healthy participants

Dear Dr. Makeig:
Please be advised that the Board has been notified that co-investigator, Dr. Murphy, has financial or other
interests related to the research that may constitute a conflict of interest. The HRPP staff have been unable
to identify where these interests have been disclosed in the Research Plan or other materials submitted for
IRB review. You are requested clarify whether or not Dr. Murphy has financial or other interests related
to the research or the investigational product. If Dr. Murphy has such interests and they were previously
disclosed to the IRB, please specify the document and submission date. If Dr. Murphy has such interests
and they were not disclosed to the IRB, please submit an amendment that:
 Modifies study documents accordingly;
 Provides the IRB with the outcomes of any Conflict of Interest review;
 Updates the IRB about how many subjects have provided consent since the start of the study and
where they stand in the study timeline;
 Updates the IRB about how many of those subjects were consented and/or underwent research
procedures by or under the direction of Dr. Murphy;
 Describes corrective actions to assure proper disclose in the future.
Please upload your reply in the form of a cover letter that clearly describes, on a point-by-point basis, how
the request(s) for clarification/revision were satisfied and two copies of revised study documents (one
copy that clearly and specifically highlights all changes made such as by using the track changes function
in Microsoft Word and one clean copy) to this project number using e-IRB services at https://irb.ucsd.edu.
Final approval will be forwarded just as soon as we can determine that your responses are satisfactory.
