This summary was provided to employer and employee representatives that participated in a PERRP enforcement visit closing conference held on this date. Item # Standard 1 29 CFR 1910.67(c)(2)(ii): Only trained persons shall operate an aerial lift. On or about June 17, 2019, employees were not trained on minimum approach boundaries, while operating an Altec insulating aerial device (1FVAACYDY38DAC9062); which exposed employees to electrocution, shock and arc flash hazards. Corrected During Inspection 2 29 CFR 1910.137(c)(2)(viii): Electrical protective equipment shall be subjected to periodic electrical tests. Test voltages and the maximum intervals between tests shall be in accordance with Table I-4 and Table I-5. On or about June 17, 2019, rubber insulating gloves were not subjected to periodic electrical testing in accordance with Table I-4 and I-5 in that; the most recent electrical tests were performed in September 2018, and some gloves in the workplace had inspections dated June 2, 2017. Corrected During Inspection 29 CFR 1910.303(e)(1)(ii): Marking - other markings giving voltage, current, wattage, or other ratings as necessary. On or about June 17, 2019, the street light control cabinet (240 volts) on the Duke Pole #HI23581RT, was not marked with the nominal voltage; which exposed employees to electrocution, shock, and arc-flash hazards; at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. Corrected During Inspection 3 4 29 CFR 1910.332(c): Type of training. The training required by this section shall be of the classroom or on-the-job type. The degree of training provided shall be determined by the risk to the employee. Finding(s) On or about June 17, 2019, employees that performed electrical maintenance activities did not receive training on topics that included but were not necessarily limited to: risk factors, duties, voltages, line clearances, and the removal, installation, or maintenance of a street light luminaires; which exposed employees to, electric shock, explosion, and arc-flash hazards; at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. PAD Item # 5 6 Standard 29 CFR 1910.333(b)(2)(i): Procedures: The employer shall maintain a written copy of the procedures outlined in paragraph (b)(2) and shall make it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives 29 CFR 1910.333(c)(2): "Work on energized equipment." Only qualified persons may work on electric circuit parts or equipment that have not been deenergized under the procedures of paragraph (b) of this section. Such persons shall be capable of working safely on energized circuits and shall be familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools. Finding(s) PAD On or about June 17, 2019, the employer did not maintain a written copy of procedures to ensure a lock out tag out process rendered equipment inoperative before maintenance of a street light luminaire; which exposed employees to electric shock, explosion, and arc-flash hazards; at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. Corrected During Inspection On or about June 17, 2019, identification of qualified persons that performed installation and maintenance of street light luminaires, wooden poles, and duplex service lines did not include a determination that they were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. Note: The work practices used by qualified persons installing insulating devices on overhead power transmission or distribution lines are covered by 1910.269 of this Part, not by 1910.332 through 1910.335 of this Part. Under paragraph (c)(2) of this section, unqualified persons are prohibited from performing this type of work. If work is to be performed near overhead lines, the lines shall be deenergized and grounded, or other protective measures shall be provided before work is started. If the lines are to be deenergized, arrangements shall be made with the person or organization that operates or controls the electric circuits involved to deenergize and ground them. If protective measures, such as guarding, isolating, or insulating, are provided, these precautions shall prevent employees from contacting such lines directly with any part of their body or indirectly through conductive materials, tools, or equipment. On or about June 17, 2019, employees relied on the operation of a photoelectric switch to deenergize street lights before installation and maintenance activities were performed, which exposed employees to shock and electrocution hazards; at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. 8 29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed. On or about June 17, 2019, the employer did not provide and require employees to use appropriate electrical equipment that included but was not necessarily limited to: insulating sleeves; face shield and Fire Rated (FR) clothing and was not based upon a shock and arc-flash risk assessment for electrical equipment maintenance and repair tasks; which exposed employees to electrocution and shock hazards. 9 29 CFR 1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee shall use insulated tools or handling equipment if the tools or handling equipment might make contact with such conductors or parts. If the insulating capability of insulated tools or handling equipment is subject to damage, the insulating material shall be protected. On or about June 17, 2019, an employee that performed installation and maintenance of street light luminaires, wooden poles, and duplex service lines did not use electrically insulated tools that included but were not necessarily limited to: insulated side-cutting pliers; insulated round-shank screw drivers; and an insulated wire/stripper cutter when work was performed on energized electrical equipment which exposed the employee to electrocution and shock hazards. The employer shall post and maintain proper warning signs where such a circuit exists. The employer shall advise employees of the location of such lines, the hazards involved, and the protective measures to be taken. 12 29 CFR 1926.417(b): Equipment and circuits. Equipment or circuits that are deenergized shall be rendered inoperative and shall have tags attached at all points where such equipment or circuits can be energized. On or about June 17, 2019, employees were installing a pole using a noninsulated Altec digger derrick (vin# 1FVHG5CY9EHFS0551), work was performed closer than minimum approach distance of 10 feet near a 7.2KV energized power circuit, at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. b. On or about June 17, 2019, an employee was installing a duplex cable to a ceramic insulator ball and performed work closer than minimum approach distance of 2 feet and 0 inches near a 7.2KV energized power circuit using an Altec Bucket Truck (vin #1FVAACYDY38DAC9062), at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. a. On or about June 17, 2019, employees were not advised of the hazards involved and the corrective measures to take while installing a pole using a non-insulated Altec digger derrick (vin# 1FVHG5CY9EHFS0551), at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. b. On or about June 17, 2019, an employee was not advised of the hazards involved and the corrective measures to take while installing a duplex cable to a ceramic insulator ball using an Altec insulating aerial device (vin #1FVAACYDY38DAC9062), at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. a. On or about June 17, 2019, employees did not deenergize and tagout circuits while installing a pole using a non-insulated Altec digger derrick (vin# 1FVHG5CY9EHFS0551) near a 7.2KV energized power line, at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223. b. On or about June 17, 2019, an employee did not deenergize and tag out circuits while installing a duplex cable to a ceramic insulator ball near a 7.2KV energized power line using an Altec insulating aerial device (vin #1FVAACYDY38DAC9062), at a temporary work location situated at or about 4796 Hawaiian Terrace, Cincinnati, Ohio 45223.
