Investigation 04/26/2011 On 4/26/11 Dep Brian Spiegel's DAVID certificate was utilized to conduct an inquiry on Sheriff Nocco's FL DL Anthony 33664 Serrano SI2012-056 SI Supervisor Investigation 06/09/2012 On June 9, 2012 Cpl Anthony Serrano failed to complete an extra duty assignment without making proper notifications. (Sustained - LOR) Anthony 34963 Serrano IA2013-018 I.A. Investigation 03/19/2013 A) It is alleged that School Resource Officer Anthony Serrano failed to take appropriate action that potentially could have placed a sixteen year old student in danger. Specifically, it is alleged that Cpl. Serrano was informed by a student of someone dealing a new type of pill on the streets. The student asked Cpl Serrano if he wanted him to get some of the pills for Serrano to test. Cpl Serrano authorized the student to obtain the pills. The student then approached the dealer and obtained two types of pills. The student provided the pills to Serrano who subsequently initiated a "Found Property" report. (Sustained - 2 day suspension, reassigned) B) On March 20, 2013, sixteen year old Tyler Siedlecki provided SRO Anothny Serrano with two pills he allegedly obtained from a suspected drug dealer at the Land O' Lakes Recreational Center. On March 20, 2013, SRO Serrano initiated a found property report, and failed to document all the facts/circumstances leading to the discovery of the pills. It is alleged that SRO Serrano inaccurately documented that Siedlicki "found" the pills at the Land O' Lakes recreational Center and failed to accurately document all the facts/circumstances leading to the discovery of the pills. (Sustained-LOR) C) It is alleged that School Resource Officer Anthony Serrano discussed intimate information with students at Sun Lake High School. Specifically, it is alleged that SRO Serrano accessed at least (1) dating website (ChristenMingle.com) on the school computer and showed students pictures of women he dated, or was interested in dating. He told a student about giving his telephone number to one of the teachers (Ms. Dell) and later shared how upset he was when the teacher didn't call him. He discussed his interest in dating Assistant Principle Tracy Bruno and showed the student a picture of her on his personal cell phone. He spoke to students about his financial struggles and how he needed to work off duty details. (Sustained-LOR) Terra 36836 Winthrop VC2013-079 Vehicle accident 12/03/2013 Avoidable - Corporal Winthrop was making a left turn from US 19 onto Camry Drive. Southbound traffic on US 19 was heavy and the three southbound lanes had stopped with enough room for Corporal Winthrop to amke the turn. A silver scion travelling southbound in the right lane of US 19 failed to make the right turn onto Camry Drive and 1 PASCO SHERIFF’S OFFICE PROFESSIONAL STANDARDS VIOLATION HISTORY 727-815-7117 continued southbound in the right turn lane. The collision occurred in the right turn lane of US 19 and caused minor damage to both vehicles. There were no injuries reported while on scene. (Sustained - LOR) Michael 37292 Mcknight IA2014-010 I.A. Investigation 01/24/2014 A. On January 24, 2014, while off duty, Michael McKnight responded to ' (stepson) residence for a welfare check in regards to a possible domestic dispute between and his live-in girlfriend, . During Michael McKnight's contact with , attempted to leave in his vehicle. Michael McKnight prevents from leaving by opening the driver's door and entering his vehicle. Michael McKnight began to struggle with to take the car keys away. During the struggle sustained an injury to his left eye and scratches on his back. alleged that Michael McKnight intentionally used his elbow to strike him in the face with caused his black eye. Refer to the criminal investigation for complete details (Case #14-3048, Burglary/Batters - 810.02 (2) (A). (Sustained - LOR) B. (Unfounded) Donald 37947 Fly VC2014-042 Vehicle accident 05/15/2014 Corporal Fly was exiting a shopping center on Ridge Road when his patrol car made contact with a vehicle that had just pulled out of the same shopping center. The driver of other vehicle said her vehicle stalled because she started in third gear. Corporal Fly began to turn and directed his attention toward oncoming traffic and failed to observe the other vehicle stalled in the road. Corporal Fly was unable to stop before hitting the stalled vehicle. The only damage to other vehicle was a small mark on the rear bumper. Corporal Fly's patrol care only had damage to the push bar. There were no injuries reported while at the scene. Corporal Fly was issued a traffic citation for careless driving by FHP. (Sustained - LOR) Brian 38057 Hernandez SI2014-081 SI Supervisor Investigation 07/30/2014 Deputy Hernandez was scheduled to work an off duty detail 7/30/14 at Saddlebrook Resort from 0100 to 0700. Deputy Hernandez did not work the detail, he did not advise the off duty coordinator or the shift commander that he was unable to work the detail as per the listed general order. (Sustained - LOR) David 38290 Hink SI2014-092 SI Supervisor Investigation 08/31/2014 A) On or about Friday, 8/29/14, Lt. J. Law contacted Cpl. David Hink via telephone to advise him of his transfer from SRO to Patrol effective Monday, 9/1/14. He further instructed Cpl. Hink to contact District ll Capt. K. Gregory on Sunday, 8/31/14, to received his assignment for the following Monday. Cpl. Hink failed to follow Lt. Law's 2 PASCO SHERIFF’S OFFICE PROFESSIONAL STANDARDS VIOLATION HISTORY 727-815-7117 instructions and later admitted his refusal was willful and intentional. He stated he had made plans for the holiday weekend and was concerned he would be required to cancel them if they conflicted with his new assignment. He further stated the Sheriff's support in the Northeast Pasco would suffer when his transfer and subsequent retirement became public. (Sustained - resigned under investigation) B) On or about Friday, 8/29/14, Lt. J. Law contacted Cpl. David Hink via telephone to advise him of his transfer from SRO to Patrol effective Monday, 9/1/14. Lt. Law further instructed Cpl. Hink to contact District ll Capt. K. Gregory on Sunday, 8/31/14 to receive his assignment for the following Monday. Cpl. Hink did not follow Lt. Law's instructions and subsequently failed to report for work on Monday, 9/1/14. When I reached Cpl. Hink by phone, he stated he had no intention of returning to Patrol and would "be in" on Tuesday 92/14 to submit his "retirement" papers. (Sustained - resigned under investigation) Donald 38390 Fly SI2014-095 SI Supervisor Investigation 08/02/2014 On August 2, 2014, Corporal Donald Fly utilized the off duty detail system and selected to work a detail at Stagecoach subdivision on August 2, 2014 at 8:00 PM. Approximately four minutes later, Corporal Fly realized the date would conflict with another off deputy detail he was scheduled to work. August 2, 2014, was a Saturday so he emailed the off duty coordinator to advise he could not work Stagecoach Subdivision. The detail was not worked. Corporal Fly did not notify the on-duty shift commander that he would not be able to work the Stagecoach subdivision detail. (Sustained - LOR and forfeited 60 days extra duty.) William 38466 Hatfield IA2014-036 I.A. Investigation 09/10/2014 A) It is alleged that on 09/10/24, SRO Hatfield was aware that Gregg Raptelis, father of HHS student, Alex Raptelis, had an active warrant for his arrest. It is further alleged that SRO Hatfield came in contact with Alex's father on multiple occasions while he was on campus; however, SRO Hatfield did not take Raptelis into custody. Raptelis had an active misdemeanor warrant (FTA - Failure to Vaccinate) from 04/08/14 through 11/02/14. (Sustained - POR) B) On 10/03/14, SRO Hatfield completed a report concerning a Battery on School Employee under PSO case#14035189, involving special education student, Alex Raptelis, and teacher, Denise Watson. Corporal Hatfield documented in his report that he obtained a verbal statement from the victim; however, the victim alleged that no interview or verbal statement was given or asked for. Corporal Hatfield further documented that a wants and warrants check was conducted with negative results; however, a TAR request revealed that no criminal history was completed on any involved parties (juvenile suspect, victim, suspect's father). The suspect's father, Gregg Raptelis, had an outstanding misdemeanor warrant for his arrest at the time of incident. (Unsubstantiated) David 40256 Grant SI2015-104 SI Supervisor Investigation 10/11/2015 On 10/11/2015 Corporal D Grant was scheduled to work an extra duty detail at Harley Davidson, NPR, at 1000 hours. Corporal missed the detail and believed he made an error inadvertently booking the detail while reviewing it. (Sustained - LOR, 90 suspension from extra duty) 3 PASCO SHERIFF’S OFFICE PROFESSIONAL STANDARDS VIOLATION HISTORY 727-815-7117 Richard 40265 Stackon SI2015-106 SI Supervisor Investigation 11/08/2015 On or about Monday, November 9, 2015, I was advised of a post on former Corporal Jessica Gillotte's facebook page (aka - Jessica Wessica) in which current SRO Corporal Richard Stackon made a disparaging comment about the PSO command staff. Corporal Stackon wrote, "Incompetence and arrogance breeds a dangerous battlefield" in reference to the following comment that Jessica made, "....Everyone up to LT at PSO is great. After that...Captains and above...Everything went to shit. To the point that I was almost killed, but email threads about auto burglaries were more important..." It is alleged that the aforementioned Facebook post made by Corporal Stackon on November 8th, 2015, violated G.O. 26.4 due to Corporal Stackon referring to and "liking" former Corporal Gillotte's post. (Sustained - LOR) Michael 40390 Mielke IA2015-045 I.A. Investigation 12/11/2015 A) Allegation that Michael Mielke was inattentive to his job duties or otherwise neglected his work. Specifically, on December 11, 2015 from approximately 100 hours to 1030 hours, Sergeant Mielke, while on duty and driving a PSO vehicle, attended an internment ceremony for a private citizen at the Saint Anthony's Cemetery in San Antonio, Florida. Sergeant Mielke did not request or secure permission from his chain of command to attend the listed ceremony, and he did not reflect his temporary departure from duty on his signed and submitted time sheet. (Sustained - Suspension 2 days) B) Allegation that Sergeant Michael Mielke did not secure the consent of the Sheriff before making a statement to the public while on duty and in his official position. Specifically on December 11, 2015, from approximately 1300 hours to 1345 hours, Sergeant Mielke, while on duty and wearing a PSO issued uniform, delivered a eulogy for a private citizen at a Celebration of Life Ceremony at the Hodges Family Funeral Home in Dade City, Florida. Sergeant Mielke did not request or secure the consent of the Sheriff prior to the listed speaking engagement. (Sustained - LOR) C) Allegation that Sergeant Michael Mielke engaged in conduct, on duty, which impaired public respect and confidence in the Sheriff's Office and its members. Specifically, On December 11, 2015, Sergeant Mielke, while on duty wearing a PSO issued uniform and driving a PSO vehicle, attending both the morning internment ceremony and the afternoon Celebration of Life ceremony of a convicted felon. Furthermore, Sergeant Mielke delivered the eulogy at the Celebration of Life ceremony at the request of the family of the deceased, during which time he spoke about the deceased from both his personal and law enforcement perspective. (Sustained - Suspension 2 days.) Annamarie 41070 Hernandez SI2016-021 SI Supervisor Investigation 02/27/2016 Corporal Annamarie Hernandez failed to report to an extra duty detail on February 27, 2016 at 1200 hours at the Medical Center of Trinity. (LOR - 90 days loss of off duty assignment) Charles 41347 Wynn SI2016-033 SI Supervisor Investigation 04/07/2016 Corporal Wynn violated G.O. 22.6 (Extra Duty & Off Duty Employment) which states: Members wishing to participate in off-duty employment must submit a Record of Off-Duty Employment Form (PSO 1-0005) to the appropriate commander, via the chain of command, with a brief description of the employment, the name and address of the employers, the hours to be worked and the duration of the job. (Sustained - LOR) Russell 42280 Knieriem IA2016-00035 I.A. Investigation 09/07/2016 On September 7th, 2016, a Major Crimes Detective arrested Deputy R. Knieriem for Stalking under PSO case number 16034258. Deputy Knieriem had been stalking the victim for the past several months. (Sustained - Dismissed) Joseph 42694 Koehler SI2016-066 SI Supervisor Investigation 10/03/2016 On October 3, 2016, Corporal Koehler allowed his son and his son's friend to wear his official Sheriff's Office uniform polo to school for the day. This occurred during homecoming week where students dress up as a "character" for the day. They dressed in a manner which resembled police officers (including a toy gun) from the move "21 Jump Street." The son's friend also wore the polo to a restaurant during school hours. Privately taken photographs of the students wearing Koehler's uniform polos were posted on social media (Facebook) by the friend's father. (Sustained 1 day suspension/reassignment) John 43063 Widener VC2016-181 Vehicle accident 08/31/2016 Cpl. Widener struck a vehicle from behind at the intersection of Eiland Blvd. and Fort King Road in ZHL. FL. He was heading south on Fort King Road and came to a red light at Eiland Blvd. He was making a right hand turn on green and briefly took his eyes off the road to move some papers on his passenger seat. He was going around 5 mph when he noticed the car in front of him stopped. He applied the brake but it was too late and he lightly hit the vehicle in front of him causing minor paint damage to the other vehicle. No one was hurt. (Sustained - LOC) Jonathan 40289 Cross VC2015-139 Vehicle accident 11/20/2015 On the listed date and time, SRO was driving north bound on I-75 preparing to exit at SR 54. Traffic ahead stopped abruptly. Cpl. Cross rear-ended a stopped truck in the north bound lane, which, in turn, struck another stopped vehicle. Cpl. Cross sustained abrasions and swelling to his left hand and left elbow from his airbag when it deployed. (Sustained - LOR and Driver Improvement Training Course) Joshua 42341 Peters VC2016-110 Vehicle accident 09/20/2016 On 09/20/16, Cpl. Peters was driving his agency issued vehicle eastbound on Orchid Lake Rd., in the area of Chickasaw Lane. A 2001 red Mazda (FL plate # ), was also heading eastbound on Orchid Lake Road in the area of Chickasaw Lane and was directly in front of Cpl. Peters' vehicle. The Mazda was stopped to turn northbound onto Chickasaw Lane. Due to the sun shining in Cpl. Peter's eyes, Cpl. Peters had difficulty seeing the Mazda had stopped in front of him and Cpl. Peter’s vehicle struck Mazda from behind. No injuries. Cpl. Peters was cited for Careless Driving with FHP citation number A40PYOE. (Sustained - LOR) 5 PASCO SHERIFF’S OFFICE PROFESSIONAL STANDARDS VIOLATION HISTORY 727-815-7117 Jessica 43064 Ziegler VC2016-182 Vehicle accident 12/06/2016 On 12/06/2016, Cpl. Ziegler was dropping off items to the FOP. She was circling the building when she struck a wooden light pole. The front driver side area of the vehicle struck the pole. The pole broke into pieces and fell to the ground. She was driving approx. five miles per hour and when she struck the object, the airbag did not deploy. There was approximately $100.00 damage. Florida Highway Patrol was not contacted as there was only minor damage to her vehicle and no injury occurred. (Sustained - POR) Milton 43473 Arroyo SI2017-007 SI Supervisor Investigation 01/09/2017 Cpl. Arroyo engaged in conduct which resulted in an adverse impact upon the operation of the Sheriff's Office, and which has the propensity to impair public respect and confidence in his abilities to carry out his duties. To wit: Cpl. Arroyo made a crude gesture, using the middle finger of his right hand, in front of several students in the cafeteria during a lunch period. This was a video captured by a student on a social media site called Snap Chat. Several parents later observed this, were upset, and believed this was inappropriate, and filed a complaint. (Sustained - LOC) Joseph 44107 Koehler SI2017-030 SI Supervisor Investigation 04/26/2017 It is alleged on 4/26/17 and 5/3/17 Cpl. Koehler drove his marked agency vehicle to his off duty employment job Per G.O. 22.6 Extra Duty and Off Duty Employment: "No member will utilize Sheriff's Office vehicles, uniforms, equipment or other resources for the purpose of engaging in off duty employment activities. In addition, the use of an agency vehicle by any member to commute to and from any off duty employment is strictly prohibited." Cpl. Koehler has permission from the agency for his off duty job; however, he must abide by the provisions of the General Order. A parent on the team took a photo of Cpl, Koehler's patrol vehicle at the game on 4/26/17 and emailed it to the Sheriff's Office. On 5/3/17 at approximately 1900 hours, I observed Cpl. Koehler's patrol vehicle parked at the game. (Sustained - LOR, 2 day suspension, reassignment) Milton 44627 Arroyo IA2017-022 I.A. Investigation 05/25/2017 It is alleged that Corporal Milton Arroyo, Fivay High School Resource Officer, used his agency issued laptop computer to access restricted information via the Drier and Vehicle Information Database (DAVID). Cpl. Arroyo is alleged to have conducted multiple search queries utilizing the DAVID system which are alleged to have been completed for Cpl. Arroyo's personal gain, and not related to any approved law enforcement incident, purpose, or investigation. During a subsequent criminal investigation, Cpl. Arroyo was arrested and charged for misusing the DAVID system. (Sustained - terminated while under investigation) Patrick 45129 Cottrell SI2017-062 SI Supervisor Investigation 06/17/2017 On 06/17/17 Deputy Patrick Cottrell responded to the Dollar General Store on US 301 in Dade City in reference to an alarm activation. Surveillance video from the location showed a suspect shatter the northernmost lower window pane of the bank of windows on the front of the store and later returned and entered the store to commit a burglary. During the investigation, Deputy Cottrell missed the obvious point of entry into the business when walking up to the front door to check if they were locked. (Sustained - LOR) Clifford 45570 Williams SI2017-077 SI Supervisor Investigation 09/07/2017 Cpl. C. Williams was issued a direct order given by a supervisor. The order and instruction was lawful and proper. To wit: Cpl. C. Williams was ordered to report for Patrol Relief Assignment due to Hurricane Irma on 09/07/17. He was assigned to District 2 Support for September 9 and 10 from 1400 to 0200 hrs. He received this order via email. He contacted me via agency phone and advised that he was refusing the order, he would not report to District 2, and that I can write him up if I have to but he will not be going over to District 2. (Sustained - LOR/2 day suspension) Joshua 47367 Peters SI2018-066 SI Supervisor Investigation 04/05/2018 On 4/5/18 Corporal Peters was notified at the end of the school day of an alleged Involuntary Sexual Battery that was committed on one of his Ridgewood High School students. Corporal Peters failed to follow up with the investigation which was forwarded to patrol units shortly after being reported. (Sustained - LOR) Bryan 46857 Mobley VC2018-061 Vehicle accident 04/05/2018 At approximately 0625, Corporal Mobley was backing his agency issued patrol vehicle down his driveway at his residence. Cpl. Mobley did not see his wife's personally owned vehicle at the end of the driveway and backed into it. This was caused by a combination of darkness, fog, and the fact the vehicle was not parked in its normal location. The private vehicle was not occupied at the time of the incident. (Sustained - LOC) Jessica 47131 Ziegler IA2018-019 I.A. Investigation 06/13/2018 A) On 04/21/2018 at a public event, Corporal J. Ziegler was informed of an alleged sexual battery involving juveniles. During a subsequent telephone call on 04/22/2018, Corporal J. Ziegler was again made aware of the sexual battery allegation. As of 04/26/2018, Corporal J. Ziegler had not made contact with the alleged victim, contact with the Abuse Hotline, pulled a case number, call or a patrol deputy to initiate a report, or notify her immediate supervisor of the allegation and pending investigation. (Sustained - LOR) B) On 06/13/2018, during Corporal J. Ziegler's subject member interview, she claimed to have spoken to Sergeant J. Dill once on 04/27/2018. Corporal J. Ziegler's statement was in direct contradiction to Sergeant J. Dill's Professional Standards Memorandum, dated 04/27/2018. After Corporal J. Ziegler's interview, she provided a screen shot of her personal cellular telephone and Verizon billing statement for 04/27/2018. A review of the additional documentation revealed there were two conversations between Corporal J. Ziegler and Sergeant J. Dill on 04/27/2018. (Unsubstantiated) Scott 47609 Mitchell VC2018-109 Vehicle accident 06/25/2018 Corporal Scott Mitchell was in the parking lot of the Seven Eleven conducting a "show up" in reference to an aggravated assault. Corporal Mitchell was driving west in the parking lot in heavy rain when he maneuvered his vehicle to conduct the show up. Corporal Mitchell struck a metal pole filled with concrete which was affixed to the ground. Corporal Mitchell was driving patrol vehicle which sustained damage to the front passenger side of the vehicle. FHP responded and investigated the crash. (Sustained - LOC/Driver Improvement Training Course) Jennifer 48414 Rongo SI2018-090 SI Supervisor Investigation 02/01/2018 Cpl. Rongo engaged in conduct which resulted in an adverse impact upon the operation of the Sheriff's Office and which has the propensity to impair public respect and confidence in her abilities to carry out her duties. To wit: Cpl. Rongo used her position as an SRO to cultivate an unprofessional and personal relationship with a student who is a member of the softball team she coaches. Cpl. Rongo has repeatedly had the student in her office causing the student to miss instructional time. Cpl. Rongo signed passes to excuse tardies, called the classroom having the student sent to her office, and being behind closed doors with the window blind up so people passing her office can't see into the office. Cpl. Rongo continued this practice after being told not to by the Principal and Supervisor. She later challenged the principal questioning her as to why she had a student in her office. (Sustained - LOR) Daniel 44768 Mills VC2017-132 Vehicle accident 05/31/2017 Cpl. Mills was operating vehicle 2666 southbound on US Highway 41 approaching State Road 54 in Land O Lakes, Florida. Cpl. Mills was on the phone with the IT section due to an AVL issue. Cpl. Mills advised me he checked to make sure the wire for the AVL was plugged in and took his eyes off the road. Vehicle 2666 struck the load protruding out of a light blue Toyota Tacoma with Florida tag. This was a low impact crash with no reported injuries. Vehicle 2666 sustained damage to the hood area. (Sustained - LOC) Scott 48223 Mitchell IA2018-030 I.A. Investigation 08/17/2018 On August 17, 2018 at approximately 0939 hours, Cpl. Scott Mitchell neglected to take action in response to Deputy Wilkes' radio traffic that he was fighting with a student from Cypress Creek High School and needed back up. Cpl. Mitchell knew that the student Deputy Wilkes was fighting with had just walked off his campus a few minutes prior. Cpl. Mitchell heard the radio traffic from Sgt. Tedeschi about the "juvenile disturbance just outside your school," and a deputy was involved in a fight in need of backup. Cpl. Mitchell did not respond to the deputy in need, rather he announced over the radio, "not allowed to leave the property sir." Cpl. Mitchell's deliberate failure to take action endangered the safety of Deputy Wilkes who as in an active fight with someone a short distance away. (Sustained - LOC) William 50017 Gulley SI2019-013 SI Supervisor Investigation 01/14/2019 Cpl. William Gulley failed to Take Suitable Action: to wit: on 1/14/19, Cpl. Gulley was in a classroom assisting staff with a violent student. Cpl. Gulley became involved in physically restraining the student and justifiably used force. Cpl. Gulley failed to activate his body worn camera (BWC) when responding to the call for assistance. He then failed to document the incident and use of force on an official PSO report. Cpl. Mills did not work the detail, causing the detail to go unworked. At 1400 the same day, Cpl. Mills sent me a text message advising me of the unworked detail. Investigation 04/30/2019 Commission of a Misdemeanor: On April 30th, 2019, Corporal Jonathan Cross was at his scheduled work assignment as the School Resource Officer at Thomas E. Weightman Middle School. At approximately 1139 hours, Corporal Cross was assisting children in the school's cafeteria. Video surveillance captured Corporal Cross mishandle his agency issued firearm (Sig Sauer P320- serial number #58A188786), which caused the firearm to discharge. A certified Sig Sauer P-320 Armorer inspected and tested the listed firearm and Corporal Cross' holster (Safariland 6360). The certified Armorer found no defects to either piece of equipment. Major Crimes investigated the incident (Culpable Negligence) and referred the case to the State Attorney's Office (see report #19017921 for details). Video surveillance captured him mishandle his agency issued firearm, which caused an accidental discharge. The projective entered and exited his uniform pants leg and struck the wall behind him; bullet fragments were located on the ground adjacent to the lunch line where students were standing at the time of the discharge. Cpl Cross, students and/or staff were not injured during this incident. Cpl. Cross' actions created a safety issue at the school and caused an adverse impact upon the Sheriff's Office due to the media/community attention the incident generated. Citizens posted over 830 comments (positive and negative) on the Sheriff's Office Facebook page referencing this incident. (Sustained- Dismissal) Dustin 51704 Roy VC2019-132 Vehicle accident 08/07/2019 Damage to MV/Avoidable: Sgt. Roy was operating agency vehicle 3448 and was eastbound on SR 52, turning north on Shady Hills. While completing the turn, Sgt. Roy's vehicle was struck on the passenger side by a westbound Nissan Altima. Airbags deployed in both vehicles and Sgt. Roy was struck in the fact by his laptop computer. Both drivers had only minor injuries. FHP issued Sgt. Roy a UTC for violation of right way.
