Patterson Medical Treatment Facility Family Health Red Clinic (FHRC), in response to an active shooter report. On 2 Aug 18, WPAFB was having an active shooter exercise at the Kittyhawk Cha e1. At approximately 1228 hours, WPAFB Fire and 88 SFS received a 911 call from Civ I 88 MDG, WPAFB, OH reporting her friend, Civ 88 MDOS, WPAF B, OH I explained [??Em?imm injured her ankle somewhere on base and needed help. As a result, SF dispatched patrols to the to look for_ SFS members at the exercise were initially confused and believed this report was an exercise inject, as ten role players portraying injured patients were being bused to the from the active shooter exercise. While he was still at the door, beginning to back away, he heard three to six gunshots to the right of his head. He backed up quickly. saw a ?ash come across the top of his M-4 and observed damage to his weapon. was confused, and thought his weapon might have ?red when he struck the glass, so he ensured he had all his ammo. did not see who ?red the weapon. When the shooting stopped he pushed out the remaining glass and opened the door to the FHRC. The main entrance and Emergency Room (ER) is on the South side of the hospital facing south. There is a clinic entrance with a rotating door on the west side of the hospital facing west. Upon arrival, the entrance to the hospital was restricted by SFS and a crime scene was already established in the hallway leading to the FHRC. The interior of the hospital was illuminated by ?orescent lights. The FHRC was located on the ?rst floor of the hospital to the west of the main entrance and adjacent to the ER. Inside the FHRC was a waiting area, check in desks and multiple offices and patient rooms. Prior to AFOSI taking control of the scene, SFS personnel had cleared the FHRC of bystanders who had been locked in their rooms and hiding from the report of the active shooter. The scene was changed during this process and objects, medical carts, and other items were moved during the clearing process. At the start of scene processing, many military and civilian workers were attempting to gain access to the scene to retrieve cellular phones and car keys. They were not permitted to enter; however, retrieved personal items throughout the scene and provided them to large crowd assembled outs1de the perimeter of the FHRC. These items were evaluated as potential evidence before being removed. SA Iphotographed injuries sustained from his weapon being struck by one of the rounds ?red by [bit?ilbimtci I SA-conducted an initial of the scene with SA and donned personal protective equipment (PPE), which consisted of one pair of nitrile gloves. SA- noted ?ve shell casings in the hallway outside of the FHRC. At the entrance to the FHRC the door was propped open and the window was broken. Shards of glass were on the ?oor at the door, inside the FHRC and outside into the hall way of the FHRC. Metal fragments were on the floor at the door and in the hallway outside of the FHRC as well as small fragments of copper. Continuing inside the FHRC the wall straight ahead of the clinic entrance had a Stop sign on it. The wall was the north wall of room 1R19 was labelled wall A. Wall A had three to four apparent bullet defects. In front of the doorway to 1RI9 there were two copper metal fragments on the ?oor. Inside 1R19 the north wall was labelled Wall B. On Wall two apparent bullet defects were noted. The south side of room 1R19 was labelled Wall and one defect was noted. To the south of room 1R19 was room 1R33. The North wall in room 1R33 was labelled Wall and one bullet defect was noted. The south wall in room 1Rl9 was labelled Wall and one bullet defect was noted. There was a utility cart in the hallway south of room 1R33 with a blue piece of cloth pull up resting on the top of the cart. In the hallway were small pieces of red plastic and small pieces of blue fiber. There was damage in the cart to two of the red plastic bins and appeared to have been struck by a projectile. The cart was moved by SFS clearing teams but they could not name who speci?cally moved it. The only information provided was that it was seen in front of room 1R34. Under the cart along the south wall in the hallway was a copper fragment. The outside wall of IR34 was labelled Wall and had seven apparent bullet defects. Inside room IR34 on the opposite side of the wall to Wall was labelled Wall and had ?ve observed bullet defects. The south wall in IR34 was labelled Wall and consisted of three bullet defects behind a decorative sheet hanging on the wall. IR34 also had a printer on a desk on the east side of the room which contained a bullet defect. Blood was reported to be found to the east in the hallway outside of the FHRC. As well as outside of stairwell 1 on the east side of the hospital. Two spatter patterns were identi?ed on the floor in the hall and on the lower portion of the wall. Blood Pattern 1 (BPI) was on the floor in the hall and consisted of 15 20 stains, some having been wiped on the floor. Blood Pattern 2 (BP2) consisted of blood stains on the lower portion of the hall wall and consisted of approximately 32 smaller stains. SA-was noti?ed medical personnel were using fake blood as part of their exercise and EH and BP2 were suspected to be from that. SA- performed a presumptive test for blood using Quick Check Green on both BPI and BP2. The results were negative on both stains. The stains were photographed with and without scale. Imwmc? Iwas located on the inner portion of the doorway leading to a stairwell labelled stairwell 1LS1. This stairwell was on the east side of The stain appeared to be multiple transfer stains on the wall. All walls containing potential bullet defects were photographed and most of the defects were photographed. SA- ontacted IForensic Sciences Consultant (FSC), AFOSI 3rd Field Investigations Squadron (FIS), Specialty Fli_ht Joint Base San Antonio (JBSA) - Lackland, TX and briefed her on the scene. SA?established Facetime communication with SA and SA lg??mml conducted a FaceTime walk through of the scene with SA SA I?m??i?bmci Iinformed SA-she would come out to assist in crime scene reconstruction and to get to a point where processing the scene could stop and be secured until her arrival. SA- stopped documentation of the crime scene and the crime scene was secured and sealed using evidence tape. 17 Pieces of evidence were collected during the processing of this scene to include 5 brass shell casings, pieces of black fiber, pieces of copper fragments and pieces of metal fragments. AFOSI collected all evidence and equipment and exited the scene. AFOSI departed the scene and returned to AFOSI 10 F18. 5215 Thurlow St, building 70. WPAFB, OH. to log and secure all items of evidence. On 4 Aug 18, SA and SA- returned to the scene to conduct reconstruction of the shooting and conduct a search for projectiles in the walls of the FHRC. The crime scene tape placed on the door from 2 Aug 18 was still intact and did not show any signs of tamper. During the processing the walls were relabeled and the bullet defects were mapped on each surface and re-photographed (Exhibit 29). Several bullet defects were tested in an effort to determine if they were likely made by a ?red projectile. The tests used for this purpose were dithiooxamide (DTO) for the presence of copper residue (jacketing) and sodium rhodizonate (SR) to test for the presence of lead. It is possible for a bullet defect to test positive on one test but not the other depending on the makeup of the *34?4218221 1522* Page 027 File No: 1F 10-C-134-N 2-3474218221 1522 fragment which came in contact with the suspected bullet defect. All measurements taken were approximate. The exterior to the FHRC was labelled A and when facing the FHRC had a shrapnel pattern west of the broken window labelled A1. The approximate center of A1 to the floor was 4?6? and 1.5? to the right side of the door. There was a suspected bullet defect in the frame of the window on the left side of the window labelled A2. A2 was a bullet defect with a smaller diameter on surface A and a larger diameter on surface indicating a possible directionality of a projectile moving from outside the window to inside the FHRC. Al was tested using DTO and SR and had a faint positive for DTO and a positive for SR. A2 was tested using DRO and SR and had a faint positive for both tests. A3 was a defect located on the east door of A. it consisted of a dent in the wood of the door which was 4?6? from the floor and 3? 1:14? from the right door frame. A3 was tested using DTO and SR and was positive. A3 looked to be caused by an object striking the door with force. Surface A had two pieces of paper taped to the door. A2 had a hole consistent with a projectile ?red through paper indicating it was there at the time the shooting took place. A3 was made through the other piece of paper and showed a directionality of the force moving from right to left to right at a upward angle. To the right of the door was a half wall labelled and had a shrapnel pattern labelled 131. The top of the shrapnel pattern was 5? 9? from the ?oor and the lowest portion of the pattern was 4? 4? from the ?oor. A large piece of B1 was tested using DTO and SR and had a negative reaction. The inside of the FHRC door was labelled C. The opposite side of bullet Defect A2 was photographed. Wood was broken from the frame and back towards the interior of the FHRC. The external north wall of room was labelled D. had four suspected bullet defects labelled D1 D4. D1 was in the upper left corner of the wall and after closer inspection did not appear to be a bullet defect. D1 did not test positive in presumptive tests for lead and copper indicating it was most likely not related to the shooting. D2 was 4?5? from the ?oor and 7.75? from the left edge of D. D3 went through a sign shaped like a stop sign a quarter of an inch thick made of foam-core material and continued into the wall of D. D3 was 5? from the floor and 1? 7? from the left edge of D. D4 was from the ?oor and 3? from the door frame of room 1R19. D5 was located 3?3? from the ?oor and 2?8? from the door frame of room IR19. A fragmented piece of copper was observed in D4. D1 was believed to be a prior existing hole and was tested using DTO and SR. Both tests had a negative result. D4 was tested using DTO and SR and tested positive using DTO and negative using SR. A cut was put into the wall of and resulted in the ?nding of a metal fragment which stopped and rested on the ?oor inside the wall between and E. A metal fragment was recovered inside of D4 stopped in the wall. D3 and D4 appeared to travel through to E. The internal north wall of room IR19 was labelled E. had three suspected bullet defects labelled El- E3. M1 was located in a metal railing of the scale on the rail closest to 1R34. M1 was 2? from the floor. M1 perforated the railing on one side and left a dent where it failed to perforate the opposite side of the railing. The railing was removed and resulted in the recovery of several large pieces of metal and many small fragments resting where the railing met the scale. The west side of the hallway from the main entrance of the FHRC to IR34 was labelled N. had a suspected bullet defect labelled N1. N1 was 2?4? from the ?oor and 8? 374? from the corner of wall D. N1 was tested using DTO and SR and tested positive for DTO and negative for SR. There was a medical cart with supplies believed to be in front of wall I at the time of the shooting. The cart was located west of IR34 at the time of crime scene processing and was moved as Security Forces and Dayton Police Department cleared the FHRC. Tiny fragments of blue ?ber consistent with a blue cloth were on the floor in the hallway as well as broken pieces of red plastic from bins located on the medical cart. The cart was not able to be accurately placed where it may have existed at the time of the shooting. Ballistic rods were used inside the bullet defects along with strings in an effort to show potential trajectory of the projectiles. Due to many of the bullet defects being elongated, projectiles were most likely tumbling and entered the walls sideways or were altered by another external force such as striking the window, walls or the medical cart. The results of the stringing show a potential route of travel for the projectile based on the bullet defect in the wall and should be taken as an estimate and a possible route of travel only. Approximately 30 minutes after the exercise be_an, the BDOC received a call from the WPAFB Fire Department relaying they received a?call from a female who was possibly injured near the and whose nhone nossibly died before they could obtain more information. _conf1rmed withithat the call was not related to an exercise inject, so the BDOC directed a atrol to respond to the area. After confirming the call related to the injury was ?real his attention back to the exercise scenario. Due to how they were provided no times could be approximated as the ?les would play each transmission from multiple sources. Some of these transmissions could be heard in the background happening simultaneously as the previous transmission.
