Complainant consumed a Bubba's Burger, which consists of a cheeseburger (requested to be cooked medium) with bacon and egg on top, mayonnaise. Complainant also consumed bleu cheese chips and watermelon. Inspector spoke with owner who stated that no complaints have been received and no employees have reported gastrointestinal illness. The owner stated the following: -the ground beef is received frozen and thawed in the walk in refrigerator. An employee forms the ground beef into patties. One batch of patties will turnover in less than one day. -the bleu cheese dressing is made onsite. -the mayonnaise, bread is received premade. -the burger is cooked per order. -the chips (potato) are fried from a frozen state. -the watermelon is washed before it is prepped. Inspector observed the following: -proper handwashing and glove use by cook. -internal temperature of raw beef pattie, mayonnaise, and raw shell eggs at the left cooksline refrigerator at 39 degrees F. -final internal temperature of the medium cooked burger at 147 degrees F. The menus onsite have both the disclaimer and reminder statement (consumer advisory) for undercooked meats. -final internal temperature of the egg and bacon at 189 degrees F. -no cross contamination observed at time of dish reproduction. -internal temperature of the bleu cheese dressing stored at the right side cooksline refrigerator at 53.6 degrees F. This unit was impounded due to the inability to maintain potentially hazardous foods at internal temperature of 41 degrees F or below. -cut watermelon at internal temperature of 49.8 degrees F, stored in the walk in refrigerator. This unit was impounded due to the inability to maintain potentially hazardous foods at internal temperature of 41 degrees F or below. -sanitizer buckets at the cooksline with a concentration of 200 ppm quaternary ammonia. -dish machine (main sanitizing method) dispensing 0 ppm chlorine at time of arrival. This was serviced to dispense 50 ppm chlorine while inspector was onsite. -proper explanation of how to sanitize utensils and dish ware in the 3 compartment sink. The owner decided to voluntarily close the facility at time of inspection due to the walk in refrigerator impound. This facility earned a score of 80 = B.
